Like any other medical procedure, orthopedic surgery is associated with pain and complications. Recently, doxepin has been used as a tricyclic anti depressant (TCA) to decrease pain. Therefore, we designed this study to evaluate the effects of doxepin on the postoperative pain following lower limb orthopedic surgery.Methods: This randomized double blind placebo controlled trial was performed on 102 lower limb orthopedic patients in three groups of 34. The first two group received 50 and 100 mg doxepin while the last group received placebo. Patients aged between 18 and 65 years. Data was collected by a standard questionnaire and analyzed by analysis of variance (ANOVA) and chi-square test in SPSS.Findings: No significant differences were observed between the three groups in terms of pain intensity before or after surgery (P = 0.58). The only significant difference was seen between the first (50 mg doxepin) and placebo groups during the first 24 hours after surgery (P = 0.25). The groups were not different in demographic characteristics (age, gender, or education), either. In addition, received doses of morphine, and the frequency of nausea, vomiting, itchiness, and other side effects were not significantly different between groups. Conclusion: According to our results, doxepin was not effective on pain reduction after lower limb orthopedic surgery. We therefore hypothesize doxepin to have more effects on chronic pains than acute pains.}, keywords = {Doxepin,Visual analogue scale,Lower limb orthopedic surgery}, title_fa = {بررسی تأثیر مصرف خوراکی دو دوز داروی دوکسپین قبل از عمل بر شدت درد بعد از جراحی ارتوپدی اندام تحتانی و مقایسه‌ی آن با گروه شاهد}, abstract_fa = {مقدمه: یکی از عوارض اعمال جراحی، درد بعد از آن می‌باشد. این عارضه می‌تواند مشکلاتی مثل افزایش فشارخون، افزایش مصرف اکسیژن، افزایش خطر انفارکتوس میوکارد و مشکلات دیگری را باعث شود. اعمال جراحی ارتوپدی نیز مانند سایر اعمال جراحی دارای درد و عوارض حاصل از آن هستند. به تازگی دوکسپین به عنوان یک داروی ضد افسردگی سه حلقه‌ای (Tricyclic antidpressant یا TCA) جهت کاهش درد به کار رفته است. در این تحقیق، تأثیر دوکسپین روی درد و عوارض آن بعد از اعمال جراحی ارتوپدی اندام تحتانی را بررسی کردیم.روش‌ها: در این مطالعه‌ی دوسو کور، 102 بیمار تحت اعمال جراحی ارتوپدی اندام تحتانی، در 3 گروه 34 تایی تحت بررسی قرار گرفتند. یک گروه 50 میلی‌گرم دوکسپین، گروه دیگر 100 میلی‌گرم دوکسپین و گروه سوم دارونما دریافت کردند. قبل از دادن دارو، از بیماران رضایت‌نامه‌ی کتبی اخذ شد. یافته‌ها از طریق پرسش‌نامه جمع‌آوری و توسط نرم‌افزار SPSS آنالیز شد.یافته‌ها: اختلاف شدت درد در بین گروه‌های معرفی شده در طول زمان‌های مورد بررسی قبل و بعد از جراحی از نظر آماری معنی‌دار نبود (58/0 = P) و تنها در 24 ساعت بعد از عمل این تفاوت بین گروه‌های A و C معنی‌دار شد. همچنین تفاوت متغیرهای دموگرافیک (سن، جنس) و نیز وزن در بین گروه‌های مورد بررسی معنی‌دار نبود. در این بررسی میزان مخدر (مورفین) مصرفی در 24 ساعت اول بعد از عمل نیز سنجیده شد که این متغیر هم تفاوت معنی‌داری در 3 گروه نداشت (25/0 = P). همچنین در این مطالعه فراوانی عوارضی مانند تهوع، استفراغ، خارش، خواب‌آلودگی، احتباس ادرار، سرگیجه و آتاکسی در 24 ساعت اول بعد از عمل در گروه‌های مورد بررسی، تحت مطالعه قرار گرفت که هیچ ‌یک تفاوت معنی‌داری نداشتند.نتیجه‌گیری: طبق مطالعه‌ی به عمل آمده، داروی دوکسپین با دوزهای خوراکی 50 و 100 میلی‌گرم در مقایسه‌ی با دارونما تأثیر بیشتری روی کنترل درد بعد از عمل و میزان مخدر مصرفی در 24 ساعت اول بعد از عمل نداشت.}, keywords_fa = {دوکسپین,شدت درد,اعمال جراحی ارتوپدی اندام تحتانی}, url = {https://jims.mui.ac.ir/article_13668.html}, eprint = {https://jims.mui.ac.ir/article_13668_911c444a03b2358721d66ed2769d6abc.pdf} }
