Antibiotic therapy in salmonellosis is restricted to typhoid fever and acute infections. After the first reporting of resistance in S. Typhimurium DT 104, nowadays, developing resistance in Salmonella, specially, ESBL (such as CTX-M) species is an important issue in salmonellosis. In this study, we evaluated bla-ctx-m-type gene in clinical isolates of Salmonella enterica.Methods: In this cross-sectional study, we collected clinical isolates of patients in 18 months. Initially, we surveyed drug sensitivity with disk diffusion method, and then determined MIC of resistant isolates with E-test stripes. The existence of ESBL (extended spectrum beta-lactamase) enzymes was examined by ESBL disks in Double Disk method, and these resistances was evaluated with PCR by using a pair of universal primers.Finding: 36 isolates were sensitive to all of antibiotics, but, in 60 isolates there was at least one resistance. In 45 isolates, there were multi drug resistance (MDR) phenotypes. Resistance to ampicilin was the highest percent, whereas, there was not any resistance to imipenem and ciprofloxacin, which were improved with E-test strips. ESBL evaluation, showed producing cefotaxime ESBL in 5 isolates and ceftazidim ESBL in 4 isolates; 2 isolates of these had bla-ctx-m-type gene in plasmid.Conclusion: Detection of MDR trait, especially extended spectrum beta-lactamases in resistant clinical Salmonella isolates, point to the attention in usage of extended spectrum cephalosporins and detection of resistance rate in a nation level.}, keywords = {Salmonella,Extended spectrum beta-lactamase (ESBL),Multi drug resistance (MDR),CTX-M enzyme}, title_fa = {بررسی شیوع نوپدیدی ژن مقاومت بتالاکتاماز وسیع الطیف Bla-ctx-m-type در سروتیپ‌های سالمونلا انتریکا جدا شده از مدفوع بیماران}, abstract_fa = {مقدمه: سالمونلوزیس یکی از معضلات بهداشتی جوامع بشری به حساب می‌آید. با وجود این اهمیت، درمان دارویی سالمونلوزیس محدود به بیماری حاد و تب تیفوئید است. امروزه، پس از شناسایی اولین سویه‌ی مقاوم سالمونلا، گزارش‌های مبنی بر وجود انواع مقاومت‌های دارویی و به خصوص بتالاکتامازهای وسیع الطیف از سراسر دنیا ارایه می‌شود.روش‌ها: ابتدا با روش انتشار از دیسک، حساسیت دارویی سویه‌های آن‌ها بررسی شد، سپس توسط روش E-test، MIC موارد مقاوم تعیین گردید. با استفاده از دیسک‌های ESBL جهت بررسی آنزیم بتالاکتاماز به روش Double disk، وجود بتالاکتاماز در انواع مقاوم تعیین شد و این مقاومت‌ها با استفاده از روش PCR و پرایمر یونیورسال، مورد بررسی قرار گرفت.یافته‌ها: در60 سویه، مقاومت به حداقل یک آنتی‌بیوتیک وجود داشت. از این میان، 45 سویه واجد مقاومت چندگانه‌ی دارویی بود. بیشترین مقاومت نسبت به آمپی سیلین دیده شد، این در حالی بود که هیچ مقاومتی نسبت به ایمی‌پنم و سیپروفلوکساسین وجود نداشت که این نتایج توسط نتایج E-test تأیید شد. نتایج بررسی ESBLها حاکی از وجود 5 مورد مقاومت به سفوتاکسیم بود. در مورد ایزوله‌های مقاوم، 2 مورد واجد قطعه‌ی ژنی پلاسمیدی Bla-ctx-m-type بود.نتیجه‌گیری: جدا سازی مقاومت چند دارویی در سالمونلاهای ایزوله شده و اثبات وجود بتالاکتامازهای وسیع الطیف در انواع مقاوم، توجه ویژه به مصرف سفالوسپورین‌های وسیع الطیف و بررسی‌های گسترده را در سطح ملی خاطر نشان می‌سازد.}, keywords_fa = {سالمونلا,بتالاکتامازهای وسیع الطیف,مقاومت چند دارویی,آنزیم CTX-M}, url = {https://jims.mui.ac.ir/article_13232.html}, eprint = {https://jims.mui.ac.ir/article_13232_a80d61f2dc5d2a6169e53a26b060f155.pdf} }
