The predominant age range of this disease is related to adolescence and its prevalence rate in women is two to three times more than in men. Indentifyingthe factors and conditions cause diseases recurrence and in other words providing disease conditions can be effective in the prevention of diseases occurrence or recurrence. One of these conditions and requirements is menstrual cycle of women and hormonal changes that can be effective on the occurrence or recurrence of the disease and therefore indentifying the relationship between hormonal changes during menstruation seem necessary for the disease recurrence.Methods: In this study, 196 women with MS, who had regular menstrual cycle had referred due todisease attack, were studied in terms of the time of occurring the attack in menstrual cycle. Menstrual cycle of the patients was divided into three phases based on the estrogen and progesterone levels. The first phase is three days after onset of menstrual cycle until the time of ovulation and is simultaneous with increased estrogen. The second phase is since probable ovulation time (Approximately 14th day in a 28-day cycle or 14 days before the period in the other cycles) until three days before onset of the next menstruation; in this phase, the levels of both estrogen and progesterone increase. The third phase is equal to three days before bleeding until three days after the onset (decreased estrogen and progesterone levels). According to the LMP date of patients and time of attack, the phase of attack occurrence in menstrual cycle was determined for each of them.Findings: The highest recurrence rate of attack (99 attacks which was equal to 50.6% of the total attacks) occurred at the third phase of menstrual cycle in which the levels of both estrogen and progesterone was low. Twenty-seven percent of the attacks recurrence occurred at the first phase (which the estrogen level is high) and 22.4 percent of the attacks recurrence occurred at the second phase (which both estrogen and progesterone levels are high) that the results showed no significantdifference.Conclusion: The disease might be influenced by the female sex hormones and sex steroids can alter the disease activity. Estrogen may play a protective role in the disease and reduce estrogen is associated with increased recurrence of attacks.}, keywords = {Multiple Sclerosis,Menstrual cycle}, title_fa = {بررسی تأثیر سیکل قاعدگی در بروز علایم بیماران مبتلا به مولتیپل اسکلروزیس}, abstract_fa = {مقدمه: مولتیپل اسکلروزیس ( MSیا (Multiple sclerosis یک بیماری خودایمنی است که با دمیلینه شدن اکسون نورون‌های سیستم عصبی مرکزی همراه می‌باشد. دامنه‌ی سنی غالب در این بیماری مربوط به دوران جوانی افراد است و میزان شیوع بیماری در خانم‌ها دو تا سه برابر آقایان است. شناخت عوامل و شرایطی که باعث عود بیماری و به عبارتی فراهم شدن شرایط بیماری می‌شود می‌تواند در پیش‌گیری از بروز و یا عود بیماری مؤثر باشد. یکی از این شرایط، دوران سیکل قاعدگی خانم‌ها و تغییرات هورمونی حین آن است که می‌تواند بر شرایط بروز و عود این بیماری مؤثر باشد. شناخت ارتباط تغییرات هورمونی حین قاعدگی بر عود بیماری لازم به نظر می‌رسد.روش‌ها: در این مطالعه تعداد 196 بیمار خانم مبتلا به مولتیپل اسکلروزیس و دارای سیکل قاعدگی منظم که به علت حمله‌ی بیماری مراجعه کرده بودند، از نظر زمان بروز حمله در سیکل قاعدگی مورد بررسی قرار گرفتند. سیکل قاعدگی بیماران بر اساس سطح هورمون‌های استروژن و پروژسترون به سه فاز تقسیم شد. فاز اول از سه روز پس از شروع سیکل قاعدگی تا زمان تخمک‌گذاری و هم‌زمان با افزایش استروژن بود. فاز دوم از زمان تخمک‌گذاری احتمالی (حدود روز 14 در سیکل 28 روزه و یا 14 روز قبل از پریود در سایر سیکل‌ها) تا 3 روز قبل از شروع قاعدگی بعدی بود که در این فاز سطح هر دوهورمون استروژن و پروژسترون بالا است. فاز سوم برابر با سه روز قبل از شروع خونریزی تا سه روز پس از شروع آن بود (کاهش استروژن و کاهش پروژسترون). بر اساس تاریخ LMP (Last menstrual period) بیماران و زمان حمله، فاز وقوع حمله در سیکل قاعدگی برای هر یک مشخص گردید.یافته‌ها: بیشترین عود حملات بیماری (99 حمله که معادل 6/50 درصد کل حملات بود) در فاز سوم سیکل قاعدگی روی داد که در آن سطح هر دو هورمون استروژن و پروژسترون پایین است. 27 درصد عود حملات در فاز یک (که سطح استروژن بالا است) و 4/22 درصد عود حملات در فاز دو (که سطح استروزن و پروژسترون بالا است) روی داد که تفاوت معنی‌داری داشت.نتیجه‌گیری: بیماری می‌تواند از هورمون‌های جنسی زنانه تأثیرپذیر باشد و استرویید‌های جنسی می‌توانند فعالیت بیماری را تغییر دهند. استروژن نقش محافظتی را در این بیماری دارد و کاهش استروژن با افزایش عود حملات همراه است.}, keywords_fa = {مولتیپل اسکلروزیس,سیکل قاعدگی}, url = {https://jims.mui.ac.ir/article_13563.html}, eprint = {https://jims.mui.ac.ir/article_13563_e70288c33bc3cefd87cecc2c957bc4ab.pdf} }
