In addition to deformity and deviation in proximal of tibia the patients have bowing and deviation in the middle and distal part of tibia (tibia vara). This causes complications and significant disability in these patients. In the current study we performed osteotomy tibia on patients in the two proximal and distal levels, and then evaluated radiologic parameters related to correction of alignment of patients.Methods: This is a prospective clinical trial. 15 patients with genu varum associated with bowing of the middle and distal part of tibia were selected. Alignment view was taken before and after the operation (after 2-3 months), and the improvement of radiological parameters after double level osteotomy of tibia were evaluated.Findings: Mean preoperative and postoperative tibia femoral angles were 18.3 and 3.93, respectively. Mean preoperative and postoperative MPTA were 79.13 and 89.09, respectively. Mean preoperative and postoperative LDTA were 96.40 and 88.73. 73% of patients (II case) had normal mechanical akis of limb after the operation. Improvement of all angles was statistically significant. Conclusion: In patients with genu varum in addition to deformity of middle and distal part of tibia, correction of limb alignment with one level osteotomy is impossible. Osteotomy in proximal and distal part of tibia can significantly correct limb alignment, and maintain joint orientation in its normal state.}, keywords = {Genu varum,Double level tibial osteotomy,Limb alignment}, title_fa = {بررسی نتایج Double tibial osteotomy در بیماران مبتلا به ژنوواروم همراه با تیبیاوارا}, abstract_fa = {مقدمه: ژنوواروم در بیمارانی که دارای یک بیماری زمینه‌ای شناخته شده مثل ریکتز یا استئومالاسی هستند یک دفرمیتی پیچیده است. این بیماران علاوه بر دفرمیتی و انحراف در ناحیه‌ی پروگزیمال تیبیا دارای دفرمیتی و انحراف در قسمت میانی و دیستال نیز می‌باشند که می‌تواند باعث عوارض و ناتوانی‌های بارزی در این بیماران گردد. در این مطالعه بیماران در دو سطح پروگزیمال و دیستال استئوتومی بررسی شدند و پارامترهای رادیولوژیک مربوط به اصلاح راستای اندام را پس از استئوتومی در دو سطح، مورد بررسی قرار گرفت.روش‌ها: این یک مطالعه‌ یک کارآزمایی بالینی آینده‌نگر بود. 15 بیمار دارای ژنوواروم همراه با Bowing قسمت میانی و دیستال ساق وارد مطالعه شدند. قبل و بعد از عمل (بعد از یونیون) Alignment view از بیماران گرفته شد و بهبود پارامترهای رادیولوژیک به دنبال دو سطح استئوتومی بررسی شد.یافته‌ها: میانگین زاویه‌ی تیبیو فمورال قبل از عمل 3/18 درجه و پس از عمل 93/3 درجه بود. میانگین Medial proximal tibial angle (MPTA) قبل از عمل 13/79 درجه و پس از عمل 06/89 درجه بود. میانگین Lateral distal tibial angle (LDTA) قبل از عمل 40/96 درجه و پس از عمل 73/88 درجه بود و 73 درصد بیماران (11 اندام) پس از عمل محور مکانیکی طبیعی داشتند. بهبود تمامی زوایا از نظر آماری معنی‌دار بود.نتیجه‌گیری: در بیمارانی که ژنوواروم همراه با دفرمیتی قسمت میانی و دستیال تیبیا دارند امکان اصلاح راستای اندام با یک سطح استئوتومی وجود ندارد. انجام استئوتومی در قسمت پروگزیمال و سوپر مالئولار می‌تواند به طور مؤثری هم راستای اندام را اصلاح کند و هم Orientation Joint این بیماران را در وضعیت طبیعی حفظ کند.}, keywords_fa = {ژنوواروم,استئوتومی تیبیا در دو سطح,راستای اندام}, url = {https://jims.mui.ac.ir/article_13587.html}, eprint = {https://jims.mui.ac.ir/article_13587_5d1c096fea311150a34dcd622e7537b8.pdf} }
