The aim of this study was to determine the effect of high-dose selenium on maximum inspiratory pressure in critically ill patients who received blood transfusion and under mechanical ventilation in intensive care units (ICUs).Methods: In a clinical trial study, 102 patients with history of blood transfusion in intensive care units of Alzahra hospital, Isfahan, Iran, were randomly divided in three groups of receiving 1000, 500, and 0 µg intravenous selenium. Maximum inspiratory pressure and respiratory parameters were evaluated and compared between the three groups.Findings: The mean maximum inspiratory pressure in the first time of ventilation was -28.12 ± 2.67, -29.41 ± 2.65, and -28.5 ± 2.16 cmH2O in the three groups of control, and 1000 and 500 µg selenium, respectively, without any statistically difference between the three groups (P = 0.10). The mean maximum inspiratory pressure was -30.29 ± 1.47, -30.12 ± 1.39, and -30.74 ± 1.91 cmH2O at pressure support ventilation (PSV) of 8 cmH2O in mentioned groups, respectively, and no statistically difference were seen between the groups (P = 0.28). At pressure support ventilation 5 cmH2O, the mean maximum inspiratory pressure was -31.82 ± 1.91, -32.53 ± 2.68 and -30.76 ± 5.22 cmH2O in three mentioned groups, respectively, without statistically difference between the groups (P = 0.10).Conclusion: Different doses of selenium did not decrease airway resistance in patients receiving blood transfusion under mechanical ventilation. Therefore, more studies with larger sample sizes is recommended to determine the role of selenium in improving breathing and ventilation parameters, timing and weaning from mechanical ventilation and other related parameters.}, keywords = {Maximum inspiratory pressure,Ventilation,Selenium}, title_fa = {بررسی مقایسه‌ای تأثیر سلنیوم دز بالا بر حداکثر فشار عضلات دمی در بیماران بدحال دریافت کننده‌ی خون بستری در بخش مراقبت‌های ویژه}, abstract_fa = {قدمه: مطالعات قبلی نشان داده است که بیماران دریافت کننده‌ی خون که تحت تهویه‌ی مکانیکی قرار دارند، با افزایش مقاومت راه هوایی روبه‌رو می‌شوند. هدف از انجام این مطالعه، تعیین تأثیر سلنیوم با دز بالا بر حداکثر فشار عضلات دمی در بیماران بدحال بستری دریافت کننده‌ی خون در بخش مراقبت‌های ویژه‌ی بیمارستان الزهرای (س) اصفهان بود.روش‌ها: در یک مطالعه‌ی کارآزمایی بالینی، 102 بیمار تحت ونتیلاسیون با ونتیلاتور مدک که سابقه‌ی دریافت خون داشتند، در سه گروه دریافت کننده‌ی 1000 و 500 میکروگرم سلنیوم تزریقی و بدون دارو (شاهد) مورد مطالعه قرار گرفتند. بیماران در طی مدت تهویه‌ی مکانیکی از نظر پارامترهای تنفسی و میزان مقاوت راه‌های هوایی، بررسی و مقایسه شدند.یافته‌ها: میانگین حداکثر فشار عضله‌ی دمی در بدو شروع ونتیلاسیون در سه گروه شاهد و 500 میکروگرم و 1000 میکروگرم سلنیوم به ترتیب 67/2 ± 12/28-، 65/2 ± 41/29- و 16/2 ± 5/28- سانتی‌متر آب بود و اختلاف معنی‌داری بین سه گروه دیده نشد (10/0 = P). میانگین این متغیر در زمان رسیدن به Pressure support ventilation (PSV) معادل 8 سانتی‌متر آب، در سه گروه پیش‌گفته به ترتیب 47/1 ± 29/30-، 39/1 ± 12/30- و 96/1 ± 74/30- سانتی‌متر آب بود و اختلاف سه گروه معنی‌دار نبود (28/0 = P). این میانگین در زمان رسیدن به PSV معادل 5 سانتی‌متر آب به ترتیب 91/1 ± 82/31-، 68/2 ± 53/32- و 22/5 ± 76/30- سانتی‌متر آب بود؛ اختلاف سه گروه معنی‌دار نبود (10/0 = P).نتیجه‌گیری: تجویز مقادیر مختلف سلنیوم با کاهش مقاومت راه هوایی در بیماران تحت تهویه‌ی مکانیکی همراه نبود. از این رو، توصیه می‌گردد مطالعات وسیع‌تر و با حجم نمونه‌ی بالاتری در مورد نقش سلنیوم در بهبود پارامترهای تنفسی و تهویه‌ای، زمان و موفقیت جداسازی بیماران از دستگاه تهویه‌ی مکانیکی و دیگر پارامترهای تهویه‌ی مکانیکی انجام گیرد.}, keywords_fa = {حداکثر فشار عضلات دمی,تهویه‌ی مکانیکی,سلنیوم}, url = {https://jims.mui.ac.ir/article_15186.html}, eprint = {https://jims.mui.ac.ir/article_15186_7c79c2def829b72281d752cfeba09c4f.pdf} }
