R. Iran) as family physician program, in rural areas and urban regions of bellow-20-thousands population from 2005. The aim of this study was comparing the main reproductive health indicators before and after implementation of family physician program.Methods: In a trend study in 2012, we analyzed the reproductive health indices from 2001 to 2011 comparing indices in two intervals, 4 years prior and 6 years after introducing the family physician program in the Isfahan province, Iran. In the rural areas of Iran, personal, social and health data are collected on each person in the community covered by health houses and then are collected by health centers in which family physicians teams are presented. Afterward, data are gathered and assessed in district health centers. These data had been drawn from vital data software. This software existed in every Deputy Director at the District Health Centers. We assessed indicators such as neonatal mortality rate (NMR), infant mortality rate (IMR), under-5-year mortality rate (U5MR), and the percentage of the deliveries carried out by skilled attendance in hospital.Findings: The NMR decreased from 13.35 per 1000 live births to 8.8. The IMR decreased from 17.86 per 1000 live births to 14.2. The U5MR decreased from 21.06 per 1000 live births to 17.4.Conclusion: Following family physician program measurements, the main reproductive health indices such as NMR, IMR and U5MR have been decreased markedly in rural areas of the Isfahan province.}, keywords = {Health sector reform,Family physician,Reproductive health indicator,Iran,Developing Country}, title_fa = {مقایسه‌ی شاخص‌های اصلی سلامت در مناطق روستایی استان اصفهان قبل و بعد از اجرای برنامه‌ی پزشک خانواده}, abstract_fa = {مقدمه: اصلاح نظام سلامت در ایران، تحت عنوان برنامه‌ی پزشک خانواده، در کل مناطق روستایی و شهرهای با جمعیت کمتر از بیست هزار نفر، از سال 1384به اجرا گذاشته شد. هدف از این مطالعه، مقایسه‌ی شاخص‌های اصلی سلامت قبل و بعد از اجرای برنامه‌ی پزشک خانواده بود.روش‌ها: در این مطالعه که در سال 1391 به صورت Trend analysis انجام شد، شاخص‌های اصلی سلامت، طی یک دوره‌ی 10 ساله، از سال 1381 تا 1390 طی دو دوره‌ی زمانی، یک دوره‌ی زمانی 4 ساله قبل از اجرای برنامه‌ی پزشک خانواده و یک دوره‌ی 6 ساله بعد از اجرای این برنامه در استان اصفهان، مورد تجزیه و تحلیل قرار گرفت. داده‌های حیاتی در خانه‌های بهداشت و مراکز بهداشتی- درمانی روستایی، در فرم استانداردی به نام زیج حیاتی گردآوری و طبقه‌بندی می‌شود. این داده‌ها در مراکز بهداشت شهرستان، وارد نرم‌افزار مخصوص زیج حیاتی شهرستان می‌شود. این مطالعه، با بررسی اطلاعات موجود در نرم‌افزار شاخص‌های حیاتی برگرفته از زیج‌های حیاتی مناطق روستایی استان اصفهان، در واحد آمار و رایانه‌ی مرکز بهداشت استان، اجرا شد.یافته‌ها: میزان مرگ نوزادی (NMR یا Neonatal mortality rate) از 23/16 در هزار نفر تولد زنده (قبل از اجرای برنامه‌ی پزشک خانواده) به 8/8 در هزار نفر (بعد از اجرای برنامه‌ی پزشک خانواده) کاهش یافته بود. میزان مرگ شیرخواران (IMR یا Infant mortality rate) از میزان 64/21 در هزار تولد زنده، به 20/14 کاهش یافته بود. همچنین میزان مرگ کودکان زیر 5 سال (MR5U یا Under-5-year mortality rate)، از 33/26 در هزار نفر تولد زنده تا حد 40/17 پایین آمده بود.نتیجه‌گیری: به دنبال اجرای برنامه‌ی پزشک خانواده در مناطق روستایی استان اصفهان، کاهش قابل ملاحظه‌ای در شاخص‌های اصلی سلامتی مانند میزان‌های مرگ نوزادان، مرگ شیرخواران و مرگ کودکان زیر 5 سال مشاهده می‌شود.}, keywords_fa = {اصلاح نظام سلامت,پزشک خانواده,میزان مرگ و میر نوزادی,میزان مرگ و میر شیرخواران,میزان مرگ و میر کودکان کمتر از 5 سال}, url = {https://jims.mui.ac.ir/article_14341.html}, eprint = {https://jims.mui.ac.ir/article_14341_a7de358d0efbdf36dd5d293251408e6d.pdf} }
