Amyloidosis is the results of fibril formation due to plasma cell dyscrasia or rarely monoclonal gammopathy of uncertain significance (MGUS). Clinical manifestations are proteinuria, cardiomyopathy, peripheral neuropathy, large tongue and shoulder fat pad enlargement and hepatosplenomegaly. Methods: The presented patient is a 42-year old woman with clinical features of a dull epigastric and left upper abdominal pain from one year ago. Her family history and past medical history was normal except for an exertional dyspnea. In the physical examination, paleness, large tongue with indentations around it and hepatosplenomegaly were obvious, during admission, she had rectorrhagia, polipectomy led to severe hemorrhage. Echocardiography performed and revealed the typical sign of sparkling pattern with restrictive cardiomyopathy. Findings: In bone marrow evaluation plasma cells were 8%. Serum protein electrophoresis showed that monoclonal gammopathy near 2 grams in weights and all data confirm monoclonal gammopathy of uncertain significance. Duodenal biopsy verified an amyloid deposition which was confirmed by Congo red coloration. There were no data to support secondary amyloidosis in our wide investigation and patient was treated with the diagnosis of primary amyloidosis due to monoclonal gammopathy of uncertain significance. Conclusion: Primary amyloidosis secondary to monoclonal gammopathy of uncertain significance is a rare disease which should be mentioned in chronic multiorgan disorders. Key words:: Primary amyloidosis, Monoclonal gammopathies of uncertain significance, Hepatomegaly.}, keywords = {}, title_fa = {گزارش یک مورد آمیلوئیدوز اوّلیه به دنبال گاماپاتی تک دودمانی با اهمیت نامشخص (MGUS)}, abstract_fa = {مقدمه: شایع‌ترین شکل آمیلوئیدوز سیستمیک، نوع اولیه است که از علل نادر آن گاماپاتی منوکلونال با اهمیت نامشخص(MGUS) می‌باشد. تظاهرات بالینی به‌ صورت ابتلای کلیه، قلب، دستگاه گوارش، بزرگی زبان، سندرم تونل کارپال و تجمع چربی در قدام شانه است. معرفی مورد: بیمار خانم 42 ساله‌ای بود که با شکایت درد مبهم شکم و بزرگی کبد و طحال از یک سال قبل مورد بررسی قرار گرفته بود. در سیر بستری، به ‌علت پولیپ روده، دچار خونریزی گوارشی تحتانی شد که با برداشت پولیپ خونریزی گوارشی شدیدی رخ داد. زبان بزرگ و باقی ماندن جای دندان‌ها بر روی زبان و نمای جرقه‌های درخشان دراکوکاردیوگرافی دیده شد. در بررسی مغز استخوان 8 % پلاسماسل و در الکتروفورز پروتئین‌های سرم گاماپاتی تک دودمانی، در حد دو گرم یافت شد. در ادرار این یافته منفی، اما پروتئین بنز جونز مثبت بود و تشخیص MGUS مطرح شد. کلیه‌ی بررسی‌های تکمیلی جهت یافتن علل دیگر اولیه و ثانویه‌ی آمیلوئیدوز منفی بود. در بیوپسی دوازدهه با رنگ‌آمیزی قرمز کنگو رسوبات آمیلوئید رؤیت شد و بیمار با تشخیص آمیلوئیدوز اولیه به ‌دنبال MGUS درمان گردید. نتیجه گیری: آمیلوئیدوز اولیه و MGUS هر کدام به ‌تنهایی، به ‌خصوص در سن زیر 50 سال و همراهی این دو با هم نادر می‌باشد. با این وجود در ابتلای ارگان‌های متعدد باید آمیلوئیدوز را در نظر داشت و با بیوپسی بافتی و رنگ‌آمیزی قرمز کنگو اثبات کرد. واژگان کلیدی: آمیلوئیدوز اولیه، گاماپاتی منوکلونال با اهمیت نامشخص، هپاتومگالی}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13081.html}, eprint = {https://jims.mui.ac.ir/article_13081_65e15bff9dcb9af01e2d0c65319ac5e2.pdf} }
