Care of the patients with brain death is important as these patients are important sources for transplantation. Due to changes made in the brain and other organs, electrolyte, hemodynamic and hemostatic abnormalities have a high prevalence in patients with brain death. The recognition of these disorders is very important to have a correct view about the importance of prevention and management. The aim of this study was assessment of the prevalence of hemodynamic, acid and base, coagulation, electrolytes, and blood glucose abnormalities in patients with brain death on arrival at hospital intensive care unit (ICU).Methods: In a cross-sectional study during 2013-2014, the medical records of 119 patients with brain death who were hospitalized in ICUs of Alzahra Hospital, Isfahan, Iran, during 2010-2013 were studied. Hemodynamic, coagulation, electrolytes and glucose abnormalities, and the need for inotropic support for each patient were recorded on the data collection form. The frequency of organ donation in patients was assigned and the obtained data were analyzed using SPSS software.Findings:  In 92 patients (77.3%) organ donation was done. Impairment of pH (70.6%), prothrombin time (PT) (66.4%) and international normalized ratio (INR) (58.0%) were the most common disorders seen in the patients. Impaired glucose and sodium levels were statistically significant in organ donation and not-donation groups.Conclusion: Patients with brain death, due to disturbances in hemodynamic, hemostatic, and coagulation time, lose the ability of organ donation. So, the decision to do organ donation should be made in the shortest possible time.}, keywords = {Brain death,Donation,Coagulation disorder}, title_fa = {بررسی فراوانی اختلالات همودینامیک، اسید و باز، انعقادی، الکترولیتی، قند خون و نیاز به اینوتروپ در بیماران دچار مرگ مغزی در بدو ورود به بخش مراقبت‌های ویژه}, abstract_fa = {مقدمه: با توجه به تغییرات ایجاد شده در مغز و سایر اعضای بدن به دنبال مرگ مغزی، اختلالات الکترولیتی، همودینامیکی و هموستاتیک در بیماران دچار مرگ مغزی شیوع بسیار بالایی دارد. بنابراین، شناخت این اختلالات جهت داشتن دیدگاهی صحیح در مورد پیش‌گیری و مدیریت آن‌ها اهمیت به‌سزایی دارد. از این رو، مطالعه‌ی حاضر با هدف بررسی فراوانی اختلالات همودینامیک، اسید و باز، انعقادی، الکترولیتی، قند خون و نیاز به اینوتروپ در بیماران دچار مرگ مغزی بستری شده در ICU (Intensive care unit) به انجام رسید.روش‌ها: این مطالعه‌ی توصیفی- تحلیلی در سال 1393 در بیمارستان الزهرا (س) اصفهان انجام شد و طی آن، 119 بیمار دچار مرگ مغزی در سال‌های 92-1389 با استفاده از شماره پرونده‌های موجود در مدارک پزشکی بیمارستان مورد مطالعه قرار گرفتند. وجود اختلالات همودینامیک، اسید و باز، انعقادی، الکترولیتی، قند خون و نیاز به اینوتروپ برای هر بیمار تعیین و در فرم جمع‌آوری اطلاعات ثبت شد. اطلاعات در نرم‌افزار SPSS ثبت گردید و با استفاده از آزمون‌های 2χ و t مورد واکاوی قرار گرفت.یافته‌ها: در92 نفر از بیماران (3/77 درصد)، اهدای عضو صورت گرفته بود. اختلال در pH (6/70 درصد)، اختلال در PT (Prothrombin time) (4/66 درصد) و اختلال در INR (International normalized ratio) (0/58 درصد) شایع‌ترین اختلالات مشاهده شده در این بیماران بود. اختلال در سطح سدیم و قند خون در دو گروه با اهدای عضو و عدم اهدای عضو، اختلاف معنی‌دار داشت.نتیجه‌گیری: بیماران دچار مرگ مغزی به علت بروز اختلالات مختلف در همودینامیک، هموستاتیک و اختلالات انعقادی، به مرور زمان قابلیت اهدای عضو را از دست می‌دهند و از آن جایی که درصد قابل توجهی از بیماران دچار مرگ مغزی از مراکز درمانی دیگر جهت اهدای عضو به بیمارستان الزهرا (س) ارجاع می‌گردند، طولانی شدن زمان بروز مرگ مغزی تا زمان تصمیم به جداسازی عضو، ممکن است به از بین رفتن اعضای کاندیدای اهدا نظیر کلیه، کبد و قلب گردد. از این رو، تصمیم‌گیری به اهدای عضو بایستی در حداقل زمان ممکن صورت گیرد.}, keywords_fa = {مرگ مغزی,اهدای عضو,اختلال انعقادی}, url = {https://jims.mui.ac.ir/article_14650.html}, eprint = {https://jims.mui.ac.ir/article_14650_e7ae3f94230dbc0b7b0bca97cf2988a9.pdf} }
