The aim of this study was to survey the prevalence of shigatoxigenic strains and virulence genes in children of the age of under 5-years in Yasuj, Iran.Methods: This cross-sectional study was performed on 300 stool samples taken from children of the age of under 5-years with diarrhea in Yasuj. After initial identification of E. coli strains by culture and biochemical tests, shiga toxigenic Escherichia coli (STEC) genes such as eaeA, stx1 and stx2 detected by multiplex polymerase chain reaction (PCR) technique and antibiotic susceptibility of isolates was evaluated using disc diffusion method under the guidelines of Clinical and Laboratory Standards Institute (CLSI).Findings: Out of the samples, 104 (34.7%) STEC genes were separated. Out of considered virulence genes, 14 cases (13.46%) had stx1, 31 cases (29.81%)stx2, 9 cases (8.65%) genotype stx1-stx2, 12 cases (11.54%) eaeA-stx1, 25 cases (24.04%) eaeA-stx2 and 13 cases (12.5%) had third eaeA-stx1-stx2 genes. Antibiotic susceptibility test showed that the most susceptible antibiotic was imipenem for STEC; the most resistant antibiotic was ceftizoxime.Conclusion: Results showed that STEC strains have high prevalence in our study area. Therefore, hospital-wide surveillance using molecular techniques should be proposed in other regions of our country.}, keywords = {Shigatoxigenic Escherichia coli,Multiplex PCR,Antibiotic Resistance}, title_fa = {شناسایی مولکولی و مقاومت آنتی بیوتیکی سویه‌های تولید کننده‌ی شیگاتوکسین اشریشیاکلی در کودکان زیر 5 سال شهر یاسوج}, abstract_fa = {مقدمه: اشریشیاکلی‌های تولید کننده‌ی شیگاتوکسین از مهم‌ترین عوامل ایجاد کننده‌ی اسهال و همچنین بیماری‌هایی نظیر سندرم اورمی همولیتیک (HUS یا Hemolytic uremic syndrome) و کولیت هموراژیک (HC یا Hemorrhagic colitis) می‌باشند. هدف از این پژوهش، بررسی شیوع سویه‌های تولید کننده‌ی شیگاتوکسین و ژن‌های بیماری‌زای آن در کودکان زیر 5 سال شهر یاسوج می‌باشد.روش‌ها: این پژوهش به صورت مقطعی- توصیفی بر روی 300 نمونه‌‌ی مدفوع اسهالی از کودکان زیر 5 سال شهرستان یاسوج انجام شد. پس از تشخیص اولیه‌ی باکتری اشریشیاکلی با استفاده از روش‌های بیوشیمیایی، ویروتیپ‌ (STEC یا Shiga toxigenic Escherichia coli) با ارزیابی ژن‌های eaeA، 1stx و 2stx توسط تکنیک PCR (Polymerase chain reaction) چندگانه شناسایی و حساسیت آنتی‌بیوتیکی سویه‌ها با روش دیسک دیفیوژن (CLSI یا Clinical and laboratory standards institute) مورد بررسی قرار گرفت.یافته‌ها: در نمونه‌های مورد پژوهش، فراوانی ویروتیپ‌ STEC، 104 مورد (7/34 درصد) بود. از سویه‌های شناسایی شده 14 مورد (5/13 درصد) ژن 1stx، 31 مورد (8/29 درصد) ژن 2stx، 9 مورد (7/8 درصد) ژن‌های 2stx-1-stx، 12 مورد (5/11 درصد) ژن‌های 1stx eaeA-، 25 مورد (24 درصد) ژن‌های 2eaeA-stx و 13 مورد (5/12 درصد) هر سه ژن 2stx-1stx-eaeA را دارا بودند. با بررسی مقاومت آنتی ‌بیوتیکی مشخص شد که سویه‌های جداسازی شده، بیشترین میزان حساسیت را به آنتی ‌بیوتیک ایمی‌پنم و بیشترین مقاومت را به آنتی ‌بیوتیک سفتی زوکسیم دارند.نتیجه‌گیری: سویه‌های اشریشیاکلی تولید کننده‌ی شیگاتوکسین در منطقه‌ی مورد پژوهش شیوع گسترده‌ای دارند. با توجه به تنوع ژنتیکی سویه‌های جداسازی شده، پایش گسترده‌ی بیمارستانی این ویروتیپ با استفاده از روش‌های دقیق مولکولی در سایر مناطق کشور پیشنهاد می‌گردد.}, keywords_fa = {اشریشیاکلی تولید کننده‌ی شیگاتوکسین,Polymerase chain reaction چندگانه,مقاومت آنتی‌بیوتیکی}, url = {https://jims.mui.ac.ir/article_14264.html}, eprint = {https://jims.mui.ac.ir/article_14264_bccbbf23925cd66563c5b8a064acc821.pdf} }
