In this research project, we decided to compare two scoring systems of Sequential Organ Failure Assessment (SOFA) and Acute Physiology and Chronic Health Evaluation II (APACHE II) for predicting the mortality rate of patients with trauma hospitalized in intensive care unit (ICU).Methods: In this retrospective cross-sectional descriptive study, 100 patients with trauma admitted to the intensive care unit were divided into two groups of death and living patients. Demographic data were extracted from patients' records for calculating APACHE and SOFA scores. The data of this study were compared between the two groups.Findings: The mean SOFA score was 6.19 and 15.56, and mean APACHE II score was 21.26 and 46.87 in living and death groups, respectively; the mean scores of SOFA and APACHE II in the death group was significantly higher than the living group (P < 0.001). The sensitivity and specificity of APACHE II and SOFA scores was 97.44% and 84.62%, and 90.16% and 95.08%, respectively.Conclusion: The SOFA and APACHE II systems are good and effective to predict mortality in patients with trauma hospitalized in intensive care unit. The sensitivity of APACHE II and SOFA scoring systems is similar, but the specificity of APACHE II is better than SOFA.}, keywords = {Trauma,Intensive Care Unit,Acute physiology and chronic health evaluation,Sequential organ failure assessment scores}, title_fa = {بررسی مقایسه‌ای امتیاز دو سیستم نمره‌دهی SOFA و APACHE II در بدو پذیرش در تعیین پیش‌آگهی بیماران دچار ترومای بستری در بخش مراقبت‌های ویژه}, abstract_fa = {مقدمه: پس از وقوع تروما، معاینات بالینی و روش‌های آزمایشگاهی در جهت ارزیابی تروما می‌توانند یک نمای کلی از پیش‌آگهی بیماران را نشان دهند که در جهت مدیریت بهینه‌ی بیماران اهمیت زیادی دارد. این طرح پژوهشی، با هدف بررسی و مقایسه‌ی دو سیستم نمره‌دهی Sequential organ failure assessment (SOFA) و Acute physiology and chronic health evaluation II (APACHE II) از نظر توان پیش‌بینی مرگ و میر بیماران دچار ترومای بستری در Intensive care unit (ICU) انجام شد.روش‌ها: در این مطالعه‌ی مقطعی- توصیفی از نوع گذشته‌نگر، 100 بیمار دچار تروما که در ICU بستری شده بودند، به دو گروه فوت شده و گروه زنده تقسیم‌بندی شدند. اطلاعات دموگرافیک جهت محاسبه‌ی امتیازهای APACHE و SOFA از پرونده‌ی بیماران استخراج شد و اطلاعات این تحقیق در دو گروه بررسی و مقایسه گردید.یافته‌ها: میانگین سیستم نمره‌دهی SOFA در گروه زنده و فوت شده به ترتیب 19/6 و 56/15 و میانگین APACHE II در گروه زنده و فوت شده به ترتیب 26/21 و 87/46 بود و همچنین، میانگین نمره‌ی SOFA و APACHE II در گروه فوت شده به صورت معنی‌داری بالاتر از گروه زنده بود (001/0 > P). میزان حساسیت (Sensitivity) APACHE II و SOFA به ترتیب 4/97 و 4/97 درصد و میزان ویژگی (Specificity) آن‌ها به ترتیب 1/36 و 4/16 درصد بود.نتیجه‌گیری: استفاده از دو سیستم SOFA و APACHE II در جهت پیش‌بینی مرگ و میر در بیماران دچار ترومای بستری در ICU بسیار خوب و مؤثر است، اما میزان حساسیت APACHE II و SOFA شبیه به هم و میزان ویژگی APACHE II بهتر از SOFA است.}, keywords_fa = {تروما,بخش مراقبت‌های ویژه,APACHE II,SOFA}, url = {https://jims.mui.ac.ir/article_15559.html}, eprint = {https://jims.mui.ac.ir/article_15559_b30d970f1b305ca07f122e6b630d7b47.pdf} }
