The best early diagnostic method for HI in newborns is universal newborn hearing screening (UNHS). This method has been implemented since a few years ago in Iran. In this study, we introduced the findings of hearing screening in a hospital in Tehran (Iran). Methods: This cross-sectional study was conducted on all newborns (n = 3818) in Najmieh Hospital (Tehran, Iran) during 2006-07. Newborns were screened using the Transient­-Evoked Otoacoustic Emission (TEOAE) test at the first 48 hours after birth. The results were documented as "pass" (desirable) or "refer" (undesirable). Neonates with undesirable results were re-screened during the next 2-4 weeks. If the responses in both sessions (test and retest) were not desirable, the infants were referred for a final diagnosis with Auditory Brainstem Response (ABR) test before three months of age. They were then referred to an otolaryngologist for more evaluations. Findings: From 3818 neonates screened, 90% had a pass response and 10% were referred for retest at 2-4 months of age. Of those who were referred, only 11 (3.6%) infants had an undesirable response again. Among these, nine infants referred for ABR and only 3 subjects showed sensory-neural hearing loss as the final diagnosis. One of them did not have any risk factors of HI. Conclusion: In order to minimize false negative and positive results, suspected cases are suggested to be reevaluated during the 2nd to 4th weeks after birth. In addition, high risk infants should be tested by ABR. Since one third of the children with HI in this study did not have any risk factors for HI, we recommend careful hearing screening for all neonates even those without any risk factors. Keywords: Hearing impairment, Universal newborn hearing screening, Transient-evoked otoacoustic emission, Auditory brainstem response}, keywords = {}, title_fa = {بررسی نتایج غربالگری شنوایی همگانی در نوزادان با استفاده از روش‌های TEOAE و ABR: مطالعه‌ی مقطعی توصیفی همراه با مروری بر منابع}, abstract_fa = {مقدمه: کاهش شنوایی جزء شایع‌ترین نقیصه‌های مادرزادی در شیرخواران محسوب می‌شود و در واقع معلولیتی شدید و در عین حال پنهان محسوب می‌گردد. بهترین راه تشخیص زودهنگام این مشکل، غربالگری همگانی شنوایی (Universal newborn hearing screening یا UHNS) در نوزادان است. در کشور ما نیز از چند سال قبل این روش غربالگری در نوزادان صورت می‌گیرد. در این مطالعه نتایج این غربالگری در یکی بیمارستان‌های تهران گزارش شد. روش‌ها: این مطالعه به صورت مقطعی توصیفی بر روی کلیه‌ی نوزادان متولد شده در بیمارستان نجمیه‌ی تهران از اول تیر ماه 1385 تا آخر دی ماه 1386 صورت گرفت. اولین ارزیابی نوزادان طی 48 ساعت اول تولد انجام شد و نتایج به دست آمده از TEOAE (Transient evoked otoacoustic emission) به صورت قبول (Pass) و منفی (Refer) ثبت شد. در صورت غیر طبیعی بودن آزمون، تست 4-2 هفته بعد تکرار شد و اگر هر دو بار نتیجه‌ی TEOAE منفی بود، آزمون ABR (Auditory brainstem response) در سه ماهگی انجام ‌شد و بیمار به متخصص گوش و حلق و بینی ارجاع گردید. یافته‌ها: حدود 90 درصد نوزادان پاسخ مطلوب داشتند. در 10 درصد نوزادان با پاسخ نامطلوب در هفته‌ی دوم تا چهارم این تست تکرار شد که فقط در 11 (6/3 درصد) نوزاد طی مرحله‌ی دوم پاسخ نامطلوب بود. از این تعداد 9 نوزاد جهت ABR مراجعه کردند که فقط 3 نوزاد ABR غیر طبیعی داشتند و کاهش شنوایی دو طرفه در آن‌ها اثبات شد. از این سه نوزاد، یک نوزاد هیچ گونه عامل خطری نداشت. نتیجه‌گیری: با توجه به نتایج مطالعه‌ی حاضر و مطالعات مشابه قبلی بهتر است در موارد مشکوک در هفته دو تا چهارم، غربالگری مجدد انجام شود و در موارد پرخطر برای ناشنوایی از تست قطعی تشخیصی ABR استفاده گردد تا از موارد مثبت و منفی کاذب کاسته شود. ثلث نوزادان با کاهش شنوایی هیچ گونه عامل خطری نداشتند. توصیه می‌شود که ارزیابی همگانی شنوایی شیرخواران حتی در نوزادان بدون عامل خطر نیز با دقت کامل انجام شود. واژگان کلیدی: غربالگری همگان شنوایی نوزادان، Transient evoked otoacoustic emission، Auditory brainstem response، کاهش شنوایی}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13863.html}, eprint = {https://jims.mui.ac.ir/article_13863_f89442bb15f7d6fddcf19cbf06abccdd.pdf} }
