The purpose of this study was to exam the relationship of the history of hypertension and illness representations among post-myocardial infarction patients. Methods: One hundred and seventy six consecutive patients following myocardial infarction admitted to the CCU wards were selected based on the inclusive and exclusive criteria. Data obtained by using demographic questionnaire, Brief Illness Perception questionnaire (B-IPQ) and Beck Depression Inventory-II. Their medical data were obtained from medical records and history of hypertension was confirmed by cardiologist based on information recorded, history of patient and taking medications for hypertension. Therefore, the patients were divided into two groups with and without previous hypertension. χ2 test, student’s t-test, MANCOVA and Covariance were used to analyze the data. Findings: 30.7% had history of hypertension. A significant difference was seen between two groups in B-IPQ scores (F = 2.501, P = 0.033). Also, it showed significant differences in two dimensions of controllability/ treatability (F = 5.45 P = 0.026) and consequences (F = 5.054, P = 0.026). However, there were no significant differences in identity (F = 1.262, P = 0.228) and timeline (F = 0.261, P = 0.549) between two groups. Conclusion: Generally, the findings confirmed that the post-myocardial infarction patients with history of hypertension explain their cardiac attack as a more adverse event than those without history of hypertension. Also having previous hypertension may be associated with patient`s perception of consequences and controllability/treatability of myocardial infarction. However, these findings did not support such link with perception of identity and timeline of the disease. Thus, the post MI patients with history of hypertension likely develop negative schema about some dimensions of cardiac attack. Key words: Hypertension, Illness perception, Identity, Consequences, Timeline, controllability, treatability.}, keywords = {}, title_fa = {بررسی رابطه‌ی سابقه‌ی فشار خون بالا و طرحواره شناختی بیماری در بیماران پس از سکته قلبی}, abstract_fa = {مقدمه: بر اساس نظریه‌ی لونتال، بیماران جسمی بازنمایی‌های طرحواره گونه از بیماری جسمی به وجود می‌آورند که می‌تواند جنبه‌های مختلف سازگاری آنان با بیماری جسمی را تحت تأثیر قرار دهد. هدف این مطالعه بررسی ارتباط سابقه‌ی فشار خون بالا و بازنمایی‌های بیمار در بیماران پس از سکته‌ی قلبی بود. روش ها: تعداد 176 نفر بیمار 32 تا 84 سال، که با تشخیص قطعی سکته قلبی در یکی از بیمارستان‌های مجهز به واحد مراقبت قلبی شهر اصفهان طی پنج ماه اول سال 1385 بستری شده بودند، به روش نمونه‌گیری در دسترس بر اساس معیارهای ورود و خروج انتخاب شدند. داده‌های حاصل از اجرای فرم کوتاه مقیاس ادراک بیماری و پرسشنامه‌ی اطلاعات طبی، جمعیت شناختی و پرسش‌نامه افسردگی بک-2 در این مطالعه مورد استفاده قرار گرفت. اطلاعات طبی بیمار از پرونده بیمار استخراج شد و سابقه‌ی فشار خون بالا از طریق استخراج اطلاعات، شرح حال، سابقه‌ی مصرف داروهای ضد فشار خون بالا و تأیید قطعی آن توسط پزشک معالج (کاردیولوژیست) بیمار انجام گرفت. بنابراین بیماران به دو گروه با سابقه‌ی فشار خون بالا و بدون چنین سابقه‌ای تقسیم شدند. در تحلیل داده از آزمون‌های 2‌χ، t و روش تحلیل کواریانس چند متغیری و تحلیل کواریانس استفاده گردید. یافته ها: در مجموع 7/30 درصد از آزمودنی‌ها دارای سابقه فشار خون بالا بودند. یافته‌های تحلیل کواریانس چند متغیری تفاوت معنی‌داری بین دو گروه در نمرات ادراک بیماری (501/2F = ، 033/0 = P) نشان داد. همچنین نتایج تحلیل کواریانس تفاوت معنی‌داری بین دو گروه در دو بعد کنترل پذیری/درمان پذیری (45/5 F=، 026/0 = P) و پیامدهای بیماری (054/5F= ، 026/0 = P) نشان داد. با این وجود در ابعاد ماهیت بیماری (462/1 F =، 228/0 = P) و مدت زمان بیماری (361/0 F =، 549/0 = P) تفاوت معنی‌داری بین دو گروه وجود نداشت . نتیجه گیری: به طور کلی یافته‌ها تأیید نمود که بیماران با سابقه‌ی فشار خون بالا پس از سکته‌ی قلبی در مقایسه با بیماران بدون سابقه‌ی فشار خون بالا تلقی ناگوارتری از حمله‌ی قلبی خویش دارند. همچنین نتایج نشان داد سابقه‌ی فشار خون بالا ممکن است با ادراک بیمار از پیامدهای بیماری و کنترل پذیری/درمان پذیری ارتباط داشته باشد. البته، یافته‌ها چنین ارتباطی را با ابعاد ماهیت بیماری و مدت زمان بیماری مورد تأیید قرار نداد. بنابراین احتمال دارد بیماران با سابقه‌ی فشار خون بالا پس از سکته‌ی قلبی طرحواره‌های شناختی منفی در مورد برخی ابعاد حمله‌ی قلبی پدید آورند. واژگان کلیدی: سابقه‌ی فشار خون بالا، بازنمایی شناختی بیماری، ماهیت بیماری، پیامدهای بیماری، مدت زمان بیماری، کنترل پذیری، درمان پذیری.}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13120.html}, eprint = {https://jims.mui.ac.ir/article_13120_5f0e54a36a4f2aa68da6dffea2bf9ae9.pdf} }
