N-acetylcysteine (NAC) is an anti-inflammatory agent that has protective effects on kidney function. The aim of the present study was to evaluate the effects of NAC on prevention of renal failure following CPB.Methods: In this double-blind clinical trial study, 100 patients who were candidates for coronary artery bypass grafting (CABG) at Shahid Chamran hospital in Isfahan, Iran, aged 40-65 years with glomerular filtration rate (GFR) of greater than 60 and creatinine level of less than 1.5 mg/dl were selected. The intervention group received 1200 mg of NAC in Prime solution. NAC was not used in the control group. The two groups were compared in terms of GFR and mortality. Data on blood levels of urea, creatinine, urine output, and intensive care unit (ICU) stay were collected.Findings: Patients in the two groups had relatively similar conditions in terms of mean age, height, weight, aortic clamp time, and pump. No mortality was reported in any of the groups. There was no significant difference between the ICU stay and the amount of urea in the two groups of intervention and control (P > 0.050). There was a significant difference in the urine output of patients in both groups in the first 24 hours after CPB (P < 0.050); but in the second 24 hours, the difference was not significant. There was significant differences in creatinine and GFR levels of patients with diabetes mellitus in the intervention group with other patients (P < 0.050). No patient death was reported in any of the statistical population groups.Conclusion: The results show that the use of NAC reduces creatinine and increases GFR in patients with diabetes mellitus, which indicates that the use of NAC may prevent renal failure in these patients.}, keywords = {Acetylcysteine,Cardiopulmonary bypass,Renal insufficiency}, title_fa = {بررسی تأثیر ان-استیل سیستئین در محلول پرایم در جلوگیری از نارسایی کلیه در عمل‌های جراحی قلب باز}, abstract_fa = {مقدمه: تکنولوژی پمپ قلب و ریه (CPB یا Cardiopulmonary bypass) مزایای زیادی در بهبود جراحی قلب دارد. از عوارض آن، می‌توان به آسیب و نارسایی کلیه اشاره کرد. ان-استیل سیستئین (N-acetylcysteine یا NAC)، یک عامل ضد التهابی است که اثرات محافظتی بر روی عملکرد کلیه دارد. هدف از پژوهش حاضر، بررسی اثرات NAC بر کاهش صدمات کلیوی ناشی از CPB بود.روش‌ها: در این کارآزمایی بالینی دو سو کور، 100 نفر نفر از بیماران کاندیدای عمل پیوند شریان‌های کرونر در بیمارستان شهید چمران اصفهان با سن 65-40 سال با فیلتراسیون گلومرولی (Glomerular filtration rate یا GFR) بیشتر از 60 و کراتینین کمتر از 5/1 میلی‌گرم بر دسی‌لیتر انتخاب شدند. گروه مورد 1200 میلی‌گرم NAC داخل محلول Prime دریافت کرد. در گروه شاهد، از NAC استفاده نشد. دو گروه از نظر GFR و مرگ و میر مقایسه شدند. اطلاعات میزان سطح خونی اوره (Blood urea nitrogen یا BUN)، کراتینین، برون‌ده ادراری و مدت زمان اقامت بیمار در بخش مراقبت‌های ویژه (Intensive care unit یا ICU) جمع‌آوری گردید.یافته‌ها: بیماران دو گروه از نظر میانیگین سن، قد، وزن، زمان کلامپ آئورت و پمپ شرایط به نسبت یکسانی داشتند. در هیچ یک از گروه‌ها مرگ بیمار گزارش نشد. میانگین مدت زمان اقامت در ICU و میزان اوره‌ی بیماران در دو گروه مورد و شاهد اختلاف معنی‌داری نداشتند (050/0 > P). میزان برون‌ده ادرار بیماران در هر دو گروه در 24 ساعت اول پس از CPB اختلاف معنی‌داری داشتند (050/0 < P)، اما در 24 ساعت دوم میزان این اختلاف قابل توجه نیست. میزان کراتینین و GFR بیماران مبتلا به دیابت گروه مورد با سایر بیماران اختلاف معنی‌داری داشت (050/0 > P). در هیچ یک از گروه‌های جامعه‌ی آماری، مرگ بیمار گزارش نشد.نتیجه‌گیری: نتایج نشان می‌دهد که استفاده از داروی NAC باعث کاهش کراتینین و افزایش GFR در بیماران مبتلا به دیابت می‌گردد که اثر NAC بر جلوگبری از نارسایی کلیه را نشان می‌دهد.}, keywords_fa = {استیل سیستئین,تکنولوژی پمپ قلب و ریه,نارسایی کلیه}, url = {https://jims.mui.ac.ir/article_16194.html}, eprint = {https://jims.mui.ac.ir/article_16194_8b1ea1c082c24ebac09e6d55c33961f4.pdf} }
