بررسی عملکرد محور غده‌ی پاراتیرویید در بیماران مبتلا به پارکینسون

چکیده

مقدمه: هورمون پاراتیرویید معروف به پاراتورمون (PTH یا Parathyroid hormone) با ایجاد تغییرات در سطح بیومارکرهای سرمی، می‌تواند در سیر پیشرفت و علایم ناشی از طیف گسترده‌ای از بیماری‌ها اثرگذار باشد. پارکینسون یکی از این بیماری‌ها است و با توجه به مطالعات انجام شده، به نظر می‌رسد عملکرد غده‌ی پاراتیرویید می‌تواند در شدت علایم ناشی از آن، مؤثر باشد. این مطالعه با توجه به فقدان مطالعات لازم در این زمینه در خاور میانه با هدف بررسی این ارتباط انجام شد.روش‌ها: این مطالعه به روش مورد- شاهدی انجام گرفت. 125 بیمار مبتلا به پارکینسون، که تشخیص بیماری در آن‌ها توسط نورولوژیست به اثبات رسید و 112 شاهد سالم که از نظر سن و جنسیت با گروه بیمار مطابقت داشتند، در این مطالعه شرکت کردند. شدت علایم حرکتی توسط بخش حرکتی پرسش‌نامه‌ی UPDRS (Unified Parkinson's disease rating scale) اندازه‌گیری شد. سطح سرمی پاراتورمون، کلسیم، فسفر و OHD-25 (Hydroxy vitamin D-25) در هر دو گروه اندازه‌گیری شد.یافته‌ها: میانگین سطح سرمی پاراتورمون در گروه مورد (8/18 ± 2/45) در مقایسه با گروه شاهد (5/28 ± 8/70) به شکل معنی‌داری اختلاف داشت (0001/0 = P). در این مطالعه، هیچ ارتباط معنی‌داری میان سطح سرمی عناصر ذکر شده و شدت علایم حرکتی بیماری پارکینسون یافت نشد. سن بیماران و طول مدت بیماری با شدت علایم حرکتی و نمره‌ی خرده مقیاس III UPDRS مرتبط بود؛ همچنین این بیماری در مردان بیشتر از زنان شیوع داشت (0080/0 = P، 03/5-27/1 = CI، 53/2 = OR).نتیجه‌گیری: این مطالعه نمی‌تواند مؤید ارتباط میان میزان عملکرد غده‌ی پاراتیرویید، سطح سرمی عناصر مرتبط با پاراتورمون و شدت علایم حرکتی ناشی از بیماری پارکینسون باشد. یکی از دلایلی که می‌تواند توجیه کننده‌ی این نتایج باشد، فقدان تعداد کافی از کاهش و یا افزایش کلسیم و کاهش و یا افزایش فسفر در نمونه‌های مورد مطالعه است.

Background: Parathyroid hormone (PTH) levels can make changes in serum biomarkers and affect progression symptoms of many diseases. One of these is Parkinson's disease (PD) and according to previous studies, it seems that parathyroid gland function is effective in severity of symptoms of PD. This study aimed to explore the role of PTH in PD progression regarding the lack of researches in this field in the Middle East.Methods: This case-control study was carried out in Alzahra hospital in Isfahan, Iran. 125 patients with PD, diagnosed by experienced neurologists and 112 healthy controls matched for age and gender with patient group were asked to participate during September to November 2011. At baseline, the severity of PD was evaluated with motor part of the Unified Parkinson’s Disease Rating Scale III (UPDRS III). The level of PTH, calcium, phosphorus, and 25-hydroxy vitamin D were measured.Findings: The mean ± SD of PTH level was significantly lower in patients group (45.2 ± 18.8 IU/l) compared to controls (70.8 ± 28.5 IU/l) (P = 0.0001); there are not any significant correlation between hormone level, elements and severity of PD. We observed that men had more risk of PD than women (Odds ratio = 2.53, [CI: 1.27‑5.03], P: 0.0080).Conclusion: This study could not confirm the relationship between the level of parathyroid gland function, the level of serum PTH and severity of motor symptoms in PD. It could be explained by the lack of a sufficient number of Hypo and hypercalcemia, phosphatemia and Hypo-or hyperparathyroidism in our samples.
