تأثیر ترکیب سیمواستاتین موضعی با کلسی‌پوتریول موضعی در درمان پلاک‌های پوستی پسوریازیس

چکیده

مقدمه: پسوریازیس یک اختلال متدوال پوستی با پاسخ متغیر به درمان است. با توجه به اثرات ثابت شده‌ی تعدیل‌کنندگی سیمواستاتین خوراکی بر روی سیستم ایمنی در درمان پسوریازیس، این مطالعه به منظور بررسی اثر ضد پسوریازیسی سیمواستاتین به شکل موضعی انجام شد. همچنین، آنالوگ‌های ویتامین D به عنوان خط اول در درمان موارد خفیف تا متوسط پسوریازیس مؤثر شناخته شده‌اند. در این مطالعه، تأثیر پماد موضعی کلسی‌پوتریول 005/0 درصد (به عنوان یک راه حل استاندارد در درمان سوریازیس) در مقابل ترکیب کلسی‌پوتریول به علاوه سیمواستاتین موضعی در درمان پسوریازیس مقایسه شد.روش‌ها: 80 بیمار مبتلا به پسوریازیس سیمتریک با گرفتاری سطح بدن تا 20 درصد به طور تصادفی به دو گروه تقسیم شدند. گروه الف با پماد کلسی‌پوتریول 005/0 درصد دو بار در روز و گروه ب با کلسی‌پوتریول 005/0 درصد دو بار در روز و پماد سیمواستاتین 3 درصد روزانه دو بار، برای 12 هفته تحت درمان قرار گرفتند. نتایج توسط یک متخصص پوست Blind با استفاده از شاخص شدت منطقه‌ی مبتلا (PASI یا Psoriasis Area and Severity Index)، که در هفته‌های 4، 8 و 12 درمان امتیازدهی می‌شد، ارزیابی شد. در روشی موازی و مشابه، یک ارزیابی ذهنی توسط خود بیماران بر اساس بررسی عکس‌ها در پایان مطالعه انجام گرفت.یافته‌ها: با وجود کاهش منظم PASI در هر دو گروه، با توجه به ارزیابی پزشکان (603/0 = P) و همچنین بیماران (243/0 = P) در پایان مطالعه، به لحاظ آماری استفاده از سیمواستاتین موضعی بیشتر از درمان معمولی پسوریازیس مؤثر نیست.نتیجه‌گیری: این مطالعه نشان داد که سیمواستاتین موضعی در مقایسه با شکل خوراکی، اثرات قابل توجهی در درمان سوریازیس به همراه ندارد. همچنین، این مطالعه نشان داد که پسوریازیس یک اختلال سیستمیک با تظاهرات پوستی مختلف است.

Background: Psoriasis is a common dermatologic disorder, with fluctuating response to treatment. Considering the proven immunomodulatory effects of oral simvastatin in psoriasis, this study aimed to determine whether the topical form has also antipsoriatic effects. Vitamin D analogs known to be effective and are considered the first line of therapy in mild to moderate cases. In this study, the efficacy of topical calcipotriol 0.005% ointment (as a standard method of treatment for psoriasis), versus combination of calcipotriol plus topical simvastatin was compared in the treatment of psoriasis.Methods: 80 patients with symmetric psoriasis who had body surface involvement up to 20% were divided randomly into 2 groups. Group A were treated with calcipotriol 0.005% ointment twice daily and group B with calcipotriol 0.005% ointment twice daily and simvastatin 3% ointment twice daily, for 12 weeks. The results were evaluated by a blind dermatologist using psoriasis area severity index (PASI) score at baseline, 4th, 8th and 12th weeks of treatment. In a similar way, a subjective assessment performed by patients based on photo-evaluation at the end of the study.Findings: Despite a continuous reduction in PASI score in both groups, according to both physician (P = 0.603) and patient (P = 0.243) assessments, topical simvastatin was not statistically more effective than conventional treatment of psoriasis.Conclusion: This study indicates that topical simvastatin is not associated with significant impacts in the treatment of psoriasis as compared to oral form. This study indicates that psoriasis is a systemic disorder with variable skin manifestations.
