In this condition, there is gathering of fungal mycelium in sinus cavity without bone or vessels invasion. It presented as a chronic sinusitis with headache, nasal discharge and obstruction. Diagnosis of aspergillosis infection has often dependent on clinical judgment combined with radiologic evaluation and demonstration of the fungus in tissue specimens obtained from the presumed site of infection.Case Report: A 53-year-old man with uncontrolled insulin dependent diabetes mellitus presented with six month history of headache. He had used various drug prescribed by different physicians in the past six months without any effect. Because of progressive headache, he was admitted in the department of otolaringology and surgical debridement was done. Histopathological examination was suggestive of maxillary sinusitis with fungal ball of aspergilluma. The patient treated with itraconazole and after 6 months, he felt better with no clinical and paraclinical signs.Conclusion: Since fungal sinusitis presentation is similar to chronic sinusitis and in the other hand, the incidence of aspergillosis is increasing, the early diagnosis and treatment is important especially when the clinical and paraclinical signs are suggestive. }, keywords = {Sinus mycetoma,Aspergilloma,Fungal sinusitis}, title_fa = {معرفی یک مورد مایستومای سینوس ماگزیلاری ناشی از آسپرژیلوس}, abstract_fa = {مقدمه: گرانولوم آسپرژیلوس سینوس‌های پارانازال یک عفونت مزمن و نادر سینوس‌ها می‌باشد که به شکل سینوزیت نمایان می‌شود. تشخیص این بیماری به طور تیپیک امکان پذیر نمی‌باشد مگر این که تصویر برداری و بیوپسی از سینوس صورت گیرد. در این گزارش یک مورد مایستومای ناشی از آسپرژیلوس که تنها با سردرد و ترشحات پشت حلق تظاهر کرده بود، معرفی می‌شود که در ابتدا به صورت یک سینوزیت باکتریال حاد درمان می‌شده است. معمول بودن علایم این بیمار باعث 6 ماه تأخیر در تشخیص و درمان بیماری گردید.معرفی بیمار: بیمار آقای 53 ساله‌ای مبتلا به دیابت وابسته به انسولین غیر کنترل بود که با سردردهای 6 ماهه به متخصص گوش و حلق و بینی مراجعه کرده است. برای وی رینوسکوپی انجام ‌شد و پس از انجام سی‌تی‌ اسکن از سینوس ماگزیلاری در طی آندوسکوپی چندین بیوپسی از ضایعات گرفته و عمل دبریدمان انجام شد و نمونه‌ها برای پاتولوژیست ارسال گردید. گزارش پاتولوژی نمونه‌ها، گونه‌ی آسپرژیلوس را تأیید نمود و بر اساس آن بیمار تحت درمان با ایتراکونازول قرار گرفت. بعد از یک هفته، سردردهای بیمار بهبود نسبی پیدا کرد و مرخص شد و پس از 6 ماه پی‌گیری بهبودی کامل حاصل گردید.نتیجه‌گیری: با توجه به شباهت علایم سینوزیت قارچی به سینوزیت مزمن، این تشخیص نیز مد نظر قرار گیرد.}, keywords_fa = {آسپرژیلوما,مایستومای سینوس,سینوزیت قارچی}, url = {https://jims.mui.ac.ir/article_13352.html}, eprint = {https://jims.mui.ac.ir/article_13352_79a29dc3f7b743e9e8934b5fb40f4d47.pdf} }
