METHODS: A randomized, double-blind, placebo controlled clinical trial was performed on 18-45 years old schizophrenic patients. Baseline information including vital signs, height, weight, smoking status, demographic characteristics, (past) psychiatric history, medication history and medication-related adverse effects was collected. Patients were randomly assigned to topiramate or placebo group. Efficacy was assessed by administering positive and negative syndrome scale (PANSS), and tolerability was recorded in both groups on days 0 (baseline), 28, and 56. FINDINGS: Total PANSS score in topiramate group was 96.87 (85.37-108.37), 85.68 (74.67-96.70) and 76.87 (66.06-87.69) compared with 101.87 (90.37-113.37), 100.31 (89.29-111.32) and 100.56 (89.74-111.37) in placebo group in baseline, 28th and 56th days, respectively (95% confidence interval). General linear Model for repeated measure analysis showed that topiramate has lowered PANSS score significantly (p<0.05). Significant decline pattern was also found in all three PANSS components (negative, positive and psychopathology symptoms) (p<0.05). CONCLUSION: Topiramate can be an effective medication in controlling schizophrenic symptoms, because of its effect on decreasing negative symptoms and controlling antipsychotic-associated weight gain. KEY WORDS: Topiramate, schizophrenia, positive and negative syndrome scale, clinical trial}, keywords = {}, title_fa = {افزودن توپیرامات به درمان اسکیزوفرنی: کارآزمایی بالینی دوسوکور}, abstract_fa = {مقدمه: به کارگیری آنتاگونیست‌های گلوتامات، مانند توپیرامات، بر اساس تئوری گلوتامات در اسکیزوفرنی پیشنهاد شده است؛ خواص این دارو نیز خود مشوق مطالعه و تحقیق در زمینه‌ی استفاده از آن برای درمان اسکیزوفرنی می‌باشد. روش ها: این کارآزمایی بالینی، دوسوکور و تصادفی با گروه کنترل دارونما، بر روی بیماران 18 تا 45 ساله اسکیزوفرن تحت درمان با کلوزاپین (حداقل 100میلی‌گرم) انجام شد. ابتدا، اطلاعات پایه شامل علائم حیاتی، قد، وزن، وضعیت مصرف سیگار، مشخصات دموگرافیک، سوابق روان‌پزشکی، سابقه‌ی درمان‌های طبی و عوارض جانبی دارویی ثبت گردید. سپس بیماران به شکل تصادفی به دو گروه تقسیم شدند و توپیرامات (25 تا 300 میلی‌گرم در روز) و دارو‌نما به درمان آن‌ها اضافه شد. اثر بخشی درمان با کمک مقیاس علائم مثبت و منفی (PANSS)، تحمل و عوارض داروها در روزهای صفر (آغاز)، 28 و 56 بررسی گردید. یافته ها: امتیازهای کلی PANSS در گروه توپیرامات 87/96 (37/108-37/85)، 68/85 (70/96-67/74) و 87/76 (69/87-06/66) در مقایسه با گروه دارونما 87/101 (37/113-37/90)، 31/100 (32/11-29/89) و 56/100 (37/111-74/89) به ترتیب در روزهای صفر، 28 و 56 (با فاصله اطمینان 95%) بود. آنالیز مدل خطی کلی (GLM) نشان داد در بیماران گروه توپیرامات کاهش قابل توجه امتیاز در تمام اجزای علائم مثبت، علائم منفی و سایکوپاتولوژی کلی (PANSS) مشهود است. نتیجه گیری: توپیرامات، به خصوص با توجه به تأثیر آن بر روی علائم منفی و کنترل افزایش وزن ناشی از مصرف داروهای آنتی سایکوتیک، می‌تواند به عنوان یک درمان مؤثر کمکی در کنترل علائم اسکیزوفرنی مطرح باشد. واژگان کلیدی: توپیرامات، اسکیزوفرنی، علائم مثبت، علائم منفی و سایکوپاتولوژی کلی، کارآزمایی بالینی.}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_12918.html}, eprint = {https://jims.mui.ac.ir/article_12918_9f0537a43369c7667393833cc45ada14.pdf} }
