رابطه‌‌ی‌ بین ادراک بیماری، متغیرهای شخصیتی و پیروی از درمان در بیماران سکته‌ی قلبی

چکیده

مقدمه: اگر چه مطالعات نقش عوامل روان‌شناختی در پیروی از درمان و توصیه‌های درمانی را مورد تأیید قرار داده است، اما جنبه‌های مختلف نقش ادراک بیماری در پیروی از درمان به طور کامل روشن نیست. مطالعه‌ی حاضر، به منظور بررسی روابط بین متغیرهای ویژگی‌های شخصیتی، ادراک از بیماری و پیروی از درمان و همچنین، بررسی نقش واسطه‌ای ادراک از بیماری بین ویژگی‌های شخصیتی و پیروی از درمان، انجام گرفت.روش‌ها: پژوهش حاضر، یک مطالعه‌ی مقطعی و از نوع همبستگی می‌باشد. در این مطالعه، 87 نفر از بیماران مبتلا به سکته‌ی قلبی حاد که در سال 1394 جهت پی‌گیری و ادامه‌ی درمان به بیمارستان‌های تخصصی قلب اصفهان مراجعه نمودند، به روش نمونه‌گیری پی در پی و بر اساس معیارهای ورود و خروج انتخاب گردیدند. پرسش‌نامه‌ی اطلاعات دموگرافیک، پرسش‌نامه‌ی کوتاه شده‌ی ادراک بیماری، پرسش‌نامه‌ی پنج عاملی ویژگی شخصیتی NEO و پرسش‌نامه‌ی پیروی عمومی و اختصاصی از درمان توسط بیماران تکمیل گردید. سپس، داده‌ها با استفاده از تحلیل مسیر مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: تحلیل مسیر داده‌ها ارتباط معنی‌دار بین ویژگی مسؤولیت‌پذیری (263/0 = r)، روان‌نژندخویی (214/0- = r)، برون‌گرایی (245/0 = r) و گشودگی (167/0 = r) با پیروی عمومی از درمان را نشان داد. همچنین، مشاهده گردید که بین ویژگی برون‌گرایی (309/0 = r) با پیروی اختصاصی از درمان رابطه‌ی معنی‌داری وجود دارد. همچنین، یافته‌ها نقش واسطه‌ای ادراک از بیماری را بین ویژگی برون‌گرایی با پیروی عمومی (154/0 = r) و اختصاصی (139/0 = r) از درمان به طور معنی‌داری نشان داد.نتیجه‌گیری: به نظر می‌رسد علاوه بر این که ادراک بیماران از بیماری می‌تواند بر پیروی از درمان تأثیر مستقیمی داشته باشد؛ همچنین، می‌تواند از طریق برخی ویژگی‌های شخصیتی، اثر غیر مستقیمی بر روی پیروی از درمان اعمال کند. از این رو، پیشنهاد می‌گردد در مداخلات روان‌شناختی، به منظور ارتقای پذیرش درمان بیماران، پدیده‌ی ادراک از بیماری مورد توجه ویژه قرار گیرد.

The Relationship between Illness Perception, Personality Variables, and Adherence among the Patients with Myocardial Infarction

چکیده [English]

Background: Previous studies has proved that psychological factors could affect treatment adherence, in spite of this, the role of illness perception in treatment adherence is unclear. Current study was conducted in order to investigate the relations between personality characters and treatment adherence and also the role of the illness perception as mediating factor between these two.Methods: In this cross-sectional correlation study, 87 in-patients with acute myocardial infarction, who was admitted to cardiology wards in hospitals in Isfahan City, Iran, were enrolled into this study using consecutive selection method. Questionnaire of demographics information and brief-illness perception, NEO five-factor inventory and general and specific treatment adherence were completed for the study group, and was analyzed using path analysis.Findings: Analysis of data path showed a significant relation between conscientiousness (r = 0.263), neuroticism (r = -0.214), extraversion (r = 0.245), and openness to experience (r = 0.167) with general treatment adherence, as well as the extraversion (r = 0.309) with specific treatment adherence. Moreover, with considering the mediating role of illness perception, it showed that extraversion had strong and meaningful relation with general (r = 0.154) and specific (r = 0.139) treatment adherence.Conclusion: It seems that illness perception as mediating factor could affect the relation of personality characters and treatment adherence in patients with acute myocardial infarction. So in psychological interventions, a special attention should be given to strengthen and reform of illness perception that could improve compliance among the patients.
