Provided that intravenous midazolam can increase duration of lidocaine interathecal block, it would solve the problem.Methods: This was a double-blind clinical trial study in which adults underwent elective abdominal surgery and lower extremities of ASA, I, II and studied in two 18-member groups. Midazolam group (0.03 mg/kg) underwent the intervention as Midazolam 2mg + fentanyl 1μg/kg (5 min after administration of midazolam) and the control group as normal saline 2cc + fentanyl 1μg/kg (5 min after administration of normal saline). The highest level of sensory block, duration of sensory block, duration of motor (movement) block and recovery duration were measured and documented.Finding: Mean duration of sensory block had no significant difference between the two groups; however, duration of motor block for the Midazolam group was 82.9 ± 27.3 minutes which significantly was more than control group with 59.1 ± 26.5 minutes (P = 0.01). The median of sensory block level for midazolam and control groups respectively were T8 and T10 and according to nonparametric median test, the median of sensory block level had a significant difference in the two groups (P = 0.02). The average duration of stay in the recovery, in the patients of midazolam group was 116.1 ± 29.6 minutes and in the control group also was 87.8 ± 24.1 minutes which significantly it was higher in midazolam group (P = 0.002).Conclusion: The present study indicated that intravenous midazolam injection in spinal anesthesia with lidocaine increased the duration of motor block without any side effects (nausea, vomiting and respiratory distress), and caused more hemodynamic and saturation loss of arterial blood pressure.}, keywords = {Midazolam,Lidocaine,Spinal anesthesia}, title_fa = {تأثیر میدازولام وریدی بر طول مدت بیهوشی اسپاینال با لیدوکائین}, abstract_fa = {مقدمه: یکی از مشکلاتی که همواره متخصصین بیهوشی در بلوک های اسپاینال با آن روبه‌رو هستند، افزایش مدت عمل جراحی بیش از حد تخمین زده شده می‌باشد. در صورتی که میدازولام وریدی بتواند باعث افزایش مدت بلوک اینتراتکال لیدو کائین شود، در این موارد راه‌گشا و باعث حل مشکل می‌شود.روش‌ها: در یک مطالعه‌ی کارآزمایی بالینی دو سو کور بالغین کاندید عمل جراحی الکتیو شکم و اندامهای تحتانی گروه I,II ASA در دو گروه 18 نفره انتخاب و بررسی شدند. گروه میدازولام (mg/kg 03/0)، mg 2 میدازولام + g/kgµ 1 فنتانیل (5 دقیقه پس از تجویز میدازولام) و گروه شاهد به صورت cc 2 نرمال سالین + g/kgµ 1 فنتانیل (5 دقیقه پس از تجویز نرمال سالین) تحت مداخله قرار گرفتند. بالاترین سطح بلوک حسی، طول مدت بلوک حسی، طول مدت بلوک حرکتی و مدت ریکاوری اندازه گیری و ثبت گردید.یافته‌ها: میانگین مدت زمان بلوک حسی بین دو گروه تفاوت معنی داری نداشت ولی میانگین مدت زمان بلوک حرکتی برای بیماران گروه میدازولام 3/27 ± 9/82 بود که به طور معنی داری از گروه شاهد با 5/26 ± 1/59 دقیقه بیشتر بود (01/0 = P). میانه سطح بلوک حسی برای گروه میدازولام و گروه شاهد به ترتیب T8 و T10 بود و طبق آزمون ناپارامتری میانه، میانه سطح بلوک حسی در دو گروه تفاوت معنی داری داشت (02/0 = P). میانگین مدت زمان ماندن در ریکاوری در بیماران گروه میدازولام 6/29 ± 1/116 دقیقه و در گروه شاهد 4/21 ± 8/87 دقیقه بود که بطور معنی داری در گروه میدازولام بالاتر بوده است (002/0 = P).نتیجه‌گیری: پژوهش حاضر نشان داد که تزریق میدازولام وریدی در بیهوشی اسپاینال با لیدوکائین باعث افزایش طول مدت بلوک حرکتی بدون ایجاد عوارض جانبی (تهوع– استفراغ و دیسترس تنفسی)، افت همودینامیک و افت اشباع فشار خون شریانی بیشتر در بیماران می‌شود.}, keywords_fa = {میدازولام,بیهوشی اسپاینال,لیدوکائین}, url = {https://jims.mui.ac.ir/article_13260.html}, eprint = {https://jims.mui.ac.ir/article_13260_5cdb84bed56322b49d488ce62b79f6be.pdf} }
