It is intrinsically resistance to many antibiotics. Nowadays, carbapenem is the last drug to be used for the treatment of infection of multidrug-resistant (MDR) Acinetobacter baumannii. Carbapenem-resistance in Acinetobacter baumannii strains is also expanding and in turn. The present study aimed to assess the frequency of metallo-β-lactamase (MBL) and antimicrobial resistance patterns of Acinetobacter baumannii in carbapenem-resistant isolates from intensive care units (ICUs).Methods: In a cross-sectional study during 2012-2013, Acinetobacter baumannii isolates from clinical specimens of patients hospitalized in the intensive care units (ICU) in hospitals of Isfahan city, Iran, were identified using genetic and biochemical methods. The susceptibility of isolates was determined via standard disk diffusion method according to the Clinical and Laboratory Standards Institute (CLSI). Metallo-β-lactamase-producing isolates were identified using Double Disc Synergy Test (DDST).Findings: 100 isolates were determined as Acinetobacter baumannii. The antimicrobial patterns of isolates showed that 62% of isolates were resistant to amikacin, 88% to tetracycline, 92% to ceftazidime, 96% to imipenem and meropenem, 93% to ampicillin-sulbactam, and 98% to ciprofloxacin, trimethoprim-sulfamethoxazole and cefepime. From 96 non-susceptible Acinetobacter baumannii strains imipenem, 95 (97.9%) were found to produce metallo-β-lactamase.Conclusion: To prevent spreading of the nosocomial infections caused by Acinetobacter baumannii in intensive care units, the rapid and accurate report of metallo-β-lactamase seems to be necessary in order to better monitor and more accurate tracking of multidrug-resistant strains.}, keywords = {Acinetobacter baumannii,Metallo-β-lactamase (MBL),Antibiotic resistance pattern,Intensive Care Unit}, title_fa = {فراوانی متالوبتالاکتاماز و تعیین الگوی مقاومت دارویی سویه‌های اسینتوباکتربومانی مقاوم به کارباپنم، ایزوله شده از بخش‌های مراقبت‌های ویژه‌ی بیمارستان‌های شهر اصفهان}, abstract_fa = {مقدمه: اسینتوباکتر بومانی یک باکتری گرم منفی غیر تخمیری به صورت کوکسی یا کوکوباسیل بی‌حرکت است که اغلب در خاک، منابع آبی مختلف و بسیاری از محیط‌های بهداشتی- درمانی یافت می‌شود. این باکتری دارای مقاومت ذاتی بـه بسـیاری از آنتـی‌بیوتیـک‌هـا اسـت. امروزه از کارباپنم‌ها به عنوان آخرین داروی درمانی مناسب برای درمان عفونت MDR (Multiple drug resistance) اسینتوباکتربومانی استفاده می‌شود. مقاومت به کارباپنم‌ها نیز در میان سویه‌های اسینتوباکتربومانی در حال گسترش می‌باشد که این امر، سبب بحران مقاومت دارویی در میان سویه‌های اسینتوباکتربومانی می‌شود. هدف از این مطالعه، بررسی فراوانی متالوبتالاکتاماز و تعیین الگوی مقاومت دارویی سویه‌های اسینتوباکتربومانی مقاوم به کارباپنم ایزوله شده از بخش مراقبت‌های ویژه‌ (ICU یا Intensive care unit) بود.روش‌ها: در یک مطالعه‌ی مقطعی در سال‌های 92-1391 جدایه‌های اسینتوباکتربومانی از نمونه‌های کلینیکی ﺑﻴمـﺎران بستری بخش ICU ﺑـﻪ روش بیوشیمیایی و ژنتیکی ﺗﻌﻴﻴﻦ ﻫﻮیت شدند. ﺳﭙﺲ اﻟﮕﻮی ﻣﻘﺎوﻣﺖ آﻧﺘﻲ‌ﺑﻴﻮﺗﻴﻜﻲ آن‌ﻫﺎ ﺑﻪ روش ﻛﺮبی- ﺑاﺋﺮ (Kirby-Bauer) ﺗﻌﻴـﻴﻦ ﮔﺮدید. ﺳـﻮﻳﻪ‌ﻫـﺎی ﺗﻮﻟﻴﺪ ﻛﻨﻨﺪه‌ی ﻣﺘﺎﻟﻮﺑﺘﺎﻻﻛﺘﺎﻣﺎز، ﺑﻪ روش DDST (Double disk synergy test) ﺷﻨﺎﺳﺎﻳﻲ ﺷـﺪﻧﺪ.یافته‌ها: 100 سویه‌ی اسینتوباکتربومانی با آزمایش‌های فنوتیپی و مولکولی از نمونه‌های کلینیکی به دست آمد. میزان فراوانی الگوهای ضد میکروبی جدایه‌ها نشان داد که 62 درصد ایزوله‌ها به آمیکاسین، 88 درصد به تتراسایکلین، 92 درصد به سفتازیدیم، 96 درصد به ایمی‌پنم و مروپنم، 93 درصد به آمپی‌سیلین- سولباکتام و 98 درصد به سیپروفلوکسازین و تری‌متوپریم- سولفامتوکسازول و سفپیم مقاوم بودند. با آزمایش DDST از 96 سویه‌ی اسینتوباکتربومانی غیر حساس به ایمی‌پنم، 95 سویه (9/97 درصد) تولید کننده‌ی MBL (Metallo-β-lactamase) بودند.نتیجه‌گیری: به منظور جلوگیری از گسترش عفونت‌های بیمارستانی بخش ICU ناشی از اسینتوباکتربومانی، ﮔﺰارش ﺳـﺮﻳﻊ و دﻗﻴﻖ آنزیم‌های متالوبتالاکتاماز جهت ﻧﻈـﺎرت ﻫـﺮ ﭼـﻪ ﺑﻬﺘـﺮ و دﻗﻴـﻖ‌ﺗـﺮ و ردﻳـﺎﺑﻲ ﻣﻘﺎوﻣﺖﻫﺎی ﭼﻨﺪﮔﺎﻧـﻪ‌ی اسینتوباکتربومانی در آزمایشگاه‌های بیمارستان‌ها ضروری به نظر می‌رسد. }, keywords_fa = {اسینتوباکتربومانی,Mannose-binding lectin,مقاومت دارویی,Intensive Care Unit}, url = {https://jims.mui.ac.ir/article_14499.html}, eprint = {https://jims.mui.ac.ir/article_14499_fbca891130a89d4d742e539389bad245.pdf} }
