Lymphadenitis is the most frequent form of extra pulmonary tuberculosis. Breast tuberculosis is an extremely rare form of extra pulmonary tuberculosis. We introduced a case of breast tuberculosis that presented with axillary sinus tract, left supraclavicular lymph node and left breast edema. Case Report: A 15 year-old girl with a history of left breast swelling and left axillary sinus tract discharge from 2 months ago was admitted to our hospital. The physical examination showed left supraclavicular lymphadenopathy (size=1×0.5, fixed, firm with mild tenderness), left breast mass (size=2×2, firm, mild tenderness) with normal overlying skin, left axillary sinus tract with large mass (size=8×8, tender, erythematous and firm). She has also edema of the chest wall especially in the left hemi thorax. Chest X-Ray was normal and ultrasonographic study of breast showed an ovaloid mass (13×29) with cystic structure and multiple septations. Abdominopelvic sonography was normal and PPD was negative. Excisional biopsy of left axillary lesion showed granuloma and caseous necrosis compatible with tuberculosis. Within 2 weeks of anti-TB medication, the patient developed with left breast discharge. After 6 weeks of anti-TB therapy discharge from this site and left axillary sinus tract was stopped. Conclusion: The tuberculosis of breast is a disease of younger age group and the younger patients usually manifests sign of a pyogenic breast abscess. In our case left breast swelling and left axillary sinus tract discharge were the first presentation of disease. Key words: Breast Tuberculosis, Edema, Axillary sinus tract.  }, keywords = {}, title_fa = {سل پستان: گزارش مورد}, abstract_fa = {  مقدمه:   سل پستان شکل بسیار نادر از انواع سل خارج ریوی است. سالانه تنها پانصد مورد سل پستان در سراسر جهان گزارش می‌شود. در این مقاله یک مورد از سل پستان که با علایم سینوس تراکت زیر بغل، غده‌ی لنفاوی فوق ترقوه‌‌ای چپ و ادم پستان چپ مراجعه کرده بود، معرفی شده است. معرفی بیمار: بیمار دختر 15 ساله‌ای بودکه با تورم پستان چپ و ترشح زیر بغل چپ از دو ماه قبل، مراجعه نمود. در معاینه‌ی غده‌ی لنفاوی فوق ترقوه‌ای، توده‌ی پستان چپ با پوست طبیعی قابل لمس، همراه با سینوس تراکت وسیع با توده‌ی زیر بغل چپ و ادم دیواره‌ی قفسه سینه به خصوص سمت چپ بود. سونوگرافی پستان یک توده‌ی تخم مرغی شکل با ساختمان کیستیک و دیواره‌های متعدد را نشان داد. بر اساس یافته‌های بالینی آنتی‌بیوتیک شروع شد، ولی به علت منفی بودن آزمایش‌های باکتریولوژیک و نبودن یافته‌ای دال بر اکتینومایکوز و یا باسیل اسید فاست، نمونه‌برداری از ضایعه زیر بغل انجام شد. گزارش آسیب‌شناسی، گرانولوم با نکروز پنیری مطابق با سل بود. درمان ضد سل شروع و ترشح بعد از شش هفته متوقف شد. تورم غده‌ی لنفاوی فوق ترقوه‌ای سمت چپ و پستان فروکش کرد. توده‌ی پستان و زیر بغل چپ نیز تحلیل رفت. نتیجه‌گیری: پستان مانند عضله‌ی اسکلتی و طحال نسبت به سل مقاوم است. سل پستان بیماری جوانان است و در افراد مسن‌ ممکن است با یک توده‌ی شبیه به کارسینوم تظاهر نماید. در حالی که بیماران جوان‌تر علایم آبسه چرکی پستان را نشان می‌دهد. واژگان کلیدی: سل پستان، ادم، سینوس تراکت زیر بغل.}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_12987.html}, eprint = {https://jims.mui.ac.ir/article_12987_aad7b7b318f4e816e0485a50d3db0e9b.pdf} }
