چکیده
مقدمه: ویروس سیتومگال انسانی (Human cytomegalovirus یا HCMV) یکی از ویروسهای خانوادهی هرپس میباشد. تشخیص به موقع این ویروس از اهمیت بسیار زیادی در افراد دریافتکنندهی پیوند کلیه برخوردار است. علت این امر، وسعت تظاهرات بالینی آلودگی به این ویروس در این افراد، از پس زدن پیوند تا حتی مرگ، میباشد. هدف این پژوهش بررسی دقت دو روش رایج (Real-time Polymerase chain reaction) Real-time PCR و ایمونوفلورسانس غیر مستقیم (pp65 antigen assay) در دریافتکنندگان پیوند کلیه برای شناسایی HCMV بود. روشها: در این پژوهش خون کامل 135 نفر از دریافتکنندگان پیوند کلیه در شهر اصفهان طبق پروتکل کیت کیاژن (Qiagen DNA mini kit) استخراج گردید. برای تشخیص HCMV از دو روش Real-time PCR و ایمنوفلورسانس غیر مستقیم (pp65 antigen assay) استفاده شد. روش اول بر پایهی روبش کاوشگر TaqMan و روش دوم بر پایهی شناسایی آنتیژن pp65 به روش ایمنوفلورسانس غیر مستقیم استوار بودند. یافتهها: تعداد افرادی که نتیجهی آزمایش آنها در روش Real-time PCR مثبت گزارش شد 36 نفر (27 درصد) بود، در حالی که، این تعداد در روش pp65 antigen assay در مجموع 29 مورد (21 درصد) بود. همچنین مشخص شد که، بیشینهی فراوانی عفونت فعال در فصل بهار (28 درصد) و کمینهی فراوانی آن متعلق به فصل زمستان (16 درصد) بود؛ اما، رابطهی معنیداری بین فراوانی عفونت HCMV و فصل مشاهده نشد. نتیجهگیری: یافتههای این مطالعه نشان داد که روش Real-time PCR با دقت بیشتری قادر به تشخیص عفونت HCMV در افراد دریافتکنندهی پیوند کلیه میباشد. همچنین مشخص شد که بیشترین فراوانی عفونت HCMV چه به صورت نهفته و چه به صورت فعال، متعلق به فصل بهار است که البته این ارتباط معنیدار نبود. واژگان کلیدی: ویروس سیتومگال انسانی، پیوند کلیه، ایمونوفلورسانس غیر مستقیم، Real-time PCR

چکیده English
Background: Human cytomegalovirus (HCMV) is a member of herpes virus family and its early diagnosis could be very critical in kidney transplant patients. The clinical manifestations of HCMV infection include a wide range from tissue rejection to death. The aim of this study was to compare the accuracy of two common methods of HCMV monitoring, i.e. real-time polymerase chain reaction (PCR) and pp65 antigen assay. Methods: We extracted the genomic DNA of 135 kidney transplant patients from their whole blood. HCMV monitoring was performed using TaqMan scanning probes and pp65 surface antigen detection by real-time PCR and pp65 antigen assay, respectively. Findings: According to our results, real-time PCR and pp65 antigen assay showed positive results in 36 (27%) and 29 patients (21%), respectively. In addition, the maximum and minimum rates of infection with HCMV were detected in spring (28%) and winter (16%), respectively. However, no significant association was established between HCMV infection and seasonal patterns. Conclusion: Our results indicated that real-time PCR was more accurate than pp65 antigen assay to detect HCMV infection in kidney transplant patients. We could also show that the maximum rate of infections with HCMV belonged to spring. However, the rates in the 4 seasons were not significantly different.
