The cut-of-point and diagnostic values were determined in comparison with Cormack-Lehane index.Findings: The NC/TMH test had the highest sensitivity (80.6%) and AASI had the highest specificity (94.8%). Moreover, among the six testes, NC/TMH had the highest positive predictive value (PPV), and ULBT had highest negative predictive value (NPV). ULBT had the highest positive likelihood ratio and TMH had the highest negative likelihood ratio.Conclusion: Compared with the other tests, NC/TMH, AASI, and TMH had better diagnostic values for evaluating airway, and can be used to detect difficult laryngoscopy.}, keywords = {Laryngoscopy,Forecasting,Indexes,Anesthesia, general}, title_fa = {بررسی مقایسه‌ای پیش‌بینی لوله‌گذاری مشکل از طریق شش شاخص ارتفاع تیرومنتال (TMH)، Acromio-Axillo-Suprasternal Notch Index (AASI)، آزمون تغییر یافته‌ی Mallampati (MMT)، آزمون گاز گرفتن لب فوقانی (ULBT)، نسبت قد به فاصله‌ی تیرومنتال (RHTMD) و نسبت محیط دور}, abstract_fa = {مقدمه: پژوهش حاضر با هدف مقایسه‌ی‌ پیش‌بینی لوله‌گذاری مشکل از طریق شش شاخص ارتفاع تیرومنتال (Thyromental height یا TMH)، Acromio-axillo-suprasternal notch index (AASI)، آزمون تغییر یافته‌ی Mallampati (Modified Mallampati test یا MMT)، آزمون گاز گرفتن لب فوقانی (Upper lip bite test یا ULBT)، نسبت قد به فاصله‌ی تیرومنتال (Ratio of height to thyromental distance یا RHTMD) و نسبت محیط دور گردن به ارتفاع تیرومنتال (Neck circumference/Thyromental height یا NC/TMH) در بیماران تحت بیهوشی عمومی انجام شد.روش‌ها: در این مطالعه‌ی مقطعی، 600 بیمار کاندید عمل جراحی انتخاب شدند و وضعیت لوله‌گذاری در آن‌ها با استفاده از شش شاخص مذکور تعیین گردید. نقطه‌ی برش و ارزش تشخیصی شاخص‌های مورد بررسی با کمک معیار Cormack-Lehane مورد ارزیابی قرار گرفت.یافته‌ها: شاخص NC/TMH دارای بالاترین حساسیت (6/80 درصد) و AASI دارای بالاترین ویژگی (8/94 درصد) بود. از بین شاخص‌های مورد بررسی، بالاترین ارزش اخباری مثبت به NC/TMH و بالاترین ارزش اخباری منفی نیز به ULBT اختصاص داشت. از لحاظ درست‌نمایی، ULBT دارای بالاترین نسبت ارزش اخباری مثبت و TMH دارای بالاترین نسبت ارزش اخباری منفی بود.نتیجه‌گیری: معیارهای NC/TMH، AASI و TMH در مقایسه با سایر شاخص‌های ارزیابی راه هوایی، از ارزش تشخیصی بالاتری برخوردار می‌باشند و می‌توان از آن‌ها جهت تعیین وضعیت لوله‌گذاری بیماران تحت عمل جراحی استفاده نمود.}, keywords_fa = {لارنگوسکوپی,پیش بینی,شاخص,بیهوشی عمومی}, url = {https://jims.mui.ac.ir/article_15797.html}, eprint = {https://jims.mui.ac.ir/article_15797_edbbfd17c0a6d92392eeb7d740dc0dc5.pdf} }
