The aim of this study was to evaluate the efficacy of adding SCB to intravenous sedation and topical anesthesia for phacoemulsification surgery. Methods: After the approval from the institutional ethics committee and obtaining written consents from the patients, 64 adults scheduled for phacoemulsification were enrolled in this double-blind clinical trial. The subjects were randomly allocated to two equal groups of case and control. All patients received similar preoperative preparation, intravenous sedation (1.5 µg/kg fentanyl and 0.15 mg/kg ketamine), and topical anesthesia (0.05% tetracaine) before surgery. Intraoperative additional fentanyl and ketamine was administered as needed. The cases received SCB with 0.1 ml of bupivacaine 0.5% at the site of incision. The controls received equal volume of balanced salt solution. Patient cooperation and satisfaction, severity of intraoperative pain, additional drug requirement, postoperative nausea and vomiting, recovery time and surgeon satisfaction were determined and recorded. Data was analyzed using t-test, and chi-square, Fisher’s exact, and Mann-Whitney tests. P < 0.05 was considered significant. Findings: Demographic data was similar in the two groups. The frequency of excellent patient satisfaction was 28 (87.5%) in the case group and 15 (46.9%) in the control group. The corresponding values were 25 (78.1%) and 18 (56.3%) for patient cooperation and 24 (75.0%) and 19 (59.4%) for surgeon satisfaction (P < 0.05). Intraoperative additional fentanyl requirement, intra- and postoperative severity of pain, and recovery time were significantly lower in the case group than in the control group. Conclusion: This study showed that adding subconjunctival block to intravenous sedation and topical anesthesia in phacoemulsification surgery may enhance patients’ comfort and the conditions of the operation. Keywords: Recovery time, Subconjunctival block, Phacoemulsification}, keywords = {}, title_fa = {بررسی اثر تزریق زیر ملتحمه‌ای بوپیواکایین 5/0 درصد در محل برش جراحی فیکوامولسیفیکاسیون بر میزان درد و رضایتمندی بیمار و جراح و کیفیت ریکاوری}, abstract_fa = {مقدمه: بی‌حسی زیر ملتحمه‌ای در جراحی استرابیسم، سیکلوفوتوکواگولاسیون با لیزر و جراحی گلوکوم مفید گزارش شده است. هدف از این مطالعه، بررسی تأثیر بی‌حسی زیر ملتحمه ای همراه با آرام‌بخشی (Sedation) سیستمیک و بی‌حسی موضعی (Topical) در جراحی فیکوامولسیفیکاسیون بود. روش‌ها: پس از تأیید کمیته‌ی اخلاق و اخذ رضایت کتبی از بیماران 64 بیمار کاندیدای جراحی فیکوامولسیفیکاسیون در این مطالعه‌ی دوسوکور وارد شدند و به طور تصادفی به دو گروه مساوی مورد و شاهد تقسیم گردیدند. همه‌ی بیماران مراقبت قبل از عمل مشابه، آرام‌بخشی وریدی و بی‌حسی موضعی دریافت نمودند. در گروه مورد 1/0 میلی‌لیتر بوپیواکایین 5/0 درصد و در گروه شاهد محلول دارونما در هر محل برش جراحی فیکوامولسیفیکاسیون تزریق شد. رضایتمندی و همکاری بیمار حین عمل، رضایتمندی جراح، شدت درد حین و پس از عمل، نیاز به مسکن اضافه، تهوع و استفراغ بعد از عمل و زمان ریکاوری بررسی و ثبت شد. آنالیز داده‌ها توسط آزمون‌های Mann-Whitney، 2χ، Fisher’s exact و Student-t انجام شد. یافته‌ها: اطلاعات دموگرافیک دو گروه یکسان بود. فراوانی رضایتمندی و همکاری عالی بیمار و رضایتمندی عالی جراح به ترتیب 28 نفر (5/87 درصد) در برابر 15 نفر (9/46 درصد، 25 نفر (1/78 درصد) در برابر 18 نفر (3/56 درصد) و 24 نفر (75 درصد) در مقابل 19 نفر (4/59 درصد) در گروه مورد نسبت به گروه شاهد بود (05/0 > P). فنتاتیل اضافه‌ی مورد نیاز حین جراحی، شدت درد حین و پس از عمل و زمان ریکاوری در گروه مورد کمتر از گروه شاهد بود (05/0 > P). نتیجه‌گیری: این مطالعه نشان داد افزودن بلوک زیر ملتحمه‌ای به آرام‌بخشی وریدی و بی‌حسی موضعی در جراحی فیکوامولسیفیکاسیون می‌تواند رضایت بیمار و شرایط جراحی را بهبود بخشد. واژگان کلیدی: زمان ریکاوری، بی‌حسی زیر ملتحمه‌ای، فیکو امولسیفیکاسیون}, keywords_fa = {}, url = {https://jims.mui.ac.ir/article_13970.html}, eprint = {https://jims.mui.ac.ir/article_13970_697418d32ff89bd8e925ae577264c6c9.pdf} }
