The aim of this study was to compare the level of arterial blood gas pressure and the percentage of arterial oxygen saturation in the supine position with the lateral position in patients with ventilator-associated pneumonia.Methods: In a clinical trial study with a sample size of 50 patients with ischemic stroke under mechanical ventilation admitted to the intensive care unit of Kashani hospital affiliated to Shahrekord University of Medical Sciences, Shahrekord, Iran, in 2019 were selected using convenience sampling method. Oxygen saturation percentage was measured by a pulse oximeter in patients lying on their backs and then, on their sides for two hours; arterial blood pressure and carbon dioxide levels were measured with arterial blood sample by laboratory device, and recorded in the relevant checklist as well.Findings: In both supine and lateral positions, the mean arterial oxygen pressure and the percentage of hemoglobin oxygen saturation were not significantly different (P > 0.050); while the arterial carbon dioxide pressure in the lateral position was significantly lower than the supine position (P = 0.008).Conclusion: The lateral position has no significant effect on the improvement of arterial blood gases and oxygen saturation in patients with pneumonia due to mechanical ventilation; but has significantly reduce the pressure of carbon dioxide.}, keywords = {Posture,Oxygen saturation,Mechanical Ventilation,Pneumonia}, title_fa = {اثربخشی وضعیت خوابیده به پهلو بر میزان فشار اکسیژن شریانی، فشار دی‌اکسیدکربن شریانی و درصد اشباع اکسیژن در بیماران سکته‌ی مغزی ایسکمیک مبتلا به پنومونی ناشی از ونتیلاتور}, abstract_fa = {مقدمه: تبادلات گازی بهتر، زمینه‌ساز نتیجه‌ی مطلوب‌تری با فشارهای پایین‌تر اکسیژناسیون است. وضعیت قرارگیری بیمار در بیماران تحت تهویه‌ی مکانیکی، یکی از عوامل مؤثر بر راندمان تبادل گازهای تنفسی می‌باشد. هدف از انجام پژوهش حاضر، مقایسه‌ی سطح فشار گازهای خون شریانی و درصد اشباع اکسیژن در وضعیت خوابیده به پهلو با وضعیت خوابیده به پشت در بیماران مبتلا به پنومونی ناشی از تهویه‌ی مکانیکی بود.روش‌ها: در این مطالعه‌ی کارآزمایی بالینی، 50 بیمار سکته‌ی مغزی ایسکمیک مبتلا به پنومونی ناشی از تهویه‌ی مکانیکی بستری در بخش مراقبت‌های ویژه بیمارستان کاشانی شهرکرد در سال 1398، به روش نمونه‌گیری در دسترس انتخاب شدند. سپس درصد اشباع اکسیژن بیماران ‌در وضعیت خوابیده به پشت و دو ساعت خوابیده به پهلو با استفاده از دستگاه پالس اکسی‌متر و سطح فشار گازهای اکسیژن و دی‌اکسیدکربن خون شریانی نیز به کمک نمونه‌ی خون شریانی ارزیابی و در چک‌لیست ثبت گردید.یافته‌ها: در هر دو وضعیت خوابیده به پهلو و خوابیده به پشت، میانگین فشار اکسیژن شریانی و درصد اشباع اکسیژن هموگلوبین تفاوت معنی‌داری نداشت (050/0 < P)؛ در حالی که میزان فشار دی‌اکسیدکربن شریانی در وضعیت خوابیده به پهلو، به طور معنی‌داری کمتر از وضعیت خوابیده به پشت بود (008/0 = P).نتیجه‌گیری: تغییر وضعیت خوابیده به پهلو، تأثیر مثبتی در بهبود گازهای خون شریانی و درصد اشباع اکسیژن بیماران سکته‌ی مغزی ایسکمیک مبتلا به پنومونی ناشی از تهویه‌ی مکانیکی نداشت، اما منجر به کاهش معنی‌دار فشار دی‌اکسیدکربن شد.}, keywords_fa = {وضعیت,اشباع اکسیژن,تهویه‌ی مکانیکی,ذات‌الریه}, url = {https://jims.mui.ac.ir/article_16262.html}, eprint = {https://jims.mui.ac.ir/article_16262_af8a3f7f975b3323bc6d468c6a2d6aae.pdf} }
