A silicone balloon expander is inserted under the skin, near the area to be repaired, and then gradually filled with salt water over time, causing the skin to stretch and grow. This procedure is most commonly used for breast reconstruction following breast removal, but it is also used to repair skin damaged by birth defects, accidents or surgery, and in certain cosmetic procedures. Two kinds of expanders are available for the above procedures: expander with external port and expander with internal port. This study was done in order to determine the prevalence of complications and patient satisfaction in surgery with internal and external port of tissue expander.Methods: This is a randomized clinical trial conducted in Alzahra and Imam Mousa Kazem hospitals, Isfahan, Iran, during 2008-2009. 76 patients who were candidates for tissue expansion were randomly divided into two groups. The patients in the first group were operated by the internal port, and the patients in the second group were operated by the external port of tissue expander. All patients were followed until the removal of the expander, and the complications of the expander were recorded. Moreover, patient satisfaction was measured at the end of study. Finally all data were analyzed by SPSS software.Findings: According to the results of this study the frequency distribution of tissue expansion complications in internal and external ports were 31.6% and 42.1%, respectively. No statistical differences were observed between the two groups (P = 0.34). Moreover, the overall patient satisfaction in internal and external ports were 70.2% and 78.9% respectively, and there was no statistical difference between the two groups (P = 0.81). Conclusion: In the internal port there is a necessity for another cavity (other than the space for the tissue expander), the port is placed under the skin, and the liquid is injected into the port during several sessions. In this method because the place of the port is not visible it results in the satisfaction of patients, however, the repeated injections cause discomfort for the patients, and increase the risk of complications such as skin infections. The external port method causes patient dissatisfaction due to its visibility and appearance. Therefore, it is best to survey the patients on the two expander methods after the patients are discharged from the hospital and the expander is removed.}, keywords = {Tissue expander,Satisfaction,Complication}, title_fa = {مقایسه‌ی توزیع فراوانی نسبی عوارض و رضایت‌مندی بیماران تحت عمل جراحی با اکسپاندر بافتی با پورت داخلی و پورت خارجی}, abstract_fa = {مقدمه: جهت بازسازی نقص‌های بافت نرم در سر و گردن بهترین حالت استفاده از بافت نرم همان ناحیه می‌باشد. بدین منظور از اکسپاندرهای بافتی استفاده می‌شود. اکسپاندرها در دو نوع با پورت داخلی و پورت خارجی در دسترس هستند که هر کدام دارای مزایا و معایبی می‌باشند. هدف از این مطالعه تعیین توزیع فراوانی عوارض و رضایت‌مندی بیماران تحت عمل جراحی با اکسپاندر بافتی با پورت داخلی و پورت خارجی بود.روش‌ها: این مطالعه یک کارآزمایی بالینی بود که در سال‌های 1387 و 1388 در مراکز آموزشی-درمانی الزهرا (س) و امام موسی کاظم (ع) اصفهان انجام شد. جامعه‌ی آماری مورد مطالعه شامل بیماران کاندید عمل کاشت اکسپاندر بافتی بودند. بیماران با استفاده از روش تخصیـص تصادفی به دو گروه تقسیم شدند. محل و نوع تـرومای پوستی در دو گروه یکسـان بود. در گروه اول از اکسپاندر بافتی با پورت داخـلی و در گروه دوم از اکسپاندر بافتی با پورت خارجی استفاده شد. پس از عمل، بیماران تا خارج کردن اکسپاندر مورد بررسی قرار گرفتند و عوارض ایجاد شده در آن‌ها مقایسه شد. رضایت‌مندی بیماران دو گروه نیز در زمان خارج کردن اکسپاندر مورد ارزیابی قرار گرفت. داده‌های به وسیله‌ی نرم‌افزار SPSS تجزیه و تحلیل شد.یافته‌ها: 12 نفر از گروه اکسپاندر داخلی و 16 نفر از گروه اکسپاندر خارجی دارای عوارض بودند (6/31 درصد در مقابل 1/42 درصد) (34/0 = P). 10 نفر از گروه اکسپاندر داخلی و 13 نفر از گروه اکسپاندر خارجی به طور کامل راضی بودند (27 درصد در مقابل 2/34 درصد). همچنین در دو گروه داخلی و خارجی به ترتیب 16 و 17 نفر راضی (2/43 درصد در مقابل 7/44 درصد) و 2 و 1 نفر ناراضی بودند (4/5 درصد در مقابل 6/2 درصد). در دو گروه داخلی و خارجی 9 و 7 نفر نیز نظری نداشتند (3/24 درصد در مقابل 4/18 درصد) (81/0 = P).نتیجه‌گیری: نیاز به ایجاد یک حفره‌ی دیگر در زیر پوست جهت تزریق مایع در جلسات متعدد در پورت داخلی ممکن است سبب ناراحتی بیمار و افزایش عوارضی مثل عفونت پوستی شود. در پورت خارجی، مشکلات مربوط به پورت داخلی وجود ندارد ولی به علت واضح بودن پورت و ظاهر نامناسب آن، ممکن است در بیمار ایجاد نارضایتی بنماید. از این رو به منظور نظرسنجی از بیماران در مورد دو نوع اکسپاندر مورد استفاده، بهتر است این کار به بعد از ترخیص بیمار از بیمارستان در پایان استفاده از اکسپاندر انجام گیرد.}, keywords_fa = {اکسپاندر بافتی,رضایت‌مندی,عوارض}, url = {https://jims.mui.ac.ir/article_13590.html}, eprint = {https://jims.mui.ac.ir/article_13590_6ac0325740e2e58ba680207b4dda48f6.pdf} }
