بررسی ارتباط سن حاملگی نوزادان، نوع زایمان و استحکام خانواده با ظهور سندرم‌های اختلال دفع در کودکان

چکیده

مقدمه: اغلب کودکان در سن 5-3 سالگی توانایی کنترل ارادی ادرار کردن و دفع مدفوع را به ‌دست می‌آورند که این زمان می‌تواند تحت تأثیر فاکتورهای متعددی قرار گیرد. اختلالات دفع ادرار و مدفوع(Dysfunctional voiding)  در کودکان طیف وسیعی از علایم کلینیکی را شامل می‌شود که به تازگی تحت عنوان «سندرم‌های اختلال دفع» (DES یا Dysfunctional elimination syndrome) طبقه بندی می‌شود.هدف از مطالعه‌ی حاضر، بررسی ارتباط زمان، نوع زایمان و با هم یا بدون هم بودن والدین با وقوع DES در کودکان بوده است؛ چرا که این فاکتورها ممکن است بتوانند پیش‌بینی کننده‌ی شانس وقوع DES در کودکان باشند.روش‌ها: به منظور دستیابی به هدف فوق 544 نفر (272 دختر و 272 پسر) از کودکان دبستانی 10 مدرسه‌ی عادی در شهر تهران انتخاب و با استفاده از پرسش‌نامه‌ی PLUTSS (Pediatric lower urinary tract scoring system) و یک پرسش‌نامه‌ی دموگرافیک حاوی فاکتورهای مورد بررسی در این مطالعه توسط والدین ارزیابی شدند. سپس بر اساس سیستم نمره دهی PLUTSS، کودکان مبتلا به DES شناسایی شدند و فاکتورهای مورد نظر مطالعه و پرسش‌نامه‌ی دموگرافیک مورد بررسی آماری قرار گرفت.یافته‌ها: میان ابتلا به DES در کودکان و زمان زایمان (ترم یا پره ترم بودن)، نوع زایمان (سزارین یا طبیعی) و با هم یا جدا از هم بودن والدین ارتباط معنی‌داری وجود نداشت.نتیجه‌گیری: وقوع DES در کودکان ارتباطی با زمان، نوع زایمان و با هم یا جدا از هم بودن والدین ندارد.

Background: Most of the 3-5 years old children can control urination and defecation; this time can be influenced by several factors. Voiding dysfunction in children encompasses a wide spectrum of clinical entities, recently classified collectively as dysfunctional elimination syndromes (DES). The aim of this study was to identify some factors which may be associated with dysfunctional elimination syndromes in children.Methods: We assessed 544 children 7 to 10 years old (mean age 8.68 ± 1.48 years) randomly selected from 10 elementary schools. All children and parents were requested to fill pediatric lower urinary tract scoring system (PLUTSS) and demographic questionnaire related to symptoms of dysfunctional elimination syndromes and some other factors. After statistical analysis, questions from the initial form were selected to form a final scale.Finding: Neither neonatal gestational age and kind of delivery nor family (parents) living together was associated with dysfunctional elimination syndromes in childre.Conclusion: Dysfunctional elimination syndromes do not belong to neonatal gestational age, kind of delivery and family living together.
