Type II diabetes and impaired glucose tolerance are seen in 10% and 1.5% of non-obese women with PCOS as well. In addition to the abnormal glucose metabolism, adolescents with PCOS have been shown to have hyperandrogenism and obesity. The aim of this study was to assess hyperinsulinemia in different phenotypes of PCOS in the 14-18 year-old high school girls in Shiraz, Iran. Methods: The present cross-sectional study was conducted on 3200 girls aged 14-18 years in Shiraz high schools in 2009-2010. Information was collected using a questionnaire that included demographic data, clinical manifestations of hyperandrogenism (acne, hirsutism, and alopecia), and oligomenorrhea. We used the ultrasound diagnostic criteria described by Adams et al. for the diagnosis of PCOS. Fasting and 2-hour post prandial blood glucose levels were measured according to the WHO criteria. Findings: The mean post-prandial glucose was higher in patients with different phenotypes of PCOS compared with the non-PCOS individuals. Mean fasting plasma glucose in hyperandrogenic patients with PCOS phenotypes was non-significantly higher than non-PCOS individuals (P > 0.05). 10 patients (3/33%) with clinical phenotype of oligomenorrhea and hyperandrogenism, 14 patients (32.6%) with oligomenorrhea and PCOS, and 18 patients (41.9%) with clinical Hyperandrogenism and PCOS had hyperinsulinemia. Fisher's test in all three PCOS phenotypes and non-PCOS participants had not produced significant values (P > 0.05). 10.3% of subjects (327 cases) were overweight and 6.2% (82 people) were obese. Conclusion: No case of diabetes mellitus was found in our study population and all participants had normal fasting glucose. However, regarding the study population age (14-18 years) and the risk of development of glucose metabolism disorders, lifestyle modification should be encouraged in this group of patients. Keywords: Hyperinsulinism, Polycystic ovary syndrome, Phenotype, Diabetes}, keywords = {}, title_fa = {هیپرانسولینمی در دختران 18-14 ساله‌‌ی مبتلا به فنوتیپ‌های مختلف سندرم تخمدان پلی‌کیستیک}, abstract_fa = {مقدمه: حدود یک سوم بیماران چاق مبتلا به سندرم تخمدان پلی‌کیستیک (Polycystic ovary syndrome یا PCOS) دچار اختلال تحمل گلوکز و 10-5/7 درصد آنان دچار دیابت نوع دو هستند. حتی در زنان غیر چاق مبتلا به این سندرم ده درصد افراد دچار اختلال تحمل گلوکز و 5/1 درصد نیز دچار دیابت نوع دو می‌شوند. این مطالعه با هدف بررسی هیپرانسولینمی در دختران 18-14 ساله‌ی مبتلا به فنوتیپ‌های مختلف PCOS انجام شد. روش‌ها: این مطالعه‌ی مقطعی بر روی 3200 نفر از دانش‌آموزان دختر 18-14 ساله در دبیرستان‌های دخترانه‌ی شیراز انجام شد. دانش‌آموزان مبتلا به هیپرآندروژنیسم بالینی (آکنه، هیرسوتیسم و آلوپسی) و الیگومنوره مشخص شدند. تشخیص PCOS با توجه به ضوابط Adams و همکاران انجام گرفت. آزمایش قند خون ناشتا، قند خون دو ساعته و انسولین خون از همه‌ی دختران مورد مطالعه گرفته شد. داده‌ها با استفاده از نرم‌افزار SPSS نسخه‌ی 5/11 و آزمون 2χ تجزیه و تحلیل شد. یافته‌ها: میانگین تست تحمل گلوکز در افراد مبتلا به فنوتیپ‌های مختلف PCOS در مقایسه با افراد غیر مبتلا بالاتر بود. میانگین قند خون ناشتا در فنوتیپ دارای هیپرآندروژنیسم و PCOS نسبت به افراد غیر مبتلا بالاتر بود، ولی این تفاوت معنی‌دار نبود. 10 نفر (3/33 درصد) از افراد مبتلا با فنوتیپ الیگومنوره و هیپرآندروژنیسم بالینی، 14 نفر (6/32 درصد) از افراد مبتلا به الیگومنوره و PCOS و 18 نفر (9/41 درصد) افراد دارای هیپرآندروژنیسم بالینی و PCOS مبتلا به هیپرانسولینمی بودند که تفاوت معنی‌داری در بین دو گروه مبتلا و غیر مبتلا در هر سه فنوتیپ مشاهده نشد. 327 نفر (3/10 درصد) از افراد دارای اضافه وزن و 82 نفر (6/2 درصد) از افراد چاق بودند. 15 نفر (5/0 درصد) مبتلا به چاقی مرکزی بودند. نتیجه‌گیری: موردی از دیابت ملیتوس در بین جامعه‌ی پژوهش یافت نشد. همه‌ی افراد شرکت‌کننده، قند خون ناشتای طبیعی داشتند، اما با توجه به سن جامعه‌ی پژوهش بهتر است احتمال خطر پیدایش اختلالات متابولیسم گلوکز به افراد مبتلا به PCOS گوشزد شود و لزوم اصلاح شیوه‌‌ی زندگی به آن‌ها یادآوری گردد. واژگان کلیدی: هیپرانسولینمی، سندرم تخمدان پلی کیستیک، دیابت}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13937.html}, eprint = {https://jims.mui.ac.ir/article_13937_dacb07c3d9123e2482561557ee26e7b7.pdf} }
