In this study, the authors investigated the analgesic effects of intravenous paracetamol, while aiming to minimalize systemic side effects.Methods: In this prospective, randomized and triple-blinded clinical trial, patients were studied in two groups: a control group (n = 42) and a group treated with paracetamol (n = 51). The paracetamol group received an intravenous injection of the drug 15 minutes before induction of anesthesia and then every 6 hours for 3 days, 1 g paracetamol in 50 ml normal saline was infused in 15 minute. In the control group, normal saline was used instead of paracetamol in all stages of study in the same volume.Findings: Reported pain severities during resting [after 24 (P = 0.002) hour] and deep breathing [after 12 (P = 0.001), 24 (P < 0.001) and 72 (P = 0.031) hour] were significantly lower in the paracetamol group. There was no significant difference between the two groups, in terms of the amount of opioid requirements during (P = 0.246) and after (P = 0.853) surgery.Conclusion: Preemptive and then continuous prescription and use of paracetamol after cardiac surgery improves the quality of postoperative pain control of opioids. However, the potency of paracetamol alone is not enough to reduce the required dose and the side effects of opioid and sedative drugs.}, keywords = {Paracetamol,Pain,Coronary Artery Bypass Graft}, title_fa = {بررسی تأثیر تجویز پیش‌دستانه‌ی پاراستامول وریدی بر میزان درد حین و بعد ازعمل جراحی پیوند عروق کرونر قلب}, abstract_fa = {مقدمه: رایج‌ترین روش کنترل درد بیماران پس از عمل جراحی قلب، استفاده از داروهای مخدر و داروهای ضد التهاب غیر استروئیدی می‌باشد. این مطالعه، با هدف بررسی اثرات بی‌دردی داروی پاراستامول وریدی که دارای حداقل عوارض سیستمیک می‌باشد، انجام شد.روش‌ها: در این کارآزمایی بالینی آینده‌نگر تصادفی و سه سو کور، بیماران در دو گروه مورد (پاراستامول) و شاهد قرار گرفتند. در گروه مورد (51 نفر) 15 دقیقه قبل از القای بیهوشی و سپس، هر 6 ساعت به مدت 3 روز، 1 گرم پاراستامول در 50 سی‌سی سرم نرمال سالین در مدت 15 دقیقه تزریق شد. در گروه شاهد (42 نفر) در کلیه‌ی مراحل تزریق، به جای داروی پاراستامول از نرمال سالین استفاده گردید.یافته‌ها: شدت درد در حالت استراحت در ساعت 24 (002/0 = P) و به دنبال دم عمیق در ساعت‌های 12 (001/0 = P)، 24 (001/0 > P) و 72 (031/0 = P) پس از ورود به بخش مراقبت‌های ویژه در گروه پاراستامول کاهش معنی‌داری داشت. از نظر میزان داروی مخدر تجویز شده حین عمل (246/0 = P) و بعد از عمل (853/0 = P) تفاوت معنی‌داری بین دو گروه وجود نداشت.نتیجه‌گیری: تجویز پاراستامول به صورت پیش‌دستانه، و ادامه‌ی تجویز آن در دوره‌ی بعد از عمل، باعث بهبود کیفیت کنترل درد داروهای مخدر می‌شود. البته قدرت اثر آن به اندازه‌ای نیست که نیاز به داروهای مخدر و آرام‌بخش و پیامدهای ناشی از این داروها را بتواند کاهش دهد.}, keywords_fa = {پاراستامول,درد,پیوند عروق کرونر}, url = {https://jims.mui.ac.ir/article_14985.html}, eprint = {https://jims.mui.ac.ir/article_14985_6e0172d261412cf95e9717aae017d3d9.pdf} }
