الکتروکموتراپی در درمان تومورها

چکیده

الکتروکموتراپی شامل انجام کموتراپی و به دنبال آن Electroporation یعنی استفاده‌ی موضعی از پالس الکتریکی در اطراف تومور به منظور افزایش ورود داروی کموتراپی به درون سلول توموری می‌باشد. روش Electroporation برای افزایش جذب داروها فقط در مورد داروهایی که انتقال آن‌ها توسط غشای پلاسمایی ممانعت می‌شود، کاربرد دارد. در بین این نوع داروها، فقط بلئومایسین و سیس‌پلاتین بررسی بیشتری شده‌اند و مراحل تست‌های پره‌کلینیکال و کارآزمایی‌های بالینی بر روی آن‌ها انجام شده است. در مطالعات In vitro به دنبال استفاده از پالس الکتریکی افزایش سیتوتوکسیستی این داروها ذکر شده است و در مطالعات In vivo هم Electroporation تومور بعد از کموتراپی (موضعی یا سیستمیک) یا به اصطلاح الکتروکموتراپی یک درمان آنتی‌توموری مؤثر است. در مطالعات دامپزشکی این روش در درمان تومورهای اولیه‌ی حیوانات از جمله گربه، سگ و اسب با موفقیت انجام شده است. در مطالعاتی که در آزمایشگاه روی چند مدل تومور در خارج بدن انجام شده فاکتورهای مؤثر در درمان تعیین شده‌اند و در مطالعات کلینیکی انسانی این روش برای بیماران مبتلا به بدخیمی‌های مختلف که بیماری پیشرفته و ندول‌های توموری قابل دسترس داشتند، با موفقیت همراه بوده است.

Electrochemotherapy involves the application of chemotherapy followed by local electrical pulse around the tumor to increase drug delivery into tumor cells. Electroporation is used to increase drug absorbance only for medications whose transport is prevented by plasma membrane. Among all available medicines, bleomycin and cisplatin have been widely studied in pre-clinical and clinical trials. In-vitro observations revealed an increase in cytotoxicity of these drugs after the application of electrical pulses. In-vivo studies have suggested tumor electroporation after local or systemic chemotherapy therapy (electrochemotherapy) as an effective antitumor treatment. This method has also been successfully used in treating primary tumors of cats, dogs, and horses. Pre-clinical research on several types of tumor has clarified the parameters resulting to the effective, local control of tumors. In human clinical trials, electrochemotherapy has been an efficient method for treating advanced diseases with accessible malignant tumors of various types.
