Thus, the aim of this study was to determine re-hospitalization frequency and related factors in patients discharged in less than 12 hours from Alzahra hospital emergency service in Isfahan, Iran.Methods: This is an applied cross sectional study of patients who referred to Alzahra hospital emergency department in Isfahan during 2015 spring and summer. In this study, 400 hospital records were randomly selected and studied for re-hospitalization. Demographic and other information including the interval between two referrals, cause of the first and second hospitalization, kind of discharge and other needed information were collected. If the causes of primary and secondary referrals were the same and the interval between the two was also less than one week, it was regarded as incomplete treatment. Finally, the data was analyzed by SPSS software.Findings: 54 patients (13.5%) had re-hospitalization and the mean interval between the two referrals was 7.4 ± 5.8 days. In 30 of these patients (55.6%), the second referral was in less than 7 days. Re-hospitalization was different based on patients’ age and more frequent in those with less than 50 years. This was also different based on their job, education and types of discharge in the first admit.Conclusion: A considerable part of patients who referred to emergency ward had re-hospitalization which was related to primary diseases in almost 50% of them and this repeated refers leads to emergency overcrowding and increase of hospital cost. On the other hand, a part of this repeated hospitalization is due to incorrect management of patients in emergency ward which is due to factors related to patients and the hospital. Thus, patient who referred to emergency ward must be managed and diagnosed correctly and receive the required treatment on time.}, keywords = {Emergency ward,Re-hospitalization,Repeated hospitalization}, title_fa = {بررسی توزیع فراوانی مراجعه‌ی مجدد و عوامل مرتبط با آن در بیماران ترخیص شده طی 12 ساعت از اورژانس مرکز آموزشی- درمانی الزهرای (س) اصفهان در نیمه‌ی اول سال 1394}, abstract_fa = {مقدمه: عوامل مختلفی در شلوغی اورژانس بیمارستان‌ها دخیل هستند. یکی از این عوامل که با وجود تأثیر احتمالی بالا، کمتر مورد توجه قرار گرفته است، مراجعه‌ی مجدد بیماران ترخیص شده از بخش اورژانس می‌باشد. از آن جایی که مطالعه‌ای در این زمینه انجام نشده بود، این پژوهش با هدف تعیین توزیع فراوانی مراجعه‌ی مجدد و عوامل مرتبط با آن، به انجام رسید.روش‌ها: این مطالعه‌ی تحلیلی- کاربردی در نیمه‌ی اول سال 1394 در بخش اورژانس بیمارستان الزهرای (س) اصفهان انجام گرفت. جامعه‌ی آماری مورد مطالعه، بیماران مراجعه کننده به اورژانس این بیمارستان در نیمه‌ی اول سال 1394 بودند. در این مطالعه، 400 پرونده از مراجعات به اورژانس بیمارستان به صورت تصادفی انتخاب شدند و مراجعه‌ی مجدد آن‌‌ها به اورژانس، به همراه اطلاعات دموگرافیک و سایر اطلاعات، شامل فاصله‌ی زمانی بین دو مراجعه، علت اولیه و علت مراجعه‌ی مجدد، نوع ترخیص در اولین مراجعه و دیگر اطلاعات از آن‌ها استخراج و در پرسش‌نامه‌ی ویژه ثبت گردید. در صورتی که علت مراجعه‌ی مجدد، همان علت مراجعه‌ی اولیه و زمان مراجعه‌ی مجدد کمتر از یک هفته بود، به عنوان نقص درمان در نظر گرفته شد و علت آن بررسی و نتیجه، در مجموعه اطلاعات هر بیمار ثبت شد. اطلاعات جمع‌آوری شده، با استفاده از نرم‌افزار SPSS تجزیه و تحلیل گردید.یافته‌ها: 54 نفر (5/13 درصد) از بیماران مراجعه‌ی مجدد داشتند. میانگین فاصله‌ی زمانی بین دو مراجعه، 8/5 ± 4/7 روز بود که در 30 نفر (6/55 درصد) کمتر از 7 روز و در 24 نفر (4/44 درصد) 7 روز و بیشتر بود. فراوانی مراجعه‌ی مجدد بر حسب گروه سنی بیماران اختلاف معنی‌دار داشت و درصد بیشتری از بیماران زیر 50 سال، دارای مراجعه‌ی مجدد بودند. همچنین، فراوانی مراجعه‌ی مجدد بر حسب شغل، تحصیلات و نوع ترخیص در اولین مراجعه اختلاف معنی‌دار داشت.نتیجه‌گیری: درصد قابل توجهی از بیماران مراجعه کننده به اورژانس بیمارستان الزهرای (س) اصفهان دارای مراجعه‌ی مجدد هستند که حدود نیمی از این مراجعات تکراری، به علت بیماری اولیه بوده است. این امر، علاوه بر افزایش ترافیک بیمار در بخش اورژانس، منجر به هدر رفت منابع مالی، انسانی و تجهیزاتی بیمارستان می‌شود و بار اقتصادی بیشتری را نیز بر بیمار تحمیل می‌نماید. با توجه به این که بخشی از مراجعات مجدد بیماران به علت مدیریت ناصحیح بیمار در بخش اورژانس است و عوامل مختلفی نظیر عوامل مربوط به بیمار و عوامل مربوط به بیمارستان در آن دخیل هستند، لازم است تدابیر لازم در این زمینه اندیشیده شود و بیماران مراجعه کننده به بخش اورژانس، طوری مدیریت شوند که خدمات تشخیصی و درمانی مورد نیاز را در اولین مراجعه دریافت نمایند.}, keywords_fa = {اورژانس,مراجعه‌ی مجدد,علت مراجعه}, url = {https://jims.mui.ac.ir/article_14881.html}, eprint = {https://jims.mui.ac.ir/article_14881_3ddfb7b56d410fde106b0406c790806a.pdf} }
