Occult liver failure and gastrointestinal malignancies can cause serious complications in this group of patients. The aim of this study was to investigate and obtain diagnostic guidelines for the identification, diagnosis, and management of asymptomatic occult liver failure and gastrointestinal malignancies in patients who are candidates for bariatric surgery.<br>Methods: This study was conducted qualitatively and by collecting the opinions of experts in the field of diagnosis and treatment of gastrointestinal and liver diseases through telephone and face-to-face interviews. Experts active in this field were identified and their opinions on the diagnosis and management of diseases were collected. The data were analyzed to identify patterns and extract key points.<br>Findings: The findings showed that most experts emphasize the need for comprehensive tests, including liver tests and imaging scans, before surgery. Also, the need for specific protocols in the identification of gastrointestinal malignancies and for patients with a history of inflammatory bowel disease was identified. Finally, a localized guideline for the diagnosis and management of these conditions was proposed.<br>Conclusion: Developing appropriate guidelines for the identification of latent liver failure and gastrointestinal malignancies in patients undergoing bariatric surgery can help reduce surgical complications and improve patient outcomes. It is recommended that specialists and medical centers pay attention to this guideline and be diligent in implementing it in order to improve the quality of services. <strong><span dir="RTL" lang="AR-SA">مقدمه:</span></strong><span dir="RTL" lang="AR-SA"> پیشرفت‌های اخیر در جراحی باریاتریک نیاز به ارزیابی دقیق بیماری‌های همراه پیش از عمل را ضروری ساخته است. نارسایی کبد نهفته و بدخیمی‌های گوارشی، می‌توانند عوارض جدی در این گروه از بیماران ایجاد کنند. هدف از این مطالعه، بررسی و دستیابی به گایدلاین‌های تشخیصی برای شناسایی و تشخیص و مدیریت نارسایی کبد نهفته و بدخیمی‌های گوارشی بدون علامت در بیماران کاندید جراحی باریاتریک بود</span>. <strong><span dir="RTL" lang="AR-SA">روش‌ها:</span></strong><span dir="RTL" lang="AR-SA"> </span><span dir="RTL" lang="FA">این مطالعه به‌صورت کیفی و از روش گردآوری نظرات متخصصین در زمینه تشخیص و درمان بیماری‌های گوارشی و کبدی از طریق مصاحبه‌های تلفنی و حضوری انجام شد. متخصصین فعال در این حوزه شناسایی و نظرات آن‌ها درباره‌ی نحوه‌ی تشخیص و مدیریت بیماری‌ها جمع‌آوری شد. داده‌ها به‌منظور شناسایی الگوها و استخراج نکات کلیدی تحلیل گردید</span>. <strong><span dir="RTL" lang="FA">یافته‌ها:</span></strong><span dir="RTL" lang="FA"> یافته‌ها نشان داد که اکثر متخصصین بر لزوم انجام آزمایشات جامع، از جمله آزمون‌های کبدی و اسکن‌های تصویربرداری قبل از جراحی تأکید دارند. همچنین، نیاز به پروتکل‌های مشخص در شناسایی بدخیمی‌های گوارشی و برای بیماران با سابقه‌ی بیماری التهابی روده مشخص شد. در نهایت، یک گایدلاین بومی‌سازی شده برای تشخیص و مدیریت این شرایط پیشنهاد گردید</span>. <strong><span dir="RTL" lang="FA">نتیجه‌گیری:</span><span dir="RTL" lang="FA"> </span></strong><span dir="RTL" lang="FA">تدوین گایدلاین مناسب برای شناسایی نارسایی کبد نهفته و بدخیمی گوارشی در بیماران کاندید جراحی باریاتریک می‌تواند به کاهش عوارض جراحی و بهبود نتیجه بیماران کمک کند. پیشنهاد می‌شود که متخصصین و مراکز درمانی به‌منظور بهبود کیفیت خدمات، به این گایدلاین توجه کنند و در اجرای آن کوشا باشند</span>.   https://jims.mui.ac.ir/article_33166_b9c72b87254f0b30ae977c99e10b729e.pdf
