In the first group, the end-tidal carbon dioxide (ETCO2) pressure was maintained in the range of 35-40 mmHg, and in the second group in the range of 40 to 45 mmHg. Patients' cognitive scores before surgery, and 24 hours, one week, and six weeks later were compared between the two groups.Findings: In the first 24 hours, the incidence of cognitive impairment was 12.5 and 11.7 percent in first and second groups, respectively; while at the first week after surgery, this rate was 6.7 and 11.7 percent, and at the sixth week, 5.0 and 9.2 percent, respectively, based on the short test of mental status. In all cases, the difference between the two groups was not statistically significant.Conclusion: ETCO2 in the range of 35 to 45 mmHg has no effect on the cognitive impairment of elderly patients under cataract surgery with general anesthesia. Therefore, further studies on the effects of different extents of ETCO2 on cognitive impairments are necessary.}, keywords = {Cognitive disorder,Cataract,Elderly}, title_fa = {بررسی مقایسه‌ی دو سطح مختلف فشار دی‌اکسید کربن انتهای بازدمی بر اختلالات شناختی بیماران سالخورده بعد از عمل جراحی کاتاراکت تحت بیهوشی عمومی}, abstract_fa = {مقدمه: این مطالعه، با هدف مقایسه‌ی تأثیر دو سطح مختلف دی‌اکسید کربن انتهای بازدمی بر اختلالات شناختی بیماران سالمند تحت عمل جراحی انجام شد.روش‌ها: در این مطالعه کارآزمایی بالینی، 240 بیمار بالاتر از 65 سال به صورت اتفاقی در دو گروه توزیع شدند. در گروه اول، میزان دی‌اکسید کربن انتهای بازدمی در محدوده‌ی 40-35 میلی‌مترجیوه و در گروه دوم در محدوده‌ی 45-40 میلی‌متر جیوه حفظ شد. نمره‌ی شناختی بیماران قبل از عمل، 24 ساعت، یک هفته و شش هفته بعد از عمل بین دو گروه مقایسه شد.یافته‌ها: در 24 ساعت اول بعد از عمل، بروز اختلالات شناختی در دو گروه اول و دوم به ترتیب 5/12 و 7/11 درصد بود؛ در حالی که هفته‌ی اول بعد از عمل، این میزان به ترتیب به 7/6 و 7/11 درصد رسید و در هفته‌ی ششم بعد از عمل، میزان بروز اختلالات شناختی بر اساس آزمون کوتاه وضعیت ذهنی در نامبردگان در دو گروه اول و دوم به ترتیب به 0/5 و 2/9 درصد کاهش یافت. در تمام موارد، اختلاف بین دو گروه از نظر آماری معنی‌دار نبود.نتیجه‌گیری: دی‌اکسید کربن انتهای بازدمی در محدوده‌ی 45-35 میلی‌متر جیوه تأثیری بر اختلالات شناختی بیماران سالخورده‌ی تحت عمل جراحی کاتاراکت با بیهوشی عمومی نداشت. از این رو، انجام مطالعات بیشتر در خصوص تأثیر سطوح و محدوده‌ی وسیع‌تری از دی‌اکسید کربن انتهای بازدمی بر این اختلالات شناختی ضرورت دارد.}, keywords_fa = {اختلال شناختی,کاتاراکت,سالخوردگی}, url = {https://jims.mui.ac.ir/article_15889.html}, eprint = {https://jims.mui.ac.ir/article_15889_df8440627018452f5a2a5c9b379672bb.pdf} }
