آترواسکلروز و استاتین‌ها‌

چکیده

مقدمه: با توجه به تناقضات موجود در مطالعات مختلف در مورد اثر استاتین‌ها‌ بر آترواسکلروز و اهمیت موضوع، هدف مطالعه‌ی حاضر مروری بر پژوهش‌ها‌ی انجام شده در زمینه‌ی استاتین‌ها‌ و آترواسکلروز بود.روش‌ها: مرور متون و استخراج مقالات مرتبط توسط جستجو در  PubMedبا کلید واژه‌ها‌ی Atherosclerosis، Statins وHyperlipidemia  با محدود کردن آن بر روی کارآزمایی‌ها‌ی بالینی در سال‌ها‌ی1980 تا 2011 انجام شد.یافته‌ها: در بیشتر مطالعات استاتین‌ها باعث کاهش کلسترول تام و  LDLیا Low density lipoprotein شده بودند. اما اثر کمی بر افزایش HDL یا High density lipoprotein داشتند. مکانیسم اثر استاتین‌ها‌ در اغلب مطالعات ثابت سازی پلاک آترواسکلروز در نتیجه‌ی مها‌ر اکسیداسیون، بهبود عملکرد اندوتلیال به وسیله‌ی افزایش نیتریک اکساید و بهبود حالت ترومبوتیک توسط مها‌ر پلاکت بود.نتیجه‌گیری: اکثر کارآزمایی‌های بالینی اثر مثبت استاتین‌ها‌ را بر کاهش آترواسکلروز و بیماری‌ها‌ی قلبی عروقی نشان داده‌اند. بنابراین استفاده‌ی از آن‌ها‌ علاوه بر درمان هیپرلیپیدمی می‌تواند به عنوان درمان و یا حتی پیش‌گیری مناسب فرایند آترواسکلروز و سوانح قلبی عروقی باشد. هر چند به علت مشخص نبودن مکانیسم اثر دقیق، نیاز به بررسی بیشتر وجود دارد.

Background: According to inconsistencies in the various studies about effect of statins on atherosclerosis and issue importance, the aim of the present research was review carried out in statins and atherosclerosis.Methods: By the use of atherosclerosis, statins and hyperlipidemia as keywords, we searched in PubMed through 1980 until 2011.Finding: Most studies showed the reduction of total cholesterol and Low density lipoprotein (LDL) by Statins, but there showed a little effect on increasing High density lipoprotein (HDL). In most studies mechanisms of action of statins were stabilization atherosclerotic plaques consequently inhibit oxidation, improve endothelial function by increase Nitric oxide and improvement thrombotic state by inhibiting platelet. Conclusion: Most Of Clinical trials had shown positive effect of statin on cardiovascular disease and atherosclerosis. Therefore, using of them for treatment of hyperlipidemia could be appropriate preventive for atherosclerosis and cardiovascular accidents. Although the exact mechanism of action is not clear, there is a need for further investigation.
