Methods: This randomized prospective trial study was performed in Kashani Hospital (Isfahan, Iran). Sixty patients undergoing primary total knee replacement were randomized to either general anesthesia followed by postoperative intravenous patient-controlled analgesia with morphine, or combined spinal-epidural anesthesia. Pain intensity was evaluated using a verbal rating scale (VRS) at the 30th and 60th minutes in the recovery room, 6, 12, 24, and 48 hours after surgery, at discharge, and two and four weeks after the operation. Pain was also measured at discharge and two and four weeks after the operation. The amount of morphine was also determined during the first 48 postoperative hours. Range of motion was assessed according to the Knee Society Score at discharge and two and four weeks after surgery. All information was recorded in a questionnaire and analyzed by t-test and Mann-Whitney test. Findings: Median pain scores in patients who had received regional anesthesia were consistently lower at 1 (P < 0.001), 6 (P = 0.080), 12 (P = 0.003), 24 (P = 0.140), and 48 hours (P = 0.007) after the operation. Although there was a trend towards fewer complications in the mentioned group, there were no statistically significant differences between the two groups with respect to the incidence of postoperative blood loss, hemodynamic instability, pruritus, nausea, vomiting, urinary retention, or other surgical/medical complications. Patients who had been under regional anesthesia resumed meals earlier (P < 0.001) and showed a trend towards earlier ambulation and hospital discharge. Conclusion: Patients undergoing total knee arthroplasty with regional anesthesia enjoyed better postoperative pain relief and resumed meals earlier than those who received general anesthesia with intravenous patient-controlled analgesia. The former also showed trends towards less adverse effects and postoperative complications. Keywords: Regional anesthesia, General anesthesia, Complication}, keywords = {}, title_fa = {بررسی مقایسه‌ی کیفیت عملکرد و میزان درد پس از تعویض مفصل زانو به دنبال آرتروپلاستی زانو در دو روش بیهوشی عمومی و اسپاینال}, abstract_fa = {مقدمه: هدف از اجرای این مطالعه، تعیین و مقایسه‌ی‌ کیفیت عملکرد و میزان درد پس از عمل تعویض مفصل زانو متعاقب عمل آرتروپلاستی در دو روش بیهوشی عمومی و اسپاینال بود. روش‌ها: این مطالعه‌ی کارآزمایی بالینی بر روی 60 بیمار 20 تا 80 سال که کاندید عمل جراحی الکتیو تعویض مفصل زانو در بیمارستان آیت‌اله کاشانی اصفهان بودند، انجام ‌شد. میزان درد در مفصل با استفاده از روش کلامی (Verbal rating scale یا VRS) در حالت بدون حرکت 30 و 60 دقیقه پس از ورود به ریکاوری و 6، 12، 24 و 48 ساعت پس از عمل جراحی و میزان درد در مفصل در حال حرکت هنگام ترخیص، 2 و 4 هفته پس از عمل ارزیابی شد. میزان مصرف مورفین در 48 ساعت اول پس از عمل و وضعیت فلکسیون زانوی تعویضی و دامنه‌ی حرکت (Range of motion یا ROM) در روز ترخیص (یک هفته بعد از عمل) و در هفته‌ی 2 و 4 توسط جراح بررسی گردید. تجزیه و تحلیل داده‌ها توسط آزمو‌ن‌های Student-t، ANOVA و Mann-Whitney انجام شد. یافته‌ها: در این مطالعه 60 بیمار شامل 55 زن (7/91 درصد) و 5 مرد (3/8 درصد) بررسی شدند. تفاوت معنی‌داری بین میانگین نمره‌ی ROM در مفصل تعویضی، در زمان ترخیص، 2 و 4 هفته پس از عمل در دو گروه وجود نداشت (1/0 = P). میانگین نمره‌ی VRS در مفصل بی‌حرکت 30 و 60 دقیقه پس از ورود به ریکاوری در بیماران تحت بیهوشی عمومی به ترتیب 5/1 ± 2/7 و 3/1 ± 4/6 بود. این اعداد در بیماران تحت بیهوشی اسپاینال به ترتیب 2/1 ± 9/6 و 7/1 ± 1/6 بود. اختلاف بین دو گروه بیماران در این دو زمان معنی‌دار بود (05/0 > P)، ولی در سایر زمان‌ها تفاوت معنی‌داری دیده نشد (05/0 < P). نتیجه‌گیری: طبق نتایج به دست آمده به نظر می‌رسد نوع بیهوشی روی درد بلافاصله بعد از عمل اثرگذار است، ولی پس از 6 ساعت اثری بر میزان درد ندارد؛ بنابراین نوع بیهوشی در این بیماران باید با در نظر گرفتن عوارض کمتر بیهوشی اسپینال و رضایت بیشتر بیماران، انجام شود. واژگان کلیدی: بیهوشی عمومی، بیهوشی اسپاینال، عوارض پس از عمل}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13873.html}, eprint = {https://jims.mui.ac.ir/article_13873_d34ee498eab157453a8aff23263a9d3a.pdf} }
