بررسی علل شکایت‌های مربوط به پوست و زیبایی در دفاتر سازمان‌ پزشکی قانونی استان اصفهان طی سال‌های 1381 تا 1392

چکیده

مقدمه: خطاهای پزشکی به طور تقریبی عمده‌ترین دلیل اقامه‌ی دعاوی علیه پزشکان در سال‌های اخیر بوده است و مقدار قابل توجهی از این شکایات، مربوط به مسایل قابل اجتناب است. از این رو، مطالعه‌ی حاضر با هدف بررسی علل شکایت‌های مربوط به پوست و زیبایی جهت ارایه‌ی راه‌های پیش‌گیری مناسب، انجام شد.روش‌ها: در این مطالعه‌ی توصیفی، پرونده‌های شکایت از قصور پزشکی رشته‌ی پوست موجود در سازمان‌های پزشکی قانونی استان اصفهان، طی سال‌های 92-1381 مورد مطالعه قرار گرفت. روایی صوری ابزار مطالعه (چک لیست)، توسط متخصصین مربوط مورد تأیید قرار گرفت. با استفاده از نرم‌افزار آماری SPSS، برای متغیرهای کمی میانگین و انحراف معیار و برای متغیرهای کیفی فراوانی مطلق و نسبی محاسبه شد. جهت بررسی ارتباط آماری متغیرها از آزمون 2χ استفاده شد. یافته‌ها: بیشترین فراوانی شکایت بیماران از پزشکان در استان اصفهان از متخصصین (7/62 درصد) بود. در پرونده‌های بیماران شاکی، رأی صادره برای پزشکان در 9/41 درصد قصور، 8/55 درصد برائت و 3/2 درصد مختومه بدون رأی بود. همچنین، دیه‌ی تعیین شده در 6/55 درصد پرونده‌ها کمتر از 25 درصد، در 2/22 درصد پرونده‌ها 50-25 درصد و در 2/22 درصد پرونده‌ها، بیش از 50 درصد تعیین شد. بر اساس آزمون 2χ، ارتباط آماری معنی‌داری بین رأی صادره با نوع مدرک تحصیلی پزشکان استان اصفهان مشاهده نشد (050/0 < P).نتیجه‌گیری: بیشترین قصور مربوط به پزشکان متخصص بود و این شکایات، به طور عمده از متخصصین رشته‌های غیر مرتبط وجود داشت. به همین علت، اطمینان یافتن از انجام مراحل تشخیصی و درمانی بر اساس تخصص افراد و عدم دخالت در سایر رشته‌های غیر مرتبط با تخصص پزشک، توصیه می‌گردد.

چکیده [English]

Background: Medical errors almost have been the most common causes of proceedings against physicians, and a significant amount of complaints is related to inevitable issues. This study aimed to investigate the causes of complaints related to skin and cosmetics to give over preventive solutions.Methods: In this descriptive study, the medical malpractice cases related to skin and cosmetics were studied in general offices of legal medicine of Isfahan Province, Iran, during 2002-2013. As the instrument was a checklist, the formal validity was approved by the relevant experts. Mean and standard deviation for quantitative variables and frequency for qualitative variables were calculated using SPSS software. The chi-square test was used to evaluate the correlation of variables.Findings: Most of the complaints were from specialists (62.7%). The issued votes included 41.9% failure, 55.8% acquittal and 2.3% were closed with no vote. The blood money was less than 25% in 55.6% of cases, 25-50% in 22.2% of cases and more than 50% in the 22.2% of cases. The chi-square test showed no significant relationship between the issued judgments and physicians’ educational certifications (P > 0.05).Conclusion: In general, we can say that the medical malpractices were related to specialists and the complaints mainly were from specialists in other fields. For this reason, making sure that the diagnosis is based on appropriate expertise and avoiding the interference in other fields are recommended.
