بررسی تأثیر جراحی تعویض مفصل زانو در کیفیت زندگی بیماران 70-60 ساله‌ی مبتلا به استئوآرتریت شدید زانو

چکیده

مقدمه:تعویض مفصل زانو یک روش مؤثر و معمول در جراحی استئوآرتریت شدید زانو می‌باشد. هدف مطالعه‌ی حاضر، بررسی کیفیت زندگی بیماران 70-60 ساله‌ی مبتلا به استئوآرتریت شدید پس از جراحی تعویض مفصل زانو بود.روش‌ها:در این کارآزمایی بالینی غیرتصادفی، کیفیت زندگی با استفاده از فرم کوتاه بررسی کیفیت زندگی (Short form health survey 36 یا SF-36) در 56 بیمار مبتلا به استئوآرتریت شدید که در یک برنامه‌ی زمان‌بندی در انتظار جراحی بودند، در یک سال پی‌گیری بررسی گردید. همچنین شدت درد با استفاده از مقیاس بصری ارزیابی شد.یافته‌ها: از 56 بیمار، 50 بیمار به آنالیز وارد شدند و 6 بیمار (3 بیمار فوت و 3 بیمار به دلیل عدم تمایل به ادامه) از مطالعه حذف گردیدند. میانگین سنی بیماران 21/3 ± 36/66 سال بود و 68 درصد آن‌ها زن بودند. میانگین نمره‌ی  SF-36در بیماران بعد از عمل جراحی به طور معنی‌داری نسبت به قبل افزایش یافت (به ترتیب 85/6 ± 61/53 در مقابل 71/5 ± 21/56، 041/0 > P). نمره‌ی پرسش‌نامه‌ی SF-36 در ابعاد عملکردجسمی، محدودیت جسمی، مشکلات روحی، نشاط، درد جسمی و سلامت عمومی به طور معنی‌داری بعد از جراحی بهبود یافت (05/0 > P) و بهبودی در دو بعد سلامت روان و عملکرداجتماعی معنی‌دار نبود (05/0 < P). کاهش درد بعد از جراحی در مقایسه با قبل از جراحی معنی‌دار بود (0001/0 > P).نتیجه‌گیری:مطالعه‌ی حاضر نشان داد کیفیت زندگی و شدت درد در بیماران مبتلا به استئوآرتریت شدید پس از جراحی تعویض مفصل در دوره‌ی پی‌گیری 12 ماهه بهبود یافته است.

Effects of Total Knee Replacement on Quality of Life in 60-70 Year-Old Patients with Advanced Knee Osteoarthritis

Background: Total knee replacement is an effective and common surgical procedure to relieve advanced knee osteoarthritis. The objective of this study was to assess quality of life after total joint replacement in 60-70 year-old patients with advanced knee osteoarthritis.Methods: This nonrandomized clinical trial included 56 patients with advanced knee osteoarthritis who were scheduled for total knee replacement. The quality of life among patients was assessed using the short form health survey (SF-36) during a one-year follow-up period. Additionally, pain score in patients was measured by a visual analogue scale (VAS).Findings: Since 3 patients died and 3 refused to participate, 50 patients were evaluated. Mean age of patients was 66.36 ± 3.21 years and 68% were female. Mean SF-36 score significantly improved after surgery compared with before surgery (53.61 ± 6.85 vs. 56.21 ± 5.71; P = 0.041).  In six dimensions (physical functioning, limitations due to physical health, limitations due to emotional problems, fatigue, pain and general health), SF-36 scores significantly improved after surgery (P < 0.05). However, the improvements in two dimensions (emotional well-being and social functioning) were not statistically significant (P > 0.05). Pain was also significantly reduced after surgery (P < 0.0001).. Conclusion: The present study showed that quality of life and pain improved after total knee replacement in patients with advanced knee osteoarthritis in a 12-month follow-up period.
