The clinical symptoms and radiologic findings of urinary TB are nonspecific. Current diagnostic tests are of low sensitivity and labor-intensive. Therefore, this study was aimed to evaluate diagnostic value of urine PCR in genitourinary tuberculosis (GNTB). METHODS: This was a descriptive study on 33 patients with confirmed genitourinary TB. Demographic data, clinical symptoms, laboratory and radiologic findings were collected. For each patient, three consecutive early morning urine specimens were examined by PCR. The diagnostic value of PCR in mycobacterium tuberculosis (MTB) in comparison with standard microbiological methods was assessed. FINDINGS: There were 33 patients with a mean age of 47.27 16.1 years. The most common presenting symptoms were irritative voiding symptoms (51.5%), flanks pain (27.2%), gross hematuria (9%) and suprapubic pain (9%). Laboratory findings in U/A were hematuria (75.8%) and pyuria (60.6%). IVU was abnormal in 61.5% of patients. Most common abnormalities were pyelocalyceal dilation (44%), ureteral stricture and hydroureter (37%) and multiple small calyceal deformities (25%). Of the 33 patients PCR for MTB was positive in 16 cases (48.5%). In patients with abnormal IVU, PCR was positive in 62.5%. CONCLUSION: A high index of clinical suspicion is necessary for diagnosis of GUTB. PCR is recommended for instant diagnosis and screening before further examination, it cannot be the only method in identification of GUTB. KEY WORDS: Contrast induced nephropathy, percutaneous coronary intervention}, keywords = {}, title_fa = {نقش تست Polymerase Chain Reaction ادرار در تشخیص سل دستگاه ادراری - تناسلی}, abstract_fa = {مقدمه: دستگاه ادراری- تناسلی یکی از محل‌های شایع درگیری در سل خارج ریوی است. تظاهرات و علائم آزمایشگاهی و رادیولوژیک سل دستگاه ادراری- تناسلی (Genitourinary Tuberculosis یا GUTB) اغلب غیراختصاصی و تأخیری هستند. تست‌های موجود تشخیصی نیز از حساسیت پائینی برخوردار می‌باشند و یا زمان زیادی می‌برند. بر این مبنا مطالعه‌ی حاضر نقش تست (Polymerase chain reaction یا PCR) در تشخیص این بیماری را مورد بررسی قرار داده است روش ها: در یک مطالعه‌ی توصیفی، 33 نفر از بیماران با تشخیص قطعی سل دستگاه ادراری– تناسلی انتخاب شدند. اطلاعات دموگرافیک، علائم بالینی و آزمایشگاهی و یافته‌های رادیولوژیک بیماران جمع‌آوری شد. قبل از شروع درمان ضد سل نیز آزمایش PCR در سه نوبت روی نمونه‌ی ادرار بیماران انجام و ارزش تشخیصی آن با متدهای استاندارد رایج مقایسه شد. یافته ها: میانگین سنی بیماران 1/16±27/47 سال بود. شایع‌ترین تظاهر بیماری، علائم تحریکی ادراری در 5/51% بیماران و پس از آن درد پهلوها (2/27%)، هماچوری گروس (9%‌) و درد سوپراپوبیک (9%) بود. در آزمایش (urinalysis) UA، 75/8% از بیماران هماچوری و 6/60% پیوری داشتند. Intravenous urography (IVU) در 5/61% بیماران یافته‌های غیرطبیعی داشت که شایع‌ترین یافته‌ها اتساع سیستم پیلوکالیس (44%)، تنگی حالب و هیدرویورتر (37%) و دفرمیتی‌های کوچک و متعدد کالیس‌ها (25%) بود. در 16 بیمار نتیجه‌ی تست PCR مثبت شد (5/48%)؛ اما حساسیت تست در بیمارانی که IVU غیرطبیعی داشتند، بیشتر بود و به 5/62%‌ می‌رسید. نتیجه گیری: تست PCR برای جستجوی مایکوباکتریوم توبرکلوزیس در ادرار را باید به عنوان یک روش کمکی در تشخیص GUTB در کنار سایر روش‌های تشخیصی در نظر داشت؛ ولی به‌عنوان تنها روش تشخیصی توصیه نمی‌شود. واژگان کلیدی: سل، سل دستگاه ادراری- تناسلی، PCR، مایکوباکتریوم توبرکلوزیس}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_12920.html}, eprint = {https://jims.mui.ac.ir/article_12920_6f34ed9119c852298bacfc44278a4f8d.pdf} }
