In this study the effect of using low dose ketamine through sub pleural catheter for pain relief after thoracotomy were compared with marcaine.Methods: In this prospective clinical trial study, patients undergoing elective thoracotomy were included. After thoracotomy, for pain relief the patients in case and control groups received 0.5 mg/kg/hr ketamine and 5 cc marcaine 0.5% through sub pleural catheter, respectively. Vital signs, arterial oxygen saturation, and pain level of patients in both groups were recorded and compared at 1 hour after the surgery in recovery and at 8, 16, and 24 hours after surgeryFindings: In ketamine and marcaine groups, 41 and 51 patients were studied, respectively. Mean pain level before intervention and 8, 16, and 24 hours after it was higher in ketamine group (P < 0.05). One hour after local injection of local anesthetic drugs, the level of pain relief was similar in two studied groups (P > 0.05). Systemic analgesic dose used after thoracotomy was significantly lower in marcaine group than ketamine group (P < 0.05). Mean difference of pain before and after injection of local anesthetic drugs in the two groups did not different significantly (P > 0.05).Conclusion: Low-dose ketamine likewise marcaine could be used as a known local anesthetic for post-operative pain relief but this pain reduction is less due to the short half-life of ketamine. }, keywords = {Ketamine,Marcaine,Thoracotomy,Post-operative pain}, title_fa = {تأثیر انفوزیون متناوب کتامین با دوز اندک از طریق کاتتر ساب پلورال در کاهش درد پس از توراکوتومی و مقایسه با مارکائین}, abstract_fa = {مقدمه: عدم تسکین کافی درد بیماران پس از انجام عمل جراحی توراکوتومی باعث عوارض فراوانی می‌شود. در این مطالعه استفاده از دوز کم کتامین از طریق کاتتر ساب پلورال جهت تسکین درد بیماران پس از توراکوتومی با مارکائین مقایسه شد.روش‌ها: در این کارآزمایی بالینی آینده‌نگر، بیماران کاندید عمل جراحی توراکوتومی غیراورژانسی مورد مطالعه قرار گرفتند. پس از خاتمه‌ی عمل در گروه مورد، کتامین با دوز mg/kg/hr 5/0 و در گروه شاهد، cc 5 مارکائین 5/0 درصد از طریق کاتتر انفوزیون شدند. علایم حیاتی، اشباع اکسیژن شریانی و میزان درد بیماران در دو گروه مورد مطالعه در 1 ساعت اول پس از جراحی در ریکاوری و در زمان‌های 8، 16 و 24 ساعت پس از عمل جراحی ثبت و با یکدیگر مقایسه شد.یافته‌ها: 41 بیمار در گروه کتامین و 51 بیمار در گروه مارکائین مورد مطالعه قرار گرفتند. میانگین درد بیماران قبل از تزریق داروی بی‌حس کننده‌ی موضعی در ریکاوری و زمان‌های 8، 16 و 24 ساعت پس از عمل جراحی، در گروه کتامین بیشتر از گروه مارکائین بود (05/0 > P). 1 ساعت پس از تزریق بی‌حس کننده‌ی موضعی میزان کاهش درد بیماران در دو گروه مارکائین و کتامین مشابه بود (05/0 < P). دوز مصرف مسکن سیستمیک در گروه مارکائین نیز به طور معنی‌داری کمتر از گروه کتامین بود (05/0 > P). اختلاف میانگین درد قبل و بعد از تزریق داروی بی‌حس کننده‌ی موضعی در دو گروه تفاوت معنی‌داری نداشت (05/0 < P).نتیجه‌گیری: کتامین با دوز اندک به اندازه مارکائین به عنوان یک بی‌حس کننده‌ی موضعی شناخته شده می‌تواند از درد بیماران پس از عمل جراحی بکاهد ولی این میزان کاهش درد با توجه به نیمه عمر کوتاه داروی کتامین کمتر می‌باشد.}, keywords_fa = {مارکائین,کتامین,توراکوتومی,درد پس از عمل}, url = {https://jims.mui.ac.ir/article_13343.html}, eprint = {https://jims.mui.ac.ir/article_13343_ecdb11f0ad44351fb80f01e1151fa3c2.pdf} }
