Atelectasis seems to be caused by reduced lung volume and small airway collapse. The collapse of healthy anesthetized and acutely injured lungs of patients is well described and the main mechanical treatment of such collapse conditions is built upon positive end-expiratory pressure (PEEP). The aims of this study were to investigate the hemodynamic and respiratory effect of the adding PEEP in patients with healthy and damaged lungs during mechanical ventilation.Methods: In 24 patients with healthy and 28 patients with damaged lungs at constant ventilation, PEEP was decreased from 5 to 0 cmH2O and then increased to 15 in steps of 5 cmH2O every 10 minutes. We prospectively evaluated the effects of PEEP on the respiratory and hemodynamic indexes of this patients during ventilation and then, SPO2, peak inspiratory pressure, plateau airway pressure, tidal volume, static and dynamic compliance, heart rate and blood pressure (systolic, diastolic, and mean) were recorded.Findings: Increasing PEEP levels from 0 to 15 cmH2O improved oxygenation in both patient with healthy and damaged lungs; but increasing PEEP levels decreased heart rate and systolic, diastolic and mean arterial blood pressure, although this changes were not significant. Increasing PEEP increased respiratory indexes with statistically significant difference between two groups (P < 0.001).Conclusion: Because of increasing the respiratory indexes by increasing PEEP levels in both damaged and healthy lungs without significant hemodynamic effects, this maneuver can be propose to improve oxygenation and respiratory indexes during constant ventilation in both patients with healthy and damaged lungs.}, keywords = {Positive end-expiratory pressure (PEEP),Lung,Recruitment,Airway pressure,Compliance}, title_fa = {مقایسه‌ی اعمال سطوح مختلف فشار مثبت انتهای بازدمی بر مؤلفه‌های همودینامیک و تنفسی در بیماران با ریه‌ی سالم و آسیب‌دیده}, abstract_fa = {مقدمه: حین تهویه‌ی مکانیکی امکان بروز آتلکتازی در مناطقی از ریه وجود دارد. آتلکتازی در اثر کاهش حجم ریه و کلاپس راه‌های هوایی کوچک ایجاد می‌شود. درمان اصلی کلاپس ایجادشده در بیماران تحت بیهوشی با ریه‌ی سالم و آسیب حاد ریوی بر اساس اعمال فشار مثبت انتهای بازدمی (Positive end expiratory pressure یا PEEP) استوار می‌باشد. هدف از انجام این مطالعه تعیین تأثیر اعمال PEEP در درجات مختلف (0، 5، 10، 15 سانتی‌متر آب) بر اصلاح آتلکتازی‌های موجود و پاسخ ریه‌ی معیوب و ریه‌ی سالم به این مانور‌ها بود.روش‌ها: بیماران تحت تهویه‌ی مکانیکی طی یک کارآزمایی بالینی به دو دسته، 24 بیمار با ریه‌ی سالم (گروه الف) و 28 بیمار با ریه‌ی آسیب‌دیده (گروه ب) تقسیم شدند. سپس سطوح مختلف PEEP هر یک به مدت 10 دقیقه اعمال شد. در ابتدا و در پایان هر 10 دقیقه مشخصه‌های تنفسی و همودینامیک از قبیل ظرفیت (Compliance) استاتیک و دینامیک، SPo2 (Oxygen saturation)، Peak airway pressure، Pleteau airway pressure، Expiratory VT، فشار خون سیستول و دیاستول و ضربان قلب اندازه‌گیری و ثبت گردید.یافته‌ها: افزایش سطح PEEP از 0 تا 15 سانتی‌متر آب، منجر به افزایش SPo2، کاهش میزان ضربان قلب و فشار خون سیستولی و دیاستولی شد، اگر چه میزان این تغییرات از نظر آماری معنی‌دار نبود، ولی شاخص‌های تنفسی در هر دو گروه سیر صعودی داشت و تفاوت بین دو گروه در شاخص‌های مذکور معنی‌دار بود (001/0 > P).نتیجه‌گیری: به دلیل افزایش شاخص‌های تنفسی در اثر افزایش سطوح PEEP، بدون اثرگذاری مشخص بر فاکتورهای همودینامیک، در هر 2 گروه از بیماران با ریه‌های سالم و آسیب‌دیده، انجام این مانور جهت بهبود وضعیت اکسیژن‌رسانی و شاخص‌های تنفسی در بیمارانی که تحت ونتیلاسیون طولانی مدت قرار دارند، توصیه می‌شود.}, keywords_fa = {ظرفیت,فشار مثبت انتهای بازدمی,Lung recruitment,Airway pressure}, url = {https://jims.mui.ac.ir/article_14083.html}, eprint = {https://jims.mui.ac.ir/article_14083_0b251557268a118cd3a309b9dfd9d616.pdf} }
