This study was performed to compare two scoring systems, Acute Physiology and Chronic Health Evaluation (APACHE) IV and Simplified Acute Physiology (SAPA) III, in predicting mortality rate in intensive care units (ICU).Methods: In this retrospective cross-sectional study, we calculated 60 patient during the year 2012 admitted to ICU wards in Shahid Rajaei hospital, Qazvin, Iran. APACHE IV and SAPA III scores and predicted mortality were calculated. Then, the real mortality in intensive care units were recorded. Using the area under the curve of receiver operating characteristic (ROC), performance and differentiation of these models to predict mortality were assessed. Data were analyzed using Mann-Whitney test at the significant level of P-value of less than 0.05.Findings: Of 74 patients admitted to the intensive care unit, 60 were included in the study, 38 men and 22 women. The mean age of patients was 59.0 years and the average duration of hospital staying was 12.10 days; 11 patients died. The mean scores of APACHE IV and SAPA III were 30.0 ± 3.4 and 15.0 ± 2.0, respectively. The area under ROC curve was 0.990 for APACHE IV with confidence interval of 0.930 to 1.008 and 0.97 for SAPA III with confidence interval 0.97-1.008with a significant difference (P < 0.001).Conclusion: Our study revealed that APACHE IV scoring method predicted the mortality rates in intensive care units better than SAPSIII method.}, keywords = {Intensive Care Unit,Scoring systems,Acute physiology and chronic health evaluation (APACHE) IV score,Simplified acute physiology (SAPA) III score,Predicted mortality}, title_fa = {مقایسه‌ی دو سیستم نمره‌دهی 3Simplified acute physiology score (3SAPS) و 4Acute physiology and chronic health evaluation (4APACHE) در پیش‌گویی مرگ و میر بیماران بستری در بخش مراقبت‌های ویژه}, abstract_fa = {مقدمه: پیش‌بینی مرگ در بخش‌ مراقبت‌های ویژه، با استفاده از سیستم‌های نمره‌دهی مختلفی که شدت بیماری را می‌سنجند، صورت می‌گیرد. این بررسی جهت ارزیابی سیستم امتیازدهی 4APACHE (4Acute physiology and chronic health evaluation) و 3SAPS (3Simplified acute physiology score) در پیش‌گویی میزان مرگ و میر بیماران بستری در بخش مراقبت‌های ویژه انجام شد.روش‌ها: در این مطالعه‌ی مقطعی، تعداد 60 بیمار که از خرداد تا آذر سال 1391 در بخش مراقبت‌های ویژه‌ی مرکز آموزشی- درمانی شهید رجایی بستری شده بودند، مورد بررسی قرار گرفتند. نمرات 4APACHE و 3SAPS و نیز میزان مرگ و میر پیش‌بینی شده بر اساس این مدل‌ها و میزان مرگ و میر واقعی در این بیماران بررسی شد. سپس با محاسبه‌ی مساحت سطح زیرمنحنی ROC (Receiver operating characteristic) میزان کارایی و تمایز این مدل‌ها برای پیش‌بینی مرگ و میر بررسی شد. داده‌ها با استفاده از نرم‌افزار SPSS نسخه‌ی 16 و آزمون t با هم مقایسه شدند. 050/0 > P معنی‌دار در نظر گرفته شد.یافته‌ها: از 74 بیمار بستری در بخش مراقبت‌های ویژه، 60 نفر شرایط ورود به مطالعه را داشتند که 38 نفر مرد و 22 بیمار زن بودند. میانگین سنی بیماران 59 سال و متوسط زمان بستری 12/10 روز بود که از این تعداد، 11 بیمار فوت کردند. میانگین و انحراف معیار نمرات 4APACHE 0/2 ± 0/15 و 3SAPS 4/3 ± 0/30 بود. فاصله‌ی اطمینان مساحت زیر منحنی ROC در 4APACHE (008/1-930/0) 99/0 و در 3SAPS (008/1-970/0) 97/0 بود و آزمون t اختلاف معنی‌داری را بین این دو میانگین نشان داد (001/0 > P).نتیجه‌گیری: این مطالعه نشان داد که مدل 4APACHE در پیش‌گویی مرگ و میر بیماران در بخش مراقبت‌های ویژه دارای قدرت بیشتری نسبت به مدل 3SAPS می‌باشد. }, keywords_fa = {بخش مراقبت‌های ویژه,سیستم نمره‌دهی,4Acute physiology and chronic health evaluation,3Simplified acute physiology score,پیش‌گویی مرگ و میر}, url = {https://jims.mui.ac.ir/article_14280.html}, eprint = {https://jims.mui.ac.ir/article_14280_65a3eca1f3d45256ee576a2406afdc5b.pdf} }
