بررسی یافته‌های سونوگرافی و ماموگرافی در بیماران دارای درد موضعی پستان در مقایسه با FNA

چکیده

مقدمه: با توجه به این که بسیاری از زنان در طول عمر خود تجربه‌ای از دردهای پستانی دارند که سبب بروز اضطراب در آن‌ها می‌شود، در این مطالعه سعی داشتیم حساسیت و ویژگی روش‌های غیرتهاجمی سونوگرافی و ماموگرافی را در تشخیص بدخیمی‌ها در زنان با درد پستان بررسی کنیم.روش‌ها: در این مطالعه، 74 بیمار زن که از درد پستان شاکی بودند و پزشک برای آن‌ها انجام ماموگرافی و سونوگرافی هم زمان را تجویز کرده بود، مورد بررسی قرار گرفتند. اطلاعات حاصل از انجام ماموگرافی و سونوگرافی این زنان با نتیجه‌ی Fine needle aspiration (FNA) آن‌ها مورد مقایسه قرار گرفت و حساسیت، ویژگی، ارزش اخباری مثبت و ارزش اخباری منفی این روش‌ها اندازه گیری شد.یافته‌ها: از کل بیماران مورد بررسی تنها 3 مورد در FNA مبتلا به بدخیمی پستان بودند. در این مطالعه، حساسیت و ویژگی ماموگرافی به ترتیب 100 و 2/98 درصد و حساسیت و ویژگی سونوگرافی به ترتیب 67 و 2/97 درصد به دست آمد.نتیجه‌گیری: نتایج حاصل از این مطالعه نشان می‌دهند که سونوگرافی و ماموگرافی در مواردی که شک قوی به وجود سرطان پستان وجود دارد، روش‌های مناسب جایگزین برای FNA نیستند و در این موارد انجام بیوپسی ضروری است.

Background: Considering that many women during their life experience breast pain that is causing their anxiety, in this study we tried to measure the sensitivity and specificity of breast ultrasound and mammography in diagnosis of malignancies in women with breast pain.Methods: In this study, 74 female patients who were complaining of breast pain and their doctors prescribed ultrasound and mammography, were studied. Data from mammography and ultrasound outcome was compared with data of fine needle aspiration (FNA) and the sensitivity, specificity, positive predictive value and negative predictive value of these procedures were measured.Finding: Only three cases were diagnosed breast cancer patients with FNA. In this study, mammography sensitivity and specificity were respectively 100% and 98.2% and ultrasound sensitivity and specificity were 67% and 97.2% respectively.Conclusion: The results obtained from this study indicate that in strong suspicious cases of breast cancer, ultrasound and mammography are not appropriate procedures and an alternative to FNA. In these cases biopsy is essential.
