The increasing trend of sepsis incidence and its complications may make it as the most prevalent cause of death in Intensive Care Units. In the late 80 decade, sepsis used to be the 13th cause of death in US and more than about ten billion dollars was spent annually for the cost of its therapy. Estimates reveal that more than 400,000 new cases of sepsis and 200,000 new cases of consequent septic shock were occurred annually in US in which 100,000 of them are unfortunately resulted to death. Many factors are considered for the increasing rate of sepsis and septic shock incidence in which immune system suppressive therapy, raising use of different vascular catheters, increase of life expectance and also increasing rate of resistant infections are more remarkable amongst them. Despite the proposing of new theories on pathogenesis and pathophysiology of sepsis and the introduction of new and potent antimicrobial and antifungal drugs, it seems that there is not sign of clinically significant decrease in the mortality of this syndrome. This mortality rate is still considered unpredictable and for example for gram negative sepsis within the range of 20 to 80%. Though this special clinical situation deserves to be considered and further investigated by scientist from a 360° general point of view. General practitioners whom are the main target population for this review article are not supposed to confront routinely the cases of septic shock and this misleading statement may keep them happily far from the facts mentioned above. The aim of this article is to review the current information on the definitions, etiology, pathogenesis, pathophysiology and pharmacotherapy of sepsis in a teaching and educational basis.}, keywords = {}, title_fa = {سپسیس: نشانگان یا بیماری؟}, abstract_fa = {خلاصه مقدمه: سپسیس، بعد از عفونت‌های ریه و بیماری ایدز، سومین علت مرگ به دلیل عفونت بوده، با در نظر گرفتن روند رو به تزاید بروز و مشکلات متعاقب آن (از جمله شوک سپتیک) شایع‌ترین علت مرگ در بخش‌های مراقبت ویژه است. سپسیس ناشی از باکتری‌های گرم منفی از جمله مهم‌ترین علل ایجاد سپسیس می‌باشد و در 20 سال اخیر روند ابتلا به آن رو به افزایش بوده است. سپسیس ناشی از باسیل‌های گرم منفی مرگ و میر بیشتری نسبت به باکتری‌های دیگر دارد ولی باکتری‌های گرم مثبت، بی‌هوازی‌ها، قارچ‌ها و حتی ویروس‌ها نیز می‌توانند در زمره‌ی علت‌شناسی‌های این بیماری قرار گیرند. تعاقب وقایع پاتوفیزیولوژیک ناشی از تداخل بین عامل بیماری‌زای مهاجم و میزبان انسانی هنوز از جمله مسائل متنوع و ‏پیچیده به شمار می‌رود و به نحو کامل روشن نشده است. به ویژه این نکته حائز اهمیت است که متأسفانه مشاهدات به عمل آمده ‏در مدل‌های حیوانی سپسیس به سختی در مورد مدل انسانی قابل انطباق می‌باشد.‏ بدشگون‌ترین اختلال متعاقب سپسیس گرم منفی، شوک است. به نظر می‌رسد کاهش فشار خون همراه با سپسیس با آزادی پپتیدهای مؤثر بر عروق مانند برادی‌کینین و سروتونین و آسیب وارده بر عروق با وخامت این بیماری در ارتباط باشد. شاه‏‎علامت همودینامیکی سپسیس، حالت هایپردینامیک با وجه مشخصه‌ی بالا بودن برون‌ده قلبی توأم با پایین بودن مقاومت عروق محیطی است‏. اهداف اصلی درمانی برای بیماران مبتلا به سپسیس شامل موارد پنج گانه‌ی تشخیص به موقع و بررسی نوع پاتوژن مهاجم، حذف سریع منبع عفونت، شروع درمان ضد میکروبی به صورت تهاجمی در مراحل اولیه، تدارک مناسب حمایت‌های قلبی عروقی و ریوی شامل احیاء با مایعات، استفاده از منقبض کننده‌های عروقی و تهویه‌ی مکانیکی و در نهایت در نظر داشتن درمان‌های حمایتی متابولیک می‌باشد. فرایند سپسیس، با در نظر گرفتن تعاریف اولیه‌ی ملاک‌های تشخیصی، نشانگانی سهل و ممتنع است و با وجود سادگی ‏ظاهری علت‌شناسی آن، هنوز حائز پیچیدگی‌های یک بیماری شناخته شده‌ نسبی است. ‏هدف از این مقاله، آموزشی مروری بر آخرین فرضیات و یافته‌های تشخیصی و درمانی مرتبط با سپسیس می‌باشد. واژگان کلیدی: سپسیس، پاتوفیزیولوژی، درمان‌شناسی.}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13124.html}, eprint = {https://jims.mui.ac.ir/article_13124_c18a8943cfdc5a4585023b9fce19c09e.pdf} }
