The aim of this study was to compare intravenous and epidural methods of patient-controlled analgesia (PCA) in patients undergoing hip and knee replacement surgeries. Methods: In a double-blind controlled clinical trial, 60 American Society of Anesthesiologists (ASA) I-III subjects were selected from 40-80 year-old patients undergoing hip or knee arthroplasty. They were randomly allocated into two different groups. The first group received intravenous morphine PCA while the second group received a combination of bupivacaine and fentanyl through an epidural catheter. As placebo, normal saline was administered through an epidural catheter in the first group and intravenously in the second group. Data was collected using a questionnaire and then analyzed by student's t-test, repeated measures analysis of variance (ANOVA), and Mann-Whitney and chi-square tests. Findings: Pain intensity after 12 and 24 hours was significantly higher in the epidural group compared to the intravenous group (1.71 ± 1.62 vs. 1.37 ± 0.53). Obviously, the satisfaction rate was significantly higher in the intravenous group. In addition, the side effects in the intravenous group 12 hours after surgery were less prevalent than the epidural group (92.3% vs. 100%). Itching was significantly more common in the epidural group compared to the intravenous group (31% vs. 7.1%; P < 0.05). Blood pressure at the 6th hour after surgery dropped significantly from 132 ± 22 to 111 ± 23 in the epidural group (P < 0.05). Conclusion: In this study, postoperative pain was better controlled by intravenous morphine PCA. However, the level of consciousness was lower in this group. The overall satisfaction rate was higher using this method. On the other hand, in the epidural PCA group, pain was not controlled and patients experienced reductions in blood pressure. Itching was higher in this group of patients. Keywords: Patient-controlled analgesia, Epidural anesthesia, Postoperative pain}, keywords = {}, title_fa = {مقایسه‌ی کنترل درد توسط بیمار در دو روش وریدی و اپیدورال در افراد تحت عمل جراحی تعویض مفصل هیپ یا زانو}, abstract_fa = {مقدمه: پاسخ‌های فیزیولوژیک به شدت درد و ترومای جراحی بر تمام ارگان‌ها اثر سوء می‌گذارند و می‌توانند عوارض خطرناکی را برای بیمار ایجاد کنند. کنترل درد یکی از اهداف اولیه در مدیریت بیماران است و روش‌های متعددی برای کنترل درد وجود دارد. روش آنالژزی با کنترل بیمار (PCA یا Patient-controlled analgesia) روشی است که جهت کنترل درد در سال‌های اخیر مورد استفاده قرار گرفته است. هدف از این مطالعه، مقایسه‌ی کنترل درد توسط بیماران در دو روش وریدی و اپیدورال بود. روش‌ها: این مطالعه از نوع کارآزمایی بالینی و به صورت دوسوکور بود که بر روی 60 بیمار 40 تا 80 ساله که به صورت Elective کاندید عمل جراحی آرتروپلاستی هیپ و زانو بودند، انجام شد. این بیماران دارای ASA (American Society of Anesthesiologists) 1 تا 3 بودند که به صورت تصادفی در 2 گروه قرار گرفتند. برای گروه اول مورفین به روش داخل وریدی و در گروه دوم ترکیب بوپی‌واکایین و فنتانیل از طریق اپیدورال انجام شد. علاوه بر این، در گروه اول نرمال سالین دارونما از طریق کاتتر اپیدورال و در گروه دوم نرمال سالین دارونما از طریق ورید قرار دادیم. در هر دو روش کنترل توسط خود بیمار انجام شد. پس از جمع‌آوری اطلاعات از طریق پرسش‌نامه، داده‌ها با استفاده از آزمون‌های Student-t، Repeated measures ANOVA، Mann-Whitney و 2χ مورد تجزیه و تحلیل آماری قرار گرفت. یافته‌ها: شدت درد در 12 و 24 ساعت بعد از عمل در گروه اپیدورال (ترکیب بوپی‌واکایین و فنتانیل) (71/1 ± 62/1) نسبت به گروه وریدی (مورفین) (71/1 ± 62/1) به طور معنی‌داری بیشتر بود‌. همچنین میزان رضایتمندی در گروه وریدی بیشتر بود. سطح هوشیاری در گروه وریدی در 12 ساعت بعد از عمل پایین‌تر از گروه اپیدورال بود (05/0 > P). 31 درصد از بیماران در گروه اپیدورال و 1/7 درصد در گروه وریدی خارش داشتند که این تفاوت از نظر آماری معنی‌دار بود (05/0 > P). فشار خون در زمان 6 ساعت در گروه وریدی از 22 ± 132 به 23 ± 111 افت پیدا کرد که این کاهش از نظر آماری معنی‌دار بود ( 05/0 >P ). نتیجه‌گیری: کنترل درد توسط بیمار در روش وریدی بهتر از روش اپیدورال انجام می‌شود. اگر چه افت سطح هوشیاری در روش وریدی بیشتر از روش  اپیدورال است، اما شاید این افت مربوط به تفاوت داروی مصرفی ‌باشد. احتمال می‌رود بالا بودن رضایتمندی در روش وریدی ناشی از کنترل بهتر درد در این روش ‌باشد. به نظر می‌رسد کنترل درد در روش وریدی برای بیماران بیشتر از روش اپیدورال امکان پذیر است. واژگان کلیدی: کنترل درد توسط بیمار، بی‌حسی اپیدورال، روش وریدی، درد بعد از عمل}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13753.html}, eprint = {https://jims.mui.ac.ir/article_13753_f588ba55716a0aceaeea77799ee6d0aa.pdf} }
