Over the past 30 years, physicians have begun to recognize many of the detrimental consequences of sleep disturbances produced by abnormal breathing patterns termed sleep-disordered breathing (SDB).SDB is not a newly discovered disease entity however in the past, it was not considered a serious health problem, and just during the recent decades researchers have declared its serious consequences and the condition has come into physician's attention. Researchers continue to develop an understanding of the risks created by and associated with sleep apnea syndrome (SAS). Associated risks include the following: Hypertension Stroke Ischemic heart disease Pulmonary hypertension Congestive heart failure Sexual malfunction Traffic accidents SDB and SAS are much more prevalent than older believes, in more conservative studies the prevalence had been reported at about 4 percent; in the more recent studies the rate has raised to 25-40% among adults 40 years of age or older. SDB refers to a wide spectrum of sleep-related breathing abnormalities; those related to increase upper airway resistance include snoring, upper airway resistance syndrome (UARS), and obstructive sleep apnea-hypopnea syndrome (OSAHS). Many authors regard SDB as a continuum of a spectrum of diseases ranging from simple snoring to Obstructive Sleep Apnea Syndrome (OSAS); this concept suggests that an individual who snores may be exhibiting the first manifestation of SDB and that snoring should not be viewed as normal. A patient can move gradually or rapidly through the continuum, from a less annoying to serious disabling condition. Early detection and appropriate management is essential and potentially life saving for individual patients. Nocturnal, laboratory-based polysomnography (PSG) is the most commonly used test in the diagnosis of obstructive sleep apnea syndrome (OSAS). It is often considered the criterion gold standard for diagnosing OSAS, determining the severity of the disease, and evaluating various other sleep disorders that can exist with or without OSAS. PSG consists of a simultaneous recording of multiple physiologic parameters related to sleep and wakefulness. PSG can directly monitor and quantify the number of respiratory events (i.e., obstructive, central, or complex) and the resultant hypoxemia and arousals related to the respiratory events or even independent of the respiratory events. A single-night PSG is usually adequate to determine if OSAS is present and the degree of the disorder. However, night-to-night variability may exist in patients have a high probability but a low apnea index. In addition, variability in laboratory equipment, scoring technique, and interscorer reliability may also play roles. Scoring by computer software is not reliable and a scorer has to recheck all of the events by him/herself to make a realistic report. PARAMETERS MONITORED Assessment of sleep stages requires 3 studies: to be monitored during a whole night sleep: Electroencephalography (EEG) Electrooculography (EOG), Surface electromyography (EMG). Other monitored parameters include: Airflow (nasal and/or oral) Electrocardiography Pulse oximetry Respiratory effort (thoracic and abdominal) Sound recordings to measure snoring Continuous video monitoring of body positions An experienced Sleep study physician can determine the presence of SDB and/or its severity, and suggest the most suitable management plan.}, keywords = {}, title_fa = {استفاده از پلی سومنوگرافی در سندرم آپنه انسدادی خواب}, abstract_fa = {نزدیک به یک سوم زندگی انسانه در خواب می ‌گذرد. در سی سال اخیر پزشکان با عواقب ناخوشایند اختلالات خواب (SDB) به ویژه اختلالات تنفسی در خواب آشنا شده ‌اند. اختلال تنفسی در خواب به ویژه نوع بارزتر آن یعنی وقفه تنفسی در خواب (SAS) یک بیماری تازه یا نویافته نیست ولی در گذشته یک مشکل بزرگ سلامتی تلقی نمی شد و تنها در دو سه دهه‌ی اخیر به کانون توجه پزشکان تبدیل شده، محققین روز به روز به اطلاعات جدیدی در مورد تهدیدهای سلامتی ناشی از این اختلال دست می یابند. عوارضی که در مبتلایان به اختلالات تنفسی خواب شایعتر از افراد دیگر است عبارتند از: افزایش فشار خون شریانی حوادث عروقی مغز بیماری‌ های عروق کرونری افزایش فشار شریان ریوی نارسایی قلب اختلال در تمایل یا توان جنسی تصادفات رانندگی اختلال یا وقفه‌ تنفسی در خواب بیماری‌های کمیابی نیستند، در خوش‌بینانه‌ترین مطالعات شیوع آنهارا در حدود 4% جمعیت ذکر می‌ نمودند و در مطالعات جدیدتر رقم شیوع به 25% و حتی 40% در جمعیت مسن‌ تر از 40 ساله ذکر شده است. هنگامی که از اختلالات تنفسی خواب گفتگو می ‌شود، طیف گسترده‌ای از بیماری‌ها از خرناس ساده، سندرم افزایش مقاومت مجاری تنفسی فوقانی تا سندرم وقفه‌ی تنفسی در خواب با شدت ‌های مختلف مورد نظر می ‌باشد. بسیاری از پژوهشگران این بیماری ‌هارا یک گستره‌ پیوسته در نظر می‌ گیرند که از خرناس ساده شروع و با گذشت زمان به تدریج یا به سرعت به سوی وقفه‌ی تنفسی پیشرفت می‌ نماید و از این رو از موارد خفیف چشم‌پوشی نمی ‌کنند. در این رهگذر تشخیص زودرس و درمان مناسب برای افراد مبتلا اهمیت حیاتی خواهد داشت. پلی‌سومنوگرافی (Polysomnography Nocturnal) کاربردی‌ترین و روش استاندارد برای تشخیص اختلالات تنفسی در خواب می ‌باشد که هم برای تشخیص اولیه و هم برای تعیین شدت بیماری وقفه‌ی تنفسی در خواب و هم برای کشف برخی اختلالات اولیه خواب به کار می ‌رود. پلی‌ سومنوگرافی عبارتست از مانیتورینگ همزمان و ثبت پارامترهای فیزیولوژیک بدن در طول خواب شبانه می باشد که رابطه تغییرات پارامترهایی از قبیل تغییرات ریتم قلب، هیپوکسی، تلاشهای عضلانی، خستگی عضلانی ناشی از تلاش زیاد، خرناس، تعداد موارد پریدن از خواب و ... را با مراحل مختلف خواب مشخص می سازد. سه جزء اساسی پلی سومنوگرافی عبارتند از: الکتروآنسفالوگرافی (EEG) الکترواوکولوگرافی (EOG) الکترومایوگرافی سطحی (EMG) سایر پارامترهایی که ثبت می شوند عبارتند از : جریان هوا از بین و دهان به طور جداگانه الکتروکاردیوگرافی پالس اکسی متری تلاشهای تنفسی سینه ای و شکمی ضبط صداهای تنفسی ثبت وضعیت های بدن در حین خواب با گیرنده های اختصاصی ثبت ویدیویی حرکات و وضعیت بدن برا ی اطمینان از صحت ثبت سایر گیرنده ها   پس از انجام تست نباید به جواب کامپیوتری اعتماد نمود و پزشک ارزیاب باید لحظه به لحظه وقایع حین خواب را باز بینی و با امواج EEG مقایسه نماید تا گزارش دقیقی از آنچه که رخ داده تهیه نماید. پس از انجام تست صحیح پزشک ارزیابی کننده با تجربه می تواند وجود اختلالات خواب و شدت این اختلالات را تشخیص و گزارش نموده، روش مناسب درمانی را پیشنهاد نماید}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13206.html}, eprint = {} }
