The signs and symptoms of the disease are more often overlapped with other causes of conjunctivitis. In this study, we applied polymerase chain reaction (PCR) as a sensitive test for diagnosis of adenovirus to determine the prevalence, sensitivity, and specificity of each sign and symptom of adenoviral keratoconjunctivitis. Methods: This descriptive, analytic, cross-sectional study was conducted from September 2011 to September 2012 at Feiz Hospital of Ophthalmology and laboratory of virology, affiliated to Isfahan University of Medical Sciences, Iran. After obtaining ocular specimens from 195 patients with general signs and symptoms of conjunctivitis, their signs and symptoms were recorded. The specimens were tested by PCR to evaluate adenoviral etiology. Prevalence, sensitivity, and specificity of each sign or symptom was measured based on the findings of PCR. Findings: The ocular specimens were collected from 106 (55%) males and 87(45%) females with the mean age of 27.3 ± 7.1 years old. The prevalence of adenoviral keratoconjunctivitis was 11.4 %. There was no significant difference in signs and symptoms between patients with positive and negative adenoviral PCR test. The most sensitive (100%) sign and symptom were conjunctival hyperemia and sudden onset, respectively. The least sensitive (0%) sign and symptom were blepharospasm and membrane/pseudomembrane formation, respectively. The most specific (98.84%) and the least specific (1.16%) were blepharospasm and conjunctival hyperemia, respectively. Conclusion: Although many signs and symptoms accounted for adenoviral conjunctivitis, there was no hallmark for disease diagnosis. Conjunctival hyperemia is a sensitive criterion to diagnose adenoviral keratoconjunctivitis and there was no prevalent specific criterion for this purpose. Keywords: Keratoconjunctivitis, Adenovirus, Sensitivity and specificity, Polymerase chain reaction}, keywords = {}, title_fa = {تعیین میزان شیوع، حساسیت و ویژگی علایم و نشانه‌های بالینی کراتوکونژنکتیویت آدنوویروسی}, abstract_fa = {مقدمه: کراتوکونژنکتیویت آدنوویروسی نوعی عفونت شایع است که به طور عمده ملتحمه و گاهی قرنیه‌ی چشم را درگیر می‌کند. علایم و نشانه‌های این بیماری به طور معمول با علایم و نشانه‌های دیگر انواع کونژنکتیویت هم‌پوشانی دارد. در این مطالعه با استفاده از واکنش زنجیره‌ای پلیمرازی (Polymerase chain reaction یا PCR) به عنوان تست استاندارد شیوع، حساسیت و ویژگی این معیارهای بالینی سنجیده شد. روش‌ها: این مطالعه‌ی توصیفی– تحلیلی مقطعی در طی سال‌های 91-1390 در دانشگاه علوم پزشکی اصفهان انجام شد. 195 نمونه‌ی چشمی از بیماران با تظاهرات عمومی کونژنکتیویت اخذ شد. علایم و نشانه‌های بیماری نیز ثبت گردید. نمونه‌ها به کمک PCR به عنوان استاندارد تشخیصی جهت وجود بقایای آدنوویروسی آزمایش گردید. با استفاده از آمار توصیفی و آزمون‌های حساسیت و ویژگی، داده‌‌ها تحلیل شد. یافته‌ها: تعداد 195 نمونه‌ی چشمی از 106 زن (55 درصد) و 87 مرد (45 درصد) با میانگین سنی 1/7 ± 3/27 سال جمع‌آوری شد. شیوع آدنوویروس در این نمونه‌ها 4/11 درصد بود. شیوع هر علامت و یا نشانه در دو گروه بیماران با تست مثبت و منفی تفاوت معنی‌داری نداشت. حساس‌ترین علامت شروع ناگهانی و حساس‌ترین نشانه پرخونی ملتحمه (هر دو 100 درصد) برآورد شد، در حالی که کمترین مقادیر حساسیت برای بلفارواسپاسم و تشکیل غشا یا غشای کاذب (هر دو صفر درصد) محاسبه گردید. بیشترین ویژگی متعلق به بلفارواسپاسم (84/98 درصد) و کمترین ویژگی از آن پر خونی ملتحمه (16/1 درصد) بود. نتیجه‌گیری: علایم و نشانه‌های متعددی در زمینه‌ی ابتلا به کراتوکونژنکتیویت آدنوویروسی به شمار آمده است که فراوانی آن‌ها اختلاف معنی‌داری با انواع غیر آدنوویروسی نداشتند. پر خونی ملتحمه حساس‌ترین نشانه‌ی ابتلا به انواع آدنوویروسی است. هیچ علامت اختصاصی شایعی در تشخیص این بیماری قابل کاربرد نیست. واژگان کلیدی: کراتوکونژنکتیویت، آدنوویروس، حساسیت و ویژگی، واکنش زنجیره‌ای پلیمرازی}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13949.html}, eprint = {https://jims.mui.ac.ir/article_13949_753cf3d652779820a7e8307d3e61f805.pdf} }
