An important complication of diabetes is microalbuminuria which is an important indication for development of diabetic nephropathy. The prevalence of diabetic nephropathy in patients with type 2 diabetes has been estimated to be 47%. Since the disorder initiates with microalbuminuria, early detection and prevention are of vital importance. Helicobacter pylori infection is more prevalent in diabetic patients due to their low immune tolerance. The cagA positive genotype is reported to be effective in microalbuminuria development in type 2 diabetic patients. In this study, we investigated the relation of H. pylori cagA virulence factor with microalbuminuria in patients with type 2 diabetes.Methods: This study included 88 patients with type 2 diabetes. Microalbuminuria was present in 60 subjects and absent in the other 28. Stool samples were collected from all patients and DNA was extracted using QIAamp DNA Stool Mini Kit. In order to detect H. pylori infection, a nested polymerase chain reaction (PCR) protocol was developed and all H. pylori positive samples were genotyped for cagA positivity.Findings: Overall, 16 patients (18.2%) were detected to be H. pylori positive out of whom 12 had microalbuminuria (75%) and 4 (25%) did not. CagA positivity was detected in 3 subjects who belonged to the group with microalbuminuria. Conclusion: In the population studied and the sample size used, no correlation was found between cagA gene and microalbuminuria in patients with type 2 diabetes.Keywords: Helicobacter pylori, CagA genotype, Microalbuminuria, Type 2 diabetes, Stool DNA}, keywords = {}, title_fa = {ارتباط میکروآلبومینوری و ژن cagA هلیکوباکتر پیلوری در بیماران مبتلا به دیابت نوع دو}, abstract_fa = {مقدمه: یکی از مهم‌ترین مشکلات در مبتلایان به دیابت میکروآلبومینوری است. در واقع میکروآلبومینوری ساده‌ترین و حساس‌ترین فاکتور برای ارزیابی خطر و مشخص کردن بیماری کلیوی در مبتلایان به دیابت است و اگر در مراحل اولیه تشخیص داده شود، می‌توان از بروز و پیشرفت نفروپاتی دیابتی جلوگیری به عمل آورد. هلیکوباکتر پیلوری یکی از عوامل عفونت‌زا در کل جامعه و به خصوص در بیماران مبتلا به دیابت، به دلیل سطح ایمنی پایین‌تر، می‌باشد. بر مبنای بعضی از گزارش‌ها ژنوتیپ cagA مثبت از هلیکوباکتر پیلوری به عنوان عامل مؤثر در ایجاد میکروآلبومینوری قلمداد شده است. هدف مطالعه‌ی حاضر، بررسی ارتباط ژن ویرولانس cagA هلیکوباکتر پیلوری با میکروآلبومینوری در بیماران مبتلا به دیابت نوع دو بود.روش‌ها: در این مطالعه 88 بیمار مبتلا به دیابت نوع دو مراجعه ‌کننده به مرکز تحقیقات غدد و متابولیسم اصفهان بررسی شدند که 60 نفر از آن‌ها بیماران دچار دیابت نوع دو مبتلا به میکروآلبومینوری و 28 نفر بیماران مبتلا به دیابت نوع دو بدون میکروآلبومینوری بودند. نمونه‌های مدفوع از بیماران و افراد شاهد جمع‌آوری گردید و پس از استخراج DNA برای بررسی نمونه‌ها از لحاظ عفونت با هلیکوباکتر پیلوری Nested PCR و سپس ژنوتایپینگ با پرایمرهای اختصاصی cagA انجام شد.یافته‌ها: از کل نمونه‌ها 16 نفر(2/18 درصد) آلوده به هلیکوباکتر پیلوری بودند که 12 نفر (75 درصد) از گروه بیماران دچار دیابت نوع دو مبتلا به میکروآلبومینوری و 4 نفر (25 درصد) از گروه بیماران مبتلا به دیابت نوع دو بدون میکروآلبومینوری بودند. 3 نمونه (8/18 درصد) دارای ژن cagA بودند که هر سه نفر از گروه افراد دچار دیابت نوع دو مبتلا به میکروآلبومینوری بودند. ارتباط بین ژن cagA هلیکوباکتر پیلوری و میکروآلبومینوری در بیماران مبتلا به دیابت نوع دو از نظر آماری معنی‌دار نبود (08/0 = P).نتیجه‌گیری: در این مطالعه اگر چه شیوع میکروآلبومینوری در افراد با ژنوتیپ cagA+ هلیکوباکترپیلوری بیش از افرادی بود که از لحاظ ژنوتیپ هلیکوباکتر پیلوری -cagA بودند، اما در بیماران مبتلا به دیابت نوع دو ارتباط معنی‌داری بین ژن cagA هلیکوباکتر پیلوری و میکروآلبومینوری نبود.واژگان کلیدی: هلیکوباکتر پیلوری، cagA، میکروآلبومینوری، دیابت نوع دو، Stool DNA}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13823.html}, eprint = {https://jims.mui.ac.ir/article_13823_d6673f7b3c303ac00224d1752692b9ed.pdf} }
