Several scoring systems have been developed to determine the severity and prognosis of CAP. Among these, pneumonia severity index (PSI) has become the standard reference because of the prognostic accuracy, effectiveness, and safety as a decision making aid. Methods: We conducted a descriptive, prospective study on all patients with CAP  who were admitted to the infectious diseases ward of Alzahra Hospital (Isfahan, Iran) during 15 months. We assessed our patients based on PSI scoring system and classified them into 5 risk classes. Findings: The mean PSI score in this study was 91.4 ± 33.3 (range: 19-166). According to the calculated scores for each patient, 9 patients (19.1%) belonged to class I, 7 (14.9%) to class II, 7 (14.9%) to class III, 17 (36.2%) to class IV, and 7 (14.9%) to class V. Conclusion: About half of patients admitted to the infectious diseases ward were fine enough to be managed as outpatients. On the other hand, 20% of the patients in class V needed intensive care. It seems that physicians in emergency departments should pay more attention to PSI scoring system to determine the kind and site of care. This scoring system can also be used to assess the prognosis of death. Keywords: Community-acquired pneumonia, Pneumonia severity, Infectious diseases ward}, keywords = {}, title_fa = {شدت پنومونی در بیماران با تشخیص پنومونی اکتسابی در بزرگ‌سالان بستری در بخش عفونی بیمارستان الزهرای (س) اصفهان}, abstract_fa = {مقدمه: پنومونی اکتسابی در جامعه می‌تواند از بهبودی کامل تا مرگ باشد. جهت تعیین شدت پنومونی و پیش‌آگهی، سیستم‌های نمره‌دهی متعددی ابداع شده است. شاخص شدت پنومونی (Pneumonia severity index یا PSI) یک روش استاندارد قابل قبول، با معیارهای پیش‌گویی کننده‌ی دقیق، مؤثر و مطمئن جهت تصمیم‌گیری چگونگی نوع و محل درمان می‌باشد. هدف این مطالعه، بررسی شدت پنومونی در بیماران بستری شده در بخش عفونی بزرگ‌سالان بود. روش‌ها: در این مطالعه‌ی 15 ماهه (از دی ماه 1388 لغایت اسفند 1389) بیماران با تشخیص پنومونی اکتسابی که در بخش بیماری‌های عفونی بیمارستان الزهرای (س) اصفهان بستری بودند، با استفاده از سیستم نمره‌دهی PSI مورد بررسی قرار گرفتند و در کلاس‌‌های پنج گانه دسته‌بندی شدند. یافته‌ها: میانگین نمره‌ی شدت پنومونی در بیماران مذکور 3/33 ± 4/91 با دامنه‌ی 166-19 بود که بر اساس آن، 9 بیمار (1/19 درصد) در کلاس I، 7 نفر (9/14 درصد) در کلاس II، 7 نفر (9/14 درصد) در کلاس III، 17 نفر (2/36 درصد) در کلاس IV و 7 نفر (9/14 درصد) در کلاس V قرار گرفتند. نتیجه‌گیری: حدود نیمی از بیماران مورد مطالعه‌ی ما که در بخش بیماری‌های عفونی بستری بودند، با در نظر گرفتن این سیستم نمره‌دهی می‌توانستند درمان سر پایی شوند. از طرف دیگر، حدود 20 درصد از بیماران این مطالعه که در کلاس V قرار داشتند، بر اساس این سیستم نمره‌دهی اندیکاسیون بستری در بخش مراقبت‌های ویژه را داشتند. به نظر می‌رسد باید در هنگام پذیرش بیماران در بخش اورژانس دقت بیشتری به سیستم نمره‌دهی PSI و تعیین شدت بیماری در جهت انتخاب نوع و محل درمان شود. از سوی دیگر با استفاده از این سیستم نمره‌دهی می‌توانیم تا حدودی پیش‌آگهی مرگ احتمالی بیماران را حدس بزنیم. واژگان کلیدی: پنومونی اکتسابی در جامعه، شدت پنومونی، بخش عفونی}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13772.html}, eprint = {https://jims.mui.ac.ir/article_13772_59194b64d01ce4cdfb7fa5a80fdd63e5.pdf} }
