The purpose of this study was to investigate the predictor factors of seizure in patients with organophosphate poisoning.Methods: This study was performed on patients with organophosphate poisoning hospitalized from 2007 to 2015 in university referral hospital in Isfahan City, Iran. Patients' information including age, sex, seizure, time between ingestion and admission to hospital, vital signs, duration of connection to ventilator, blood glucose, and blood pH were evaluated. The different variables were compared between patients with and without seizure. Binary logistic regression was used for determining variables for prediction of seizure.Findings: 209 patients with organophosphate poisoning were evaluated during the study period. Patients’ mean age was 33.1 ± 18.4 years (range: 2-90 years). Most of the patients were men (61.2%). The prevalence of seizure in patients was 5.7%, which was more common in patients with miosis (33.3% vs. 13.7%, P = 0.040). Patients with seizure required more intubation and longer periods of ventilation (5.1 ± 3.2 vs. 0.9 ± 0.4 days, P < 0.001). There was no significant difference between patients with and without seizure with respect to other variables. 27 patients died, of whom 3 patients had seizure. None of the above variables were predictive for seizure.Conclusion: The results of this study showed that the prevalence of seizure in patients with organophosphate poisoning was 5.7%. Seizure was more common in those with miosis. Length of ventilation was more in patients with seizure.}, keywords = {Organophosphate insecticide,Poisoning,Seizure,Miosis,Ventilation}, title_fa = {بررسی شاخص‌های پیش‌بینی کننده‌ی تشنج در مسمومیت با سموم ارگانوفسفره}, abstract_fa = {مقدمه: مسمومیت با سموم ارگانوفسفات (Organophosphate یا OP)، از جمله مسمومیت‌های شایع در بخش مسمومین به شمار می‌رود و سبب بروز عوارض زیادی در بیماران می‌شود که مهم‌ترین آن، درگیری سیستم عصبی مرکزی و تشنج است. هدف از انجام مطالعه‌ی حاضر، بررسی شاخص‌های پیش‌بینی کننده‌ی تشنج در مسمومیت با سموم OP بود.روش‌ها: این پژوهش گذشته‌نگر، بر روی بیماران دچار مسمومیت با سموم OP از ابتدای سال 1386 تا پایان سال 1394 در بیمارستان خورشید اصفهان انجام شد. اطلاعات بیماران شامل سن، جنسیت، تشنج، فاصله‌ی خوردن سم تا رسیدن به بیمارستان، علایم حیاتی، مدت اتصال به دستگاه تنفس، تنگی مردمک (Miosis) و pH خون وریدی در چک‌لیست ثبت گردید. شاخص‌های مختلف در بین بیماران با و بدون تشنج مورد مقایسه قرار گرفت.یافته‌ها: 209 بیمار در مطالعه شرکت نمودند که بازه‌ی سنی 2 تا 90 سال و میانگین 4/18 ± 1/33 سال داشتند. 128 نفر (2/61 درصد) از آنان را مردان و 81 نفر (8/38 درصد) را زنان تشکیل دادند. شیوع تشنج در بیماران، 7/5 درصد بود و بیمارانی که تشنج کرده بودند، تنگی مردمک بیشتری داشتند (فراوانی تنگی مردمک در بیماران با تشنج، 3/33 درصد و در بیماران بدون تشنج، 7/13 درصد بود) (040/0 = P). بیماران دارای تشنج به علت نارسایی تنفسی، مدت زمان بیشتری تحت حمایت با دستگاه تنفس قرار گرفتند. سایر متغیرها تفاوت معنی‌داری را بین بیماران با و بدون تشنج نشان نداد. 27 بیمار فوت کردند که از این تعداد، 3 بیمار دچار تشنج شده بودند.نتیجه‌گیری: شیوع تشنج در بیماران دچار مسمومیت با سموم OP، 7/5 درصد می‌باشد که این میزان با توجه به عوارض تشنج بر سیستم عصبی مرکزی، اهمیت فراوانی دارد. بروز تشنج در بیمارانی بیشتر است که در معاینه‌ی بالینی مردمک تنگی دارند. طول مدت استفاده از دستگاه تنفس نیز در بیماران دارای تشنج بیشتر می‌باشد.}, keywords_fa = {سموم ارگانوفسفات,مسمومیت,تشنج,تنگی مردمک,دستگاه تنفس}, url = {https://jims.mui.ac.ir/article_15440.html}, eprint = {https://jims.mui.ac.ir/article_15440_b83dfc0322d3e263d03fdf3bf3be1c06.pdf} }
