This study was designed and conducted to determine the prevalence of pancreatitis following scoliosis surgery and its associated risk factors.Methods:In a cross-sectional descriptive study, we enrolled 50 patients underwent surgery for correction of scoliosis at two hospitals in Tehran, Iran, between November 2006 and June 2007. Levels of serum amylase and lipase were measured before and two days after surgery. Duration of fasting after surgery, clinical signs of pancreatitis, duration of hospital stay, length of surgery, original curvature of the spine and degree of correction after surgery, and demographic data were recorded. The diagnosis of subclinical or biochemical pancreatitis was established with trebling of serum amylase and lipase.Findings:Twenty-seven of patients (54%) developed postoperative pancreatitis following scoliosis surgery. Sex, type of scoliosis, type of surgery, correction of Cobb's angle, and length of surgery were not found to correlate with the incidence of pancreatitis, whereas a significant relationship was found between the volume of hemorrhage and the incidence of pancreatitis (p < 0.05).Conclusion:The higher prevalence of pancreatitis in this study (54%) may be due to pancreatic ischemia. Patients are prone to ischemia owing to massive intraoperative bleeding and hypotensive anesthesia directed to reduce it. In this study patients developed pancreatitis were found to have lost more blood during surgery. This study demonstrated that pancreatitis can develop as a complication of the single-stage or two-stage surgery for correction of scoliosis and can dramatically affect patient outcome; hence we recommend that patients undergoing scoliosis surgery be monitored for increase in serum amylase and lipase, as well as any suggestive signs of clinical pancreatitis.Key words:Pancreatitis, Scoliosis}, keywords = {}, title_fa = {پانکراتیت‌های بالینی و تحت‌بالینی متعاقب اعمال جراحی اسکولیوزیس و عوامل خطر آن}, abstract_fa = {مقدمه:عارضه‌ی پانکراتیت علاوه بر اعمال جراحی مجاری صفراوی، پانکراس و بافت‌های مجاور، به دنبال عمل جراحی اسکولیوزیس نیز مشاهده و شیوع آن 19-16% گزارش شده است. این پژوهش با هدف تعیین شیوع پانکراتیت به دنبال عمل اسکولیوز و عوامل خطر احتمالی بروز آن طراحی و انجام شده است.روش‌ها:در این پژوهش مقطعی تحلیلی، تمامی 50 بیمار مبتلا به اسکلیوزیس جراحی شده در دو بیمارستان تهران (از آذر ماه 1385 تا تیر ماه 1386) بررسی شدند. در همه‌ی بیماران، پیش از عمل و روز دوم پس از آن، آمیلاز و لییپاز سرم اندازه‌گیری ‌شد. اضافه بر این، علایم بالینی پانکراتیت، مدت زمان ناشتا بودن پس از عمل، مدت زمان بستری در بیمارستان، مدت زمان انجام عمل، اندازه‌ی اولیه‌ی انحنای ستون فقرات و اندازه‌ی اصلاح آن پس از عمل و اطلاعات دموگرافیک بیمار نیز جمع‌آوری گردید. درصورت افزایش سه برابر آنزیم‌های آمیلاز و لیپاز سرمی پانکراتیت تحت بالینی و یا بیوشیمیایی در نظر گرفته شد.یافته‌ها:بیست ‌و ‌هفت بیمار (54 درصد) در پی عمل جراحی اسکولیوزیس دچار پانکراتیت شدند. بین سن، جنس، نوع اسکولیوز، نوع عمل جراحی، تصحیح زاویه‌ی Cobb و مدت عمل جراحی با بروز پانکراتیت ارتباطی مشاهده نشد در حالی‌که بین حجم خون‌ریزی با بروز پانکراتیت ارتباط معنی‌دار بود (P < 0.05).نتیجه‌گیری:در پژوهش اخیر شیوع پانکراتیت بیشتر از پژوهش‌های پیشین بود (54 درصد) که می‌تواند به علت ایسکمی پانکراس باشد. همچنین، مبتلایان به پانکراتیت در مقایسه با افراد غیرمبتلا، در حین عمل خون‌ریزی بیشتری داشتند؛ به همین دلیل و نیز هیپوتانسیو آناستازی که برای حفظ فشارخون در حین عمل انجام می‌گیرد، این بیماران برای ابتلا به ایسکمی مستعد می‌شوند. این پژوهش نشان‌داد که بروز پانکراتیت می‌تواند به ‌عنوان عارضه‌ی عمل جراحی یک و یا دو مرحله‌ای اسکلیوز مطرح باشد و از آن جا که ممکن است بر روی بیمار اثرات قابل‌توجهی داشته باشد، مانیتورینگ بیمار پس از عمل جراحی برای آمیلاز و لیپاز و نیز علایم بالینی پانکراتیت باید برای همه‌ی بیماران با هرگونه علایم هشدار پانکراتیت بالینی انجام پذیرد.واژگان کلیدی:پانکراتیت، اسکولیوز}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13008.html}, eprint = {https://jims.mui.ac.ir/article_13008_aa0878bfd6910a853b17d2e582ee02d0.pdf} }
